1.Effect of opening point method of ziwu liuzhu on myocardial ischemia in the patient of stroke.

OBJECTIVETo observe effect of opening point method of ziwu liuzhu on myocardial ischemia in the patient of stroke.
METHODSThirty cases of stroke with myocardial ischemia were treated with opening acupoint according to time of the combination of reinforcing mother point and reducing son point of the heart channel in nazi method of ziwu liuzhu, and 30 cases in the control group were treated with acupuncture at Guangming (GB 37). Changes of electrocardiogram before and after treatment were recorded.
RESULTSST-T segment raised significantly 30 min after treatment with ziwu liuzhu needling method as compared with the control group (P < 0.05), the treatment group being better than the control group.
CONCLUSIONExcessive and deficient changes of the qi and blood in the meridians and collaterals are correlated with time rhythm, and ziwu liuzhu needling method can increase clinical therapeutic effect.

4.Significance of ST Segment, R Wave, Q Wave and QRS Score for Assessing Myocardial Perfusion in Acute Myocardial Infarction.

BACKGROUND AND OBJECTIVES: The restoration of infarct-related coronary artery (IRA) patency in acute myocardial infarction (AMI) linked to a significant improvement in survival. Because of microvascular and cellular injury, patent IRA does not always represent successful reperfusion. With progress of myocardial ischemia, standard 12 lead ECG shows evolutional changes of ST-segment, R wave and Q wave. But their relations to myocardial perfusion were uncertain. METHODS: Total 41 patients of the first anterior wall AMI were enrolled and serial ECGs were taken to measure sum of ST-segment elevation (sigma ST), sum of Q wave (sigma Q), sum of R wave (sigma R), and QRS score (QRSs) proposed by Selvester in each patients before thrombolytic therapy (i), after coronary angiography at 90 minutes of thrombolytic therapy (a), and before discharge (d). Myocardial contrast echocardiography was performed within 10 days of AMI to estimate opacification score (OS) and opacification index (OI) in segments of LAD territory. We investigate the relation between evolution of ECG changes and perfusion status of infarcted myocardium. RESULTS: 1) There was no relation between OI and sigma ST, but sigma R and QRSs showed significant relation with OI before discharge (r=0.59, - 0.33, p<0.05, respectively), post thrombolytic therapy (r=0.51, - 0.61, p<0.05), and baseline ECG (r=0.53, - 0.51, p<0.05). 2) The number of segments with OS (0.5) showed no singificant relation to the degree of sigma ST and sigma Q, but number of segments with OS (0) showed singinficant relation to that of sigma R and QRSs (r of sigma Ri, sigma Ra, sigma Rd vs number of segments with OS (0)= - 0.59, - 0.66, - 0.43, p<0.05, QRSi, QRSa, QRSd vs number of segments with OS (0)=0.58, 0.58, 0.57, p<0.05). CONCLUSION: These findings suggest that the ECG changes of R wave and QRS scores could be useful markers of perfusion state in thrombolytic era.

5.Effect of spinal cord stimulation on myocardial ischemia/infarction.

Fatal arrhythmias, heart failure, and sudden cardiac death after myocardial ischemia/infarction are serious threats to human health. In recent years, studies have shown that spinal cord stimulation (SCS) can balance autonomic activity, inhibit myocardial structural remodeling, improve blood flow to ischemic myocardium, effectively reduce the incidence of arrhythmia, heart failure and sudden cardiac death after myocardial ischemia/infarction, but its specific mechanism has not yet been fully elucidated. The effect of SCS on cardiac function may be achieved by inhibiting neural remodeling, or by ameliorating structural remodeling and electrical remodeling. This article reviews the progress on the role and mechanism of SCS in myocardial ischemia/infarction.

6.Phytoestrogens in application prospect of treatment of myocardial ischemia/reperfusion injury.

Reperfusion is the most effective treatment for acute myocardial infarction, markedly reducing mortality and morbidity. Reperfusion however induces necrotic and apoptotic damages to cardiomyocytes, that were viable prior to reperfusion, a process called myocardial ischemia/reperfusion injury(MI/RI). Over the past 30 years, hundreds of experimental interventions (both pharmacologic and nonpharmacologic) have been reported to protect the ischemic myocardium in experimental animals; however, with the exception of early reperfusion, none has been translated into clinical practice. The population-based survey assessed men have about twice the total incidence of morbidity and mortality of women, and the sex gap in morbidity tends to diminish after age 45 years. So hormone replacement therapy (HRT) is given to treat the MI/RI, and lots of studies shows that the side effect is greater for estrogen, compared with phyestrogen. In this article, we review the important pathogenesis of myocardial ischemia reperfusion injury, the prevention and limitations of HRT. And we highlight the mechanism of phyestrogens treatment the MI/RI in experiment. The aim is to provide the theoretically new way of develop the safe and effective products for the researchers.

7.Study on effects of acupuncture at the heart meridian on gene expression pattern of heart in rats with acute myocardial ischemia.

OBJECTIVETo reveal the mechanism of acupuncture at the Heart meridian in treatment of myocardial ischemia from gene level.
METHODSA model of acute myocardial ischemia was made by ligation of left anterior descending branch of the coronary artery. Changes of gene expression pattern of the Lung meridian group, the Heart meridian group and the model group in the heart were compared.
RESULTSDifferential expression genes and expression sequence tags (ESTs) were 14 with signal log ratio > or = 1 and 20 with signal log ratio < or = -1, mainly included immunological and inflammatory reaction relative genes, cell signal transduction and transferrin relative genes, and others, in the Lung meridian group vs the model group. Differential expression genes and ESTs were 20 with signal log ratio > or = 1 and 70 with signal log ratio < or = -1, mainly included ion channel and transport protein relative genes, cell apoptosis and stress reaction protein relative genes, metabolism relative genes, cell signal transduction and transferrin relative genes, DNA's binding, transcription and transcriptional factor relative genes, immunological and inflammatory reaction relative genes, and others, in the Heart meridian group vs the model group.
CONCLUSIONIn number and types of differential expression genes and ESTs with signal log ratio > or = 1 or < or = -1, the changes in the Heart meridian group vs the model group were obviously different from those in the Lung meridian group vs the model group, indicating that the Heart meridian has relative specificity in protecting against acute myocardial ischemia.

10.Research status and prospects on adenosine receptor mechanism of acupuncture for myocardial ischemia.

The relationship between adenosine receptor (AdoR) and myocardial ischemia (MI), effect of acupuncture for MI and action mechanism of acupuncture improving MI by regulating AdoR are summarized. The existing researches have preliminarily reflected that the improvement of MI treated with acupuncture may be achieved by influencing the expression of AdoR. However, there are still some limitations, e.g. most of the research regimens are single-acupoint, the research results are not entirely consistent and the interaction of AdoRs are ignored, all these need to be further verified and supplemented.
