It has been known that immobilization of patient caused by chronic illness, orthopedic or neurological conditions can be one of etiological factorof urinary stone The authors have recently experienced a case of renal sand filling the pelviwalycea1 system simulating renal staghorn calculus which wasspontaneously flushed out following excessive fluid intake and forced mobilization. This patient had stones in the contralateral kidney and ureter.

4.Clinical Study of 55 Seizure Patients.

The authors had studied 55 cases with clinical seizure who were regularly treated with appropriate anticonvulsants over 6 months. In their distribution of age, teens are most freguent and their twenties followed. Males are more affected than female in the ratio of 3 to 2. Seizure may occur at any age; but our study shows that it is commoner in the first to third decades of life, about 72% of all cases, commonest in teens. The figures for 55 cases collected by us are : generalized type, 78% ; partial type, 22% ; 60% of all cases 33 are ldiopathic group, who had no demonstrable underlying neurologic abnormality. The major part of all idiopathic cases had their attacks of seizure before 30 years of age and; those of symptomatic cases are rather late over age of 30, and their figures are; cerebrovascular disorder, 6 cases; brain tumor, 5 cases; cerebral cysticercosis, 4 cases. 51 cases of all patients had evident abnormalities on their EEG. According to both their type of seizure and EEG findings, their drug was determined and, if need be, second drug was added. Freguently used drugs were dilantin, phenobarbital & Valproic acid etc. Mean therapeutic serum levels of anticonvulsants are: dilantin, 8.3 mg/L phenobarbital, 12.4 mg/L; which levels are less than those in U. S. A. In our study, the reponse to anticonvulsants were mostly good in about 2 years.

5.Two Cases of Inflammatory Linear Verrucose Epidermal Nevus.

Inflammatory linear verrucose epidermal nevus(ILVEN) is characterized by an early age of onset, predominance in females, pruritus, unresponsiveness to treatment, and a distinctive inflammatory and psoriasiform histologic appearance. We present two cases of typical ILVEN. One of them was a 16-year-old male, who had had a mild pruritic linear verrucose patch involving the right arm which grew slowly for about 8 years. The other case was a 14-year-old girl whose lesion appeared at the age of 13. This lesion was localized on the right posterior thigh and showed linear distribution and verrucose appearance. Histopathological findings of two cases showed hyperkeratosis with foci of parakeratosis, moderate to marked acanthosis, elongation of the rete ridges, and occasional slight spongiosis, and chronic inflammatory infiltration in the dermis.

6.A Study on the Changes in the Lumbar Lordosis, Lumbosacral and Sacral Inclination Angle during Aging.

Three parameters (lumbar lordosis, lumbosacral and sacral inclination angle) were measured on 512 normal plain lateral radiographs of lumbosacral spine and the patterns of change during aging were studied. The following results were obtained. 1. The lumbar lordosis angle showed a tendency to increase with age. But a brief decrease was observed after the 6th decade in both sexes, between the 2(nd) and 3(rd) decades in males, and between the 3(rd) and 4(th) decades in females. The lumbar spines of females were generally more lordotic than those of males. 2. The lumbosacral angle appeared to increase with age. The angle in males showed a decline between the 2nd and 4th decades while the angle in females decreased after the 6th decade. Males usually had greater angles than females. 3. The sacral inclination angle continued to increase until the 6th decade, after which a decrease was noted. The angle in females was generally greater than that in males. These results showed that all three angles varied with age, and the pattern of change differed between male and female.

7.A Giant Squamous Cell Carcinoma Arising in the Lesion of Discoid Lupus Erythematosus.

In the past, irritant or radiation therapy have been used to treat discoid lupus erythematosus, therefore malignant and proriferative change in the lesion of discoid lupus erythematosus were not uncommon. In recent year, however, because the preferred treatment of discoid lups erythematosus consisted of steroids and antimalarials, the association with squamous cell carcinoma has been extremely rare. We present a case of giant squamous cell carcinoma arising in the discoid lupus erythematosus in 39 year-old women. She had had a 20 year history of facial discoid lupus erythematoseus. In September of 1976, a thumb tip-sized tumor mass found on the right cheek and the maas grew into the adult palm size rapidly. The tumor show ulceration, serosanguinous discharge, easy bleeding, and foul oder. Histopathologically, the specimen obtained from plaques of the left cheek showed hyperkeratosis with plugging, atrophy of the stratum malpighii, a patchy, chiefly lymphoid cell infiltrate with a tendency to arrangement about the cutaneous appendages, hydropic degeneration of the basal cells, edema and vasodilatation in the dermis. The specimen obtained from tumor mass of the right cheek showed many nests consisted of well differentiated squamous cells, keratin pearls.

8.A Case of Wohlfart-Kugelberg-Welander Syndrome in Korean Sister.

Wohlfart et al and then Kugelberg and Welander were concerned with patients suffering from proximal limb weakness that resembled muscular dystrophy, but the muscle biopsy and EMG indicated neuronal disease. However, several reports had determined considerable variation according to age of onset, distribution of weakness and rate of progression. Most of familial cases fit an autosomal recessive pattern but dominant and X-linked forms were recorded. The syndrome is recognized by clinical criteria and laboratory abnormalities only that reflect neuronal degeneration. It merges with Werdnig-Hoffmann disease in infant and with amyotrophic lateral sclerosis in adult and with Wohlfart-Kugelberg-Welander syndrome in adolescent. In this report we respresent a case which is probably Wohlfart-Kugelberg-Welander syndrome in Korean sister.

9.Use of microplate on flxation of orbital rim fracture.

The orbitozygomatic area occupies a key anatomic position in midface, is prone to injury, and plays a prominent role in facial aesthetics. So the facial appearance including orbital shape can be altered by complications of orbitozygomatic fractures. Most possible initial complications include blindness, hyphema, retinal detatchment, and paralysis or entrapment of extraocular muscles. Long term sequelaes include infraorbital nerve dysfunction, loss of malar projection, enophthalmos, and dystopia. Accurate anatomic reduction and rigid fixation is essential for management of orbitozygomatic fractures to minimize those late sequelaes. Conventional fixation devices to fix displaced fracture of facial bone are interosseous wire and miniplate. But interosseous wirings are unstable for primary bone healing and time consuming. Miniplates have great deal in rigid fixation but their high profile and palpability are the main complaints in many patients, especially in orbital rim area. In this article, we reviewed the 30 cases of zygomamaxilla complex fractures with orbital rim fracture fixed with microplates, and discussed the stability of microplate and superiorities in final aesthetics result. The use of microplates in these area permits enough stability of fracture segment with ease of procedures and superiorities in final results without any palpability.

10.3 Case of Shy-Drager Syndrome.

Shy-Drager syndrome is a multisystem degenerative disorder of the central nervous system in which progressive autonomic failure such as orthostatic hypotension, urinary or fecal incontinence, impotence and anhidrosis is a main feature. The nonautonomic neurological abnormalities include parkinsonia, cerebellar and pyramidal signs. In 1984 we met 3 such cases at Seoul National University Hospital and present them here with a brief review of literatures.
