1.Clinical Trials of Demacre on Varioys Dermatoses.

Demacre', a new topical steroid cream containing 0.025% beclomethasone dipropionate, was used in several kinds of dermatoses. Total 35 patients including ll cases of contact dermatitis, 5 of atopic dermatitis, 3 of hand eczema, 3 of seborrheic dermatitis, 2 of neurodermatitis, 2 of nummular eczema, 2 of autosensitization dermatitis, 1 of chronic eczema, 3 of psoriasis, 1 of insect bite, 1 of miliaria rubra, and 1 of intertrigo were treated by Demacre'. Among these patients, we observed excellent effects in 4 patients (11%), good effects in 22 patients (63%), and fair effects in 3 patients (9%). Namely, among these 35 patients, 26 patients (74%) revealed excellent to good therapeutic effects on their skin conditions with Demacre'. No untoward side effects were noted in all subjects treated with Demacre.

2.A Comparison of Laparoscopic Cholecystectomy with Open Cholecystectomy in a Korean Hospital.

Laparoscopic cholecystectomy was introduced into Korea in 1990 and has been rapidly replacing open cholecystectomy when the indications were met. In this study a medical utilization and technology was assessed on the selected hospitalized patients with cholelithiasis who underwent open or laparoscopic cholecystectomy from April 1, 1991 to March 31, 1994. The results are as follows. Despite the low reimbursement rate by the health insurance, the number of laparoscopic cases have been steadily increased. The post-operative days before health insurance coverage were significantly shortened from 8.4 days to 4.6 days. The preoperative days before health insurance coverage were significantly shorted from 8.4 days to 4.0 days. The total length-of-stays in the hospital were also significantly shortened from 15.2 days to 10.7 and 9.8 days in laparoscopic cholecystectomy. The laparoscopic cholecystectomy showed low expenses in all aspects expect the average hospital charges per day. For the hospital to have cost containment, it is more effective if length-of-stay is shorter because of high daily inpatient hospital charge. The laparoscopic cholecystectomy also showed shortened anesthesia time and operation time compared with open cholecystectomy that were statistically significant. The mean anesthesia and operation time for open cholecystectomy were 113.2 and 90.2 minutes but those of laparoscopic cholecystectomy were 105.7 and 68.6 minutes. According to this study the laparoscopic cholecystectomy has reduced the medical expenditure and we recommend this procedure over open cholecystectomy. The further discussion on the different morbidity rate between two types of procedure is essential in providing quality medical care, and to educate specialist.

4.Scanning Electron Microscopic Observation on the Microvasculature in Anterior Segment of the Rabbit Eye Injected with Endothelin-1.

Using Mercox CL-2B, intravascular casting was performed follwoing intracameral injection with 2.0microgram/microliter of Endothelin-1(ET-1) in the rabbit eye. One hour after intracameral injection with ET-1 caused severe focal andsegmental constriction of major arterial circle(MAC). Obstruction of the small branch of MAC caused localized filling defect and decreased number of capillaries in the vascular cast. The diameters of vascular cast were reduced at about 22.2% in proximal MAC, 22.4% in distal MAC and 21.8% in first branch of MAC. This results were suggested that ET-1 may play a role in the regulation of blood flow in the anterior segment vasculature of the rabbit.

6.The Effect of 5HT(3) Receptor Agonist on Intraocular Pressure and Pupil Diameter in Rabbits.

PURPOSE: The effects of 5HT3 receptor agonist and antagonist on rabbit intraocular pressure (IOP) and pupil size were evaluated. METHODS: A 5HT3 receptor agonist, I-Phenylbiguanide (PBG, 1%) and a 5HT3 receptor antagonist, 3-Tropanyl-indole-carboxylate methiodide (ICS-205,930) were applied topically to the rabbit eye, and intraocular pressure and pupil diameter were checked with a Tono-PenTM XL and a ruler. RESULTS: Topical application of 1% PBG significantly increased IOP by 4.6+/-1.2 mmHg (p<0.001) over a period of up to 2 hours, the IOP maximum reached at 1 hour and caused pupil dilatation by 3.3+/-0.3mm (p<0.001) over a period of up to 5 hours, the dilation maximum reached at 2 hour. Topical preadministration of 1% ICS-205,930 prevented a rise in IOP but did not counteract on the pupil dilatation brought about by 5HT3 receptor agonist. CONCLUSIONS: These findings indicate that topically applied 5HT3 receptor agonist could raise IOP and dilate pupil.

8.A Study on the Factors Affecting Quality of Life in Children and Adolescents with Epilepsy and Their Families

OBJECTIVES: The aim of this study was to investigate the impact of clinical and psychological factors on the quality of life of children and adolescents with epilepsy and their families.METHODS: Children and adolescents with epilepsy and their families (n = 63, age range = 6–17 years) completed questionnaires on epilepsy-related variables, quality of life, children's depressive symptoms, children's anxiety, children's behavioral problems, children's attention problems, parental stress, and parental anxiety. Stepwise regression analysis was performed to determine the significant predictive variables that affect quality of life.RESULTS: In the correlational analysis, children's attention problems (r = 0.363, p = 0.004), parental anxiety (r = 0.377, p = 0.003), parental stress (r = 0.564, p < 0.001), and children's behavioral problems (r = 0.503, p < 0.001) showed a significant correlation with quality of life. Parental stress (β = 0.415, p = 0.001, adjusted R² = 0.345) and children's behavioral problems (β = 0.285, p = 0.02, adjusted R² = 0.345) were significantly related to the quality of life.CONCLUSIONS: Clinicians should pay attention to parental stress and children's behavioral problems, which affect quality of life in families with pediatric epilepsy.

9.Health Care Workers' Nasal Carriage and Outbreak Control of Epidemic Methicillin-resistant Staphylococcus aureus.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is the major pathogen of nosocomial infections. There are many reports that MRSA nasal cariers play a major role in the transmission of MRSA. We studied to assess the nasal carriage rates, therapeutic effects of nasal carriers and control of MRSA outbreak in a tertiary, acute and educational hospital. METHODS: From 1990 to 1997, eight MRSA epidemics were detected and investigated for outbreak control. We surveyed the MRSA colonization of anterior nares and hands of health care workers (HCW). MRSA was identified by staphylococcal broth, mannitol-salt- agar, Muller-Hinton-oxacillin agar. To identify the permanent carriers in the HCWs, the nasal swab was done weekly for 3 weeks. Two percent povidone iodine ointment or 2% mupirocin ointment was applied topically to treat the permanent MRSA nasal carriers. RESULTS: Of eight MRSA outbreaks, five epidemics occurred in the intensive care unit, all of them occurred in surgical departments and five of them occurred in winter seasons. For eight MRSA outbreaks, 351 HCWs were surveyed. The rates of transient carriers was 10.8% (38/351) and the rates of that were 9.0% (10/111) in medical doctors (MD), 12.5% (24/192) in registered nurses (RN) and 11.4% (4/35) in aid nurses (P=0.470). The rate of permanent carrier in doctors was 1.8%, and that in RNs was 3.7% (P=0.490). After topical therapy with 2% povidone iodine ointment or 2% mupirocin ointment for permanent carriers, MRSA was not identified for 2 months follow up. CONCLUSION: In MRSA epidemics, the rate of the transient carrier rate of MRSA in HCWs was 10.8% and the rate of the permanent carrier was 2.6%. The difference of carrier rates in HCWs was not significant statistically. The therapy for the permanent nasal carriers in HCWs with two percent povidone iodine ointment or 2% mupirocin ointment was very effective. And MRSA outbreaks were ended and controlled for 6months follow up.

10.The Change of Eyeball Contour and Factors Affecting Corneal Flap using Automated Microkeratome in LASIK.

PURPOSE: To evaluated the effect of suction ring on eyeball contour and its influence on the corneal flap made in LASIK(laser in situ keratomileusis). METHODS: We measured corneal thickness and axial length of 47 eyes, before and during application of the suction ring, in those which had undergone LASIK. The diameter and thickness of corneal flap made with Automated Corneal Shaper(R) were also measured in operation room. RESULTS: When applying suction ring while making a corneal flap, corneal thickness and axial length were decreased significantly(p<0.05), and the diameter of the flap was decreased as preoperative corneal curvature decreased(p<0.05). CONCLUSIONS: Careful attention is needed not to give an eyeball unnecessary pressure which may result in intraocular damage or flap complications such as free cap especially when their corneal curvature is too flat.
