Atrial septal defect is one of the most common congenital heart diseases in adult, of which absolute causes is not confirmed, but explained by multifactorial inheritance. In Korea, there has been no case report of atrial septal defect in identical twins in spite of some reports of atrial septal defect in relatives and twins in other countries. We present a first case of atrial septal defect in identical twins whose mother had no infection or medication history during pregnancy.

Liposarcoma is the second most common soft tissue sarcoma next to malignant fibrous histiocytoma and occurs almost exclusively in adults. It is thought to originate from primitive mesenchymal cells rather than from mature adipose tissue. Most liposarcomas usually occur in the deep soft tissue of the extremities and the retroperitoneum, and rarely occur in the cutaneous or subcutaneous tissue. We report a case of an atypical lipomatous tumor in a 63-year-old male, who presented with an asymptomatic, subcutaneous mass on his right axilla.

Primitive neuroectodermal tumors (PNETs) are malignant, round-cell sarcomas which exhibit primitive neuronal differentiation. They usually arise in deep soft tissues, but occasionally bones and visceral organs are affected. In general, PNET presents as rapidly-growing, deeply located masses measuring 5~10 cm at their greatest diameter. Superficially located cases do occur but are rare. The typical PNET is composed of sheets or lobules of small round cells containing darkly-staining, round or oval nuclei. The cytoplasm is indistinct except in areas where the cells are more mature and elongated hair-like cytoplasmic extensions coalesce to form rosettes. We herein report a case of PNET that arose from subcutaneous tissue of the buttock in a 31-year-old woman.

BACKGROUND: Trichomycosis axillaris and pubis is a bacterial infection of the hair shaft. It is characterized by nodular thickening on the hair shaft, composed of colonies of aerobic Corynebacterium. Benzoyl peroxide and erythromycin topical application can be used successfully. OBJECTIVE: We performed this study to compare treatment efficacy of benzoyl peroxide and erythromycin in trichomycosis axillaris and pubis METHOD: The clinical study was made with 12 patients of trichomycosis axillaris and pubis. RESULT: Among 12 patients, 6 patients were treated by benzoyl peroxide topical application and the other by erythromycin. 1) All the affected patients were male, and mean age of onset was 31 years old. 2) Simultaneous involvement of axillae and pubis was found in 9 cases, and involvement of axillae only in 3 cases. 3) There was no statistically significant difference in the treatment efficacy between benzoyl peroxide and erythromycin (p> 0.05). 4) Infected hair shafts were removed by shaving in 7 cases. But shaving itself did not shorten the period for complete cure (p> 0.05). 5) Mean disease duration was 6.7 months and mean period for complete cure was 3.1 weeks. And there was no significant relation between disease duration and mean period for complete cure. 6) Only 1 case (8.3%) showed recurrence within 6 month-follow up, in which erythromycin topical application without shaving was used. 7) Among 12 cases, irritation and contact dermatitis were observed in 5 cases (41.7%). And these were all related with benzoyl peroxide application. CONCLUSION: Benzoyl peroxide is as effective as erythromycin in the treatment of trichomycosis axillaris and pubis. But, benzoyl peroxide have more side effect. Shaving itself is not necessag to treat trichomycosis.

Erythema ab igne is a relatively uncommon skin lesion caused by chronic exposure to low levels of infrared heat from open fires or stoves, and is characterized by reticulated hyperpigmentation on the exposed skin. Recently, exposure to various 'new' heat sources, such as heated furniture, car heaters, or even laptop computers, has been reported to cause this condition. We present a case of 31-year-old Korean woman with reticulated hyperpigmentation on her feet, which had occurred due to bathing her feet in hot water on a daily basis.

Nevus spilus, also known as speckled lentiginous nevus, is a relatively common cutaneous lesion that is characterized by multiple pigmented macules within a pigmented patch. It may be congenital or acquired, and it can show the histological features of congenital melanocytic nevus (CMN). The possible relationship between nevus spilus and CMN has been contentious. We report a case of a congenital nevus that followed the line of Blaschko and clinically manifested as nevus spilus, but showed the histological features of CMN.

Oral focal mucinosis (OFM) is an uncommon clinicopathologic entity which is considered to be the oral counterpart of cutaneous focal mucinosis. It is comprised of a clinically-elevated mass with a histological feature of localized areas of myxomatous connective tissue. We herein report a case of oral focal mucinosis in a child, plus a review of the literature.

Extramammary Paget's disease (EMPD) is a rare intraepidermal adenocarcinoma. Mohs micrographic surgery is considered the standard treatment. Other treatment modalities such as surgical excision, radiotherapy, topical chemotherapy, and photodynamic therapy are associated with varying degrees of effectiveness, and possible loss of tissue function and disease recurrence are seen. The search for an effective, safe treatment with minimal side effects proves to be challenging. Imiquimod is the first representative of a new class of immunomodulating drugs demonstrating potent antiviral and antitumor activity, and has been approved by the FDA for treating anogenital warts, actinic keratosis and basal cell carcinoma. Recent research has demonstrated its effective usage as a 5% cream in the management of EMPD. We report a case of EMPD of the scrotum successfully treated with topical 5% imiquimod cream. This has never been published in the Korea dermatologic literature.
