2.Diagnosis and treatment of myocarditis.

Myocarditis is a focal or diffuse inflammatory process of the myocardium. Along with the deepening of research, there are new technological advances which are useful to the diagnosis and treatment of myocarditis. Cardiovascular magnetic resonance imaging has become a useful noninvasive imaging tool for diagnosis of myocarditis, but does not replace endomyocardial biopsy. The supportive treatment has remained the mainstay of therapy for patients with myocarditis, especially with fulminant myocarditis, including the comprehensive treatment of shock, heart failure, and arrhythmia. Due to the lack of large clinical data, diagnosis and treatment for myocarditis remains to be further explored.

This paper summarizes our experience of five cases of sick-sinus syndrome, hospitalized at Presbyterian Medical Center in Jeonju from January, 1980 to January, 1983. Diagnosis was made depending on clinical symptoms, physical examination, EKG and provocative test with atropine sulfate. The etiologies of 4 cases were as follows: coronary disease, hypertension, myocarditis, coronary disease with hypertension; but, the etiology of one case was not clear. Symptoms, including syncope, dizziness, and palpitation subsided after implanting the temporary pacemaker. A permanent pacemaker was implanted in all but one case, who had myocarditis.

9.A Case of Idiopathic Acute Myocarditis with Isolated Right Ventricular Failure.

A 68 year-old woman was admitted due to chest pain with dyspnea. Transthoracic echocardiography showed isolated right ventricular dysfunction. Coronary angiogram was normal. Cardiac enzyme was elevated persistently. Under the supposed diagnosis of acute idiopathic myocarditis, she was treated with supportive care but symptom and cardiac enzyme was not improved. After steroid pulse therapy, symptom was improved and cardiac enzyme was normalized. She was discharged with improvement of symptoms.

10.Hypersensitivity myocarditi caused by ceused by cafadroxil.

Hypersensitivity myocarditis is an inflammatory disease of the myocardium usually related to drug allergy. The clinical manifestation may be nonspecific, and the diagnosis is seldom suspected or established during the life. So in most of the reported patients, the diagnosis was made at autopsy. Although, retrospectively, more than 90% of the patients described have had clinically recognizable cardiac involvement before death, tole clinical suspicion of hypersensitivity myocarditis is rare. The presence of nonspecific cardiac finding in a patient receiving any drug associated with hypersensitivity should alert the clinician to the possibility of drug related myocarditis. We experienced a case of hypersensitivity myocarditis accompanying allergic skin lesion in a 58 years old man with a history of taking cefadroxil. Fortunately, the outcome was favorable, therefore we do not have histologic confirmation of the diagnosis.
