2.A Giant Gartner Duct Cyst Originating from the Uterine Cervix and Adjacent Myometrium: A Case Report.

Gartner duct cysts are relatively common congenital cysts in the anterolateral wall of the vagina. Most are small (less than 2 cm) and asymptomatic, but larger cysts may cause problems. We report a rare case of a giant Gartner duct cyst (7 cm) originating from the right lateral portion of the uterine cervix and adjacent myometrium. It appeared as an exophytic multiseptated cystic mass containing different density fluids on CT.

3.Current Medical Therapy for Uterine Leiomyomas.

Uterine leiomyomas are benign tumors arising from the myometrium and largely prevalent in the woman's reproductive years. The majority of women with leiomyomas either remain asymptomatic or develop symptoms gradually over time. When patients are symptomatic, the nature of their complaints is often attributable to the number, size, and/or location of their fibroids. Depending on a patient's symptomatology and reproductive plans, treatment options include expectant management, medical management (hormonal and non-hormonal), or surgical management (myomectomy or hysterectomy).

Background:
The Lead Vessel and Infitrating Vessels are findings in endometrial cancer that are well visualized by three-dimensional power Doppler angiography. Vessel diameter and length may be utilized as markers for deep myometrial and or cervical involvement.
Objective:
To determine the accuracy of Lead vessel and Infiltrating vessel dimensions in assessing the depth of myometrial invasion in endometrial cancer and its cervical involvement.
Participants and Methods
All women histopathologically diagnosed with endometrial cancer, undergoing surgical staging, with
informed consent were included. Sonography using GE Voluson S8 system for describing the uterus and endometrial thickness were followed by a 3D power Doppler Angiography to analyze the depth of myometrial invasion and presence or absence of cervical involvement. Vessel diameter and length in transverse and sagittal plane were measured by calipers. Predictive values and operating characteristics (sensitivity, specificity, positive and negative predictive values) were computed.

5.The association of serum CA-125 level and severity of dysmenorrhea with lesion depth in adenomyosis.

OBJECTIVE: To evaluate the association of level of CA-125 and severity of dysmenorrhea with lesion depth in adenomyosis. METHODS: Sixty-eight women who had undergone hysterectomy and were found to have pure adenomyosis on histopathologic examination were reviewed retrospectively. Specimens were stratified according to the degree of adenomyosis penetration into 4 groups: group A consisted of specimens with adenomyosis penetration into myometrium of up to 25%; group B, 26-50%; group C, 51-75%, and group D, >75%. All women were submitted to serum determination of CA-125. Severity of dysmenorrhea was assessed by means of a modification of 10-point verbal rating scale. RESULTS: Each group was not correlated with severity of dysmenorrhea (p=0.7394 Fisher's exact test). The means of serum CA-125 levels was increased corresponding to depth of adenomyosis (p=0.0441 ANOVA test). CONCLUSION: In this study, serum CA-125 level is associated with the depth lesion and severity of dysmenorrhea is not. Therefore, the level of serum CA-125 might be useful indicator in management of adenomyosis before surgery.

7.A case of focal uterine cystic adenomyoma.

The clinical and pathological features of an apparently unique case of an adenomyoitc cyst of the uterus are reported. The cyst was located within the myometrium of a 30-year-old woman suffering from vaginal bleeding for 6 months. Saline infusion sonohysterography revealed uterine cyst. After excision of the cyst, patient's symptoms improved. On histological examination, the cyst most closely resembled an adenomyotic cyst.

8.A Case of Post-Cesarean Endomyometritis Associated with Myometrial Gas Formation.

Postpartum endomyometritis is an important cause of maternal morbidity, especially after cesarean delivery. The management of postpartum endomyometritis is primarily medical with surgical intervention only occasionally required. But the gas forming abscess rarely happens, making the medical treatment difficult and needing emergency hysterectomy. We present a case of post-cesarean delivery endomyometritis complicated by intrauterine gas formation penetrating myometrium, which could not be managed by medical treatment, so finally needed emergency hysterectomy.

To elucidate the endocrine mechanism of human parturition, the expression of c-Jun and c-Fos mRNA were examined in relation to estrogen receptor (ER) and progesterone receptor (PR) in human myometrium. c-Jun mRNA was detected in all myometrial tissues (n=5) during labor but not before labor (n=5) and in oxytocin-resistant postterm pregnancy (n=3). c-Fos mRNA was detected in only one myometrial tissue from a woman in labor. The distribution and intensity of immunostaining for ER and PR were semiquantitatively scored. During the late pregnancies, no significant difference was seen in the receptor scores for myometrial ER and PR between the patients who experienced labor and those who did not. Receptor scores for ER and PR were significantly lower in postterm pregnancy than in late pregnancy, regardless of the labor status. These data suggest that there are no changes in ER and PR in human myometrium during parturition. On the other hand, postterm pregnancy is associated with low ER and PR. c-Jun, induced during labor without changes in ER and PR, may play a role as a signaling mechanism in human myometrium.

10.Adenomyosis of the Uterus: Recognition of a Characteristic Enhancement Pattern on Contrast-enhanced Dynamic MRI.

PURPOSE: To analyze the enhancement patterns of adenomyosis and evaluate the usefulness of dynamic MR imaging in detecting adenomyosis. MATERIALS AND METHODS: Dynamic MR imaging finding in 46 women with adenomyosis were analyzed retrospectively. Dynamic MR imaging was obtained in each patient. Fifteen consecutive FLASH images were obtained before, immediately after and 15, 30, 45, 90 second and 5 minute after bolus injection of Gd-DOTA. Signal intensities of adenomyosis and outer myometrium and standard deviations of image noise were measured using an electronic cursor. Contrast-to-noise ratio (C/N) of the lesion to outer myometrium were calculated as follows ; (signal intensity of outer myometrium-signal intensity of adenomyosis)/(standard deviation) of noise signal intensity. RESULTS: The mean signal intensity of adenomyosis was higher than surrounding outer myometrium before and immediately after contrast injection. On 15, 30, 45, 90 second, and 5 minute after injection the signal intensity of adenomyosis was lower than that of surrounding outer myometrium (p < 0.05). The mean C/N values between adenomyosis and surrounding myometrium on precontrast scan, immediate postcontrast, and 15, 30, 45, 90 second and 5 minute delayed images were 3.20, 5.03, 6.14, 6.87, 7.13, 7.78, 38.59, respectively. On T2-weighted images, the mean C/N value between adenomyosis and surrounding myometrium was 16.7. Mean C/N value between adenomyosis and surrounding myometrium on delayed images was significantly higher than those of other dynamic & T2-weighted images(p<0.05). CONCLUSION: Dynamic MR imaging is very useful in the evaluation of the enhancement pattern and especially in detection of adenomyosis because the mean C/N value between adenomyosis and surrounding myometrium on delayed images are higher than that of T2-weighted images.
