Atrial septal defect is one of the most common congenital heart diseases in adult, of which absolute causes is not confirmed, but explained by multifactorial inheritance. In Korea, there has been no case report of atrial septal defect in identical twins in spite of some reports of atrial septal defect in relatives and twins in other countries. We present a first case of atrial septal defect in identical twins whose mother had no infection or medication history during pregnancy.

Liposarcoma is the second most common soft tissue sarcoma next to malignant fibrous histiocytoma and occurs almost exclusively in adults. It is thought to originate from primitive mesenchymal cells rather than from mature adipose tissue. Most liposarcomas usually occur in the deep soft tissue of the extremities and the retroperitoneum, and rarely occur in the cutaneous or subcutaneous tissue. We report a case of an atypical lipomatous tumor in a 63-year-old male, who presented with an asymptomatic, subcutaneous mass on his right axilla.

Fibroelastolytic papulosis is proposed as the diagnostic term for cases previously identified as pseudoxanthoma elasticum-like papillary dermal elastolysis and white fibrous papulosis of the neck. Clinical presentation of fibroelastolytic papulosis is asymptomatic to mildly pruritic whitish-yellow papules that may coalesce into cobblestone- patterned plaques resembling pseudoxanthoma elasticum. Histology is characterized by normal, decreased, or absent elastic fibers in the papillary dermis sometimes accompanied by a thickening of collagen fibers. We report a 60-year-old woman with flesh to yellowish colored papules with cobblestone appearance, which were histologically consistent with fibroelastolytic papulosis.

Basal cell carcinoma (BCC) is the most common skin cancer of humans. The incidence of BCC is closely correlated with sun exposure, as well as older age. Because of this, BCCs are frequently found on sun-exposed areas of fair-skinned, older individuals. The axilla is one of the sun-protected sites where BCC rarely develops. We herein report three unusual cases of primary BCC which arose from the axilla.

Erythema ab igne is a relatively uncommon skin lesion caused by chronic exposure to low levels of infrared heat from open fires or stoves, and is characterized by reticulated hyperpigmentation on the exposed skin. Recently, exposure to various 'new' heat sources, such as heated furniture, car heaters, or even laptop computers, has been reported to cause this condition. We present a case of 31-year-old Korean woman with reticulated hyperpigmentation on her feet, which had occurred due to bathing her feet in hot water on a daily basis.

Nevus spilus, also known as speckled lentiginous nevus, is a relatively common cutaneous lesion that is characterized by multiple pigmented macules within a pigmented patch. It may be congenital or acquired, and it can show the histological features of congenital melanocytic nevus (CMN). The possible relationship between nevus spilus and CMN has been contentious. We report a case of a congenital nevus that followed the line of Blaschko and clinically manifested as nevus spilus, but showed the histological features of CMN.

Nodular fasciitis is a benign, reactive, myofibroblastic, proliferative process of unknown etiology. It presents as a solitary, painless, rapidly-growing nodule over several weeks' duration. The condition is self-limiting, and proper diagnosis is essential to avoid unnecessary aggressive treatment. Diagnosis is often a challenge because nodular fasciitis may be confused with a malignant tumor due to its aggressive clinical behavior and histological features. It is most commonly located on the extremities and then the trunk. Such lesions also usually occur in middle age and are rarely diagnosed in childhood. We report a case of recurrent nodular fasciitis that arose from subcutaneous tissue on the back of a pediatric patient.

Oral focal mucinosis (OFM) is an uncommon clinicopathologic entity which is considered to be the oral counterpart of cutaneous focal mucinosis. It is comprised of a clinically-elevated mass with a histological feature of localized areas of myxomatous connective tissue. We herein report a case of oral focal mucinosis in a child, plus a review of the literature.

Extramammary Paget's disease (EMPD) is a rare intraepidermal adenocarcinoma. Mohs micrographic surgery is considered the standard treatment. Other treatment modalities such as surgical excision, radiotherapy, topical chemotherapy, and photodynamic therapy are associated with varying degrees of effectiveness, and possible loss of tissue function and disease recurrence are seen. The search for an effective, safe treatment with minimal side effects proves to be challenging. Imiquimod is the first representative of a new class of immunomodulating drugs demonstrating potent antiviral and antitumor activity, and has been approved by the FDA for treating anogenital warts, actinic keratosis and basal cell carcinoma. Recent research has demonstrated its effective usage as a 5% cream in the management of EMPD. We report a case of EMPD of the scrotum successfully treated with topical 5% imiquimod cream. This has never been published in the Korea dermatologic literature.
