1.Test-Retest Differences and Assistive Function in Detecting Conductive Hearing Loss of lmpedance Audiometry.

lmpedance audiometry requires physical modifications during the test, which might influence retest data. Therefore, in Order to interprete retest data meaningfully, the range of variation should be identified in each measure of impedence audiometry. The present study obtained data on the retest variation of peak pressure, acoustic reflex threshold, static compliance and earcanal volume in impedance audiometry. ln addition, the authors wanted to know whether or not impedance data would assist otolaryngologists in the detection of conductive hearing impairment. The variation of the retest data was not clinically nor statistically significant in the measurement except for those of ear canal volume. The data on ear canal volume also suggested that the ear canal increases in size during the teenage period and that male ear canals are larger than those of females in ears over 20 years of age. The impedance data assisted the otolaryngologist in the detection of conductive hearing impairment

3.A Case of Malignant Mixed Mullerian Tumor of Uterus Developed After Pelvic Irradiation.

Mixed mullerian tumors are uncommon tumor and highly malignant containing epithelial and mesenchymal component. Most of these tumors arise spontaneously in the sixth to eight decades of life, but a certain proportion are known to occur in women who have received pelvic irradiation from five to 30 years before for benign or malignant disease. This apparent association has led to the belief that pelvic irradiation somewhat increase the neoplastic potential of the uterine corpus, although the precise role of irradiation in the genesis of uterine neoplasia remain uncertain. We experienced one case of malignant mixed mullerian tumor of uterus developed after pelvic irradiation. So we report the case with a brief review of literature.

4.Detection of IgG antibodies with immunofluorescent antibody technique in human trichomoniasis.

The indirect fluorescent antibody(IFA) test was used to detect serum IgG and IgM antibodies to Trichomonas vaginalis in 31 vaginal trichomoniasis, 7 candidiasis and in 20 non-infected healthy wonem with antigen prepared from axenic culture of Trichomonas vaginalis isolated from vulvovaginitis patient. The results were as follows: In 31 vaginal trichomoniasis the positive reactions of IgG antibody were 27 in the 1/8 dilution or higher and 4 in the 1/4 dilution whereas in healthy women the reaction showed signigicantly low as in the 1/4 dilution of below. The sensitivity and specificity of IFA test for IgG antibody to trichomonad antigen in this study were 87.1% and 100%, respectively. No significant difference of IgM antibody levels between vaginal trichomoniasis and healthy women was observed. No relation between the levels of IgG and IgM antibodies to trichomonad antigen by IFA test was observed. No relation between the time lapse and the level of serum IgG antibodies in IFA test of vaginal trichomoniasis was regarded. In conclusion the present study suggests that IFA test in trichomoniasis could be a useful tool for detection of anti-trichomonad IgG antibodies and applicable as an immundiagnostic method.

5.Development of Critical Pathway for Laparoscopic Assisted Vaginal Hysterectomy by CPA program.

OBJECTIVE: The aim of this study was to develop critical pathway for post operation care of laparoscopic assisted vaginal hysterectomy with time and cost effectiveness using a clinical pathway assistant program (MDCPA(TM), MDware Co.) METHODS: Procedure was done with the following steps; establishment of conceptual framework, development of preliminary critical pathway using CPA program, expert validity test, and confirmation of final critical pathway. RESULTS: Preliminary critical pathway was extracted by CPA program, and this result was verified by expert validity test. The process of data collection and analysis to develop the preliminary critical pathway was accomplished automatically by CPA program. The patients' hospitalization period in the aspect of time and cost effectiveness was confirmed as 6 days, and each item of assessment, test, medication, treatment, diet, activity, and teaching were finalized. CONCLUSION: The CPA program can automate the complicated process, which contains collecting and cleaning the large size EMR data, classifying and confirming the items in critical pathway, so that the time and cost to develop a critical pathway can be reduced. This program is expected to be used for development of critical pathway in a variety of clinical settings in the aspect of time and cost effectiveness.

6.Sclerosing peritonitis mimicking ovarian carcinoma: Associated with ovarian teratoma.

Sclerosing peritonitis is an unusual fibrosing condition predominantly involving the omentum and simulating carcinoma. The presenting signs and symptoms, imaging examination and cancer antigen 125 (CA-125) status in sclerosing peritonitis sometimes resemble those of ovarian cancer. Thus, the possibility of sclerosing peritonitis should be considered in the differential diagnosis of ovarian carcinoma. It may occur idiopathically and secondary to chronic peritoneal dialysis, the use of peritoneovenous shunt, practolol therapy, or in association with ovarian tumors such as ovarian teratoma. We report a case of peritonitis initially suspected as ovarian carcinoma but diagnosed as sclerosing peritonitis associated with teratoma.

7.Psychometric Properties of Korean Version of Drug Attitude Inventory(KDAI-10).

OBJECTIVE: The aim of this study was to test the psychometric properties of Korean version of 10 items of Drug Attitude Inventory (KDAI-10). METHODS: A total of 157 stabilized schizophrenic patients were entered into the study after completion of screening tests. They completed the KDAI-10. And then they were interviewed with various scales such as Subjective Response Scale (SRS), Positive and Negative Syndrome Scale (PANSS), Extrapyramidal Side Effects Rating Scale (ESRS), and Global Assessment of Functioning (GAF). Correlations of KDAI-10 with these scales were tested. RESULTS: The Cronbach's alpha used to measure the internal consistency of the scale, was high (0.83). Principal component analysis with varimax rotation produced 2 factors, which explained 50.37% of the total variances. The First factor including 6 items was the positive subjective feelings and the second factor including 4 items was the negative subjective feelings, which were identical to the original authors'. Individual item correlations with total scores were all statistically significant (r=0.48-0.72, p<.001). And correlations of KDAI-10 with SRS, PANSS, ESRS and GAF showed that they were significantly correlated. CONCLUSION: Although the results were not quite satisfactory, KDAI-10 was demonstrated to have good reliability and construct validity.

8.Difference in Volume Perception of Cooked White Rice according to Size and Color of Rice Bowl in Normal and Obese Women.

To examine the effect of obesity on volume perception according to size and color of rice bowl, we divided female college students into a normal weight group (<30% fat mass, n=100) and obese group (> or =30% fat mass, n=83) and then measured perceived volume of rice bowls of various sizes (general size; 350 ml vs. small size; 188 ml) and color (yellow, white, blue, and black) containing the same amount of cooked white rice (210 g). Normal weight group perceived that the general rice bowl contained significantly more cooked white rice compared to the small rice bowl. In contrast, the obese group perceived that the general rice bowl contained significantly less cooked white rice than the small rice bowl. The estimated variance in perceived volume of both bowls was significantly bigger in the obese group compared to the normal group. There were no differences in perceived volume among any of the subjects (both normal and obese groups) according to rice bowl color. However, the estimated variance in perceived volume in the obese group was significantly larger than that in the normal group for all of the rice bowls. In conclusion, rice bowl size and color might affect volume perception, and volume perception in obese people may be different from that of normal weight people.

9.An Experimental Study on the Immune Responses to Regenerating Allografts in the Rat.

Partial liver transplantation from a living donor or by splitting the liver was developed to expand the donor pool. Sometimes the small-for-size liver is transplanted into larger recipient and it regenerates shortly to ideal volume for the recipient. The relationship between regeneration and rejection-intensity has not defined clearly yet. In this study the 30% partial liver of DA (Dark-Agauti) rats were transplanted heterotopically to Lewis rats by microsurgical technique. End-to-side cavocaval and end-to-end portoportal (or portomesenteric) anastomoses were performed but the artery and the bile duct were not reconstructed. In regenerating group (R), total portal blood of the recipient was diverted to the graft. In non-regenerating group (N), the superior mesenteric vein below the confluence of the splenic vein was anstomosed to the portal vein of the recipient. Both groups were treated with tacrolimus (FK-506 2 mg/kg/d) and compared with non-treated groups. Transplantations were successful with minimal operative mortality. The animals could survive for the studying period in both groups. When compared on day 3, day 5, and day 7, histologic examination revealed no significant differences of cellular infiltration between two groups. In tacrolimus-treated groups, the cellular infiltration was markedly decreased and there were no significant differences in cellular infiltration between regenerating group treated with tarcrolimus (R-F) and non-regenerating group treated with tarcrolimus (N-F). These findings suggest that immune responses to the allograft are not significantly influenced by the hepatic regeneration process in small-for-size graft. Further immunologic and molecular biologic investigation may be needed.
