1.Comparison of Health Status and Nutrient Intake between Depressed Women and Non-depressed Women: Based on the 2013 Korea National Health and Nutrition Examination Survey.

This study aimed to provide supporting data for the management of dietary habits in depression by comparing health and nutrition in adult Korean women according to depression status. A total of 2,236 women aged between 19 and 64 years who participated in the 2013 Korea National Health and Nutrition Examination Survey were divided into a depression group (n = 315) and a non-depression group (n = 1,921). Among 19-29-year-old women, the depression group showed higher proportions of individuals with impairment of everyday activities, menopause, and suicidal thoughts than the non-depression group. The depression group showed lower intake of cereal, chocolate, meat, and carbonated drinks, as well as a lower index of nutritional quality (INQ) for protein, iron, and niacin. Among 30-49-year-old women, the depression group showed higher proportions of individuals with impairment of everyday activities, chronic disease, stress, and suicidal thoughts. The depression group showed lower intake of rice with mixed grains and higher intake of instant and cup noodles than the non-depression group. Among 50-64-year-old women, the depression group showed higher proportions of individuals with impairment of everyday activities, menopause, stress, and suicidal thoughts. The depression group showed lower intake of vegetables, mushrooms, and seaweed, lower nutritional intake of fat, saturated fat, and n-3 fatty acids, as well as a lower INQ for niacin and a lower Recommended Food Score. For all age groups, individuals with depression showed poorer health and nutritional intake than healthy individuals, demonstrating a correlation of depression with health and nutritional intake.

3.A Case of Omental Pregnancy.

Omental pregnancy is an extremely rare form of abdominal pregnancy that may cause life threatening massive hemorrhage in case of rupture. It may occur by primary implantation of the fertilized ovum in the omentum. The diagnosis is very difficult, but prompt surgical intervention is required when it is suspected, because perinatal mortality and maternal mortality in abdominal pregnancy were 85~95% and 0.5~6%. We experienced a case of omental pregnancy in the first times and reported it with brief review of literature.

5.Case of Mycobacterium Massiliense Infection Presenting as Recurrent Pneumonia in Sjogren's Syndrome.

Mycobacterium massiliense is an emerging pathogen that is increasingly reported as a causative agent occurring during medical procedures, at surgical sites, and intramuscularly [1]. Although previously classified as part of M. abscessus, M. massiliense has recently been identified as a new species of rapidly growing nontuberculous mycobacteria [1,3] via a comparative sequence analysis of rpoB and hsp65 [3,5]. However, the clinical manifestations of M. massiliense have not been well characterized. We report here in a case of recurrent pneumonia for 3 years that improved with antibiotic treatment for M. massiliense in a 37-year-old woman with Sjogren's syndrome. The patient showed a substantial response to treatment with a combination of antimicrobial therapies comprising clarithromycin and amikacin without cefoxitin for 6 months. This is the first report of pulmonary infection of M. massiliense with Sjogren's syndrome in Korea.

6.A Case of Tsutsugamushi Disease Complicated with Adult Respiratory Distress Syndrome and Disseminated Intravascular Coagulopathy.

Tsutsugamushi disease is an acute febrile illness caused by Orientia tsutsugamushi. It is characterized by fever, myalgia, lymphadenopathy, and rash. And it can be easily diagnosed by characteristic eschar and serologic testing. Nearly all of the patients with tsutsugamushi disease improve with antibiotics such as doxycycline. However, the fatality rate of untreated cases is seven to ten percent. The well-known causes of mortality are respiratory failure associated with pulmonary edema or adult respiratory distress syndrome. We report a case of tsutsugamushi disease complicated with acute respiratory distress syndrome and disseminated intravascular coagulopathy, despite of doxycycline treatment. A 78-year old woman was admitted to the hospital because of fever. Twelve days before admission she had suffered myalgia and some days later she developed a rash. Despite of management at a local clinic, her condition deteriorated and she was transferred to our hospital. On admission she presented with altered consciousness and two eschars on her right arm and right thigh. Under the initial diagnosis of scrub typhus, doxycycline was administered. Her fever subsided with the initiation of doxycycline. However, her hypoxia worsened progressively and she died on the fifth hospital day.

7.A Case of Bacillus licheniformis Bacteremia Associated with Bronchoscopy.

Bacillus species are aerobic, gram-positive, spore forming rods, and they are usually found in the surrounding environment. If they are isolated in the clinical specimen, they are generally considered as contaminants rather than a true pathogen. Infection with Bacillus licheniformis is usually associated with the immunocompromised state, trauma, an indwelling intravenous catheter or an intravenous drug abuser. This infection is easily controlled by removal of the catheter and surgical debridement of the local infected tissue as well as an appropriate antimicrobial therapy. We reported here on a case of Bacillus licheniformis bacteremia associated with a bronchoscopic procedure in an immune competent patient.

8.Effect of weight gain during the pregnancy on success of vaginal birth after cesarean delivery (VBAC).

PURPOSE: To examine the relationship between weight gain and the success of VBAC by using body mass index (BMI). To examine the relationship between weight gain and the success of VBAC by using body mass index (BMI). METHODS: The study compared clinical features taken from 112 patients who tried VBAC at our institute from January 2001 through December 2006. There were divided into two GROUPS: 92 patients for the success (82.1%) and 20 patients for the failure group (17.9%). Excluding 36 patients with no BMI data, we constructed Receive-operating characteristics (ROC) curve to make the optimum BMI value for the prediction of success of VBAC. Based on the BMI 26 or more, two groups of patient were surveyed the interrelation between weight gain and success of VBAC. RESULTS: Between success and failure group, the weight gain during pregnancy showed significant differences which are 11.2+/-4 kg of the success group and 13.2+/-5 kg of the other one (p<0.05) A survey on the availability of the BMI date to estimate success of VBAC, the criteria with the standard BMI 26 is not statistically valuable (p=0.837). By comparing normal weight and overweight based on BMI 26, some factors showed statistically significant discrepancies: number of prenatal visit, maternal weight gain, maternal weight at the time of delivery, use of oxytocin and birth weight. CONCLUSION: BMI value of 26 has limitations in using as an estimate criteria on success of VBAC. Patients, however, who had relatively small scale of weight gain, showed significant clinical factors to increased success rate of VBAC.

9.Expression of GnRH and GnRH-receptor mRNAs in the Human Placenta.

Placental GnRH is one of the potential paracrine regulator of hCG secretion from the trophoblasts during pregnancy. However, this paracrine function is not clearly confirmed. The present study was designed to evaluate the possible correlation between the synthesis and cellular distribution of GnRH and GnRH -receptor in the placental villi. 6 to 40 weeks termed twenty five human placental tissues were used in this study. GnRH and GnRH -receptor mRNAs were localized in the placenta by in situ hybridization using the corresponding cRNA probes. GnRH mRNA was present in all cell types of placenta including cytotrophoblasts, syncytiotrophoblasts and stromal cells. The GnRH -receptor mRNA signals were localized in both cytotrophoblasts and syncytiotrophoblasts. The GnRH -receptor mRNA signals were also localized in some stromal cells at the full term placenta. GnRH mRNA signals were detected abundantly at 6 ~7 weeks and the intensities were remarkably increased with the following gestational ages. GnRH -receptor mRNA signals were detected at 6 ~7 weeks, peaked at 9 ~10 weeks, declined at 23 ~24 weeks and focally expressed at full term placenta. The present study demonstrates that GnRH mRNA is expressed in all cell types of the placenta, however GnRH -receptor mRNA is expressed only in the cytoptrophblasts and syncytiotrophoblasts and exhibits parallel change with the time course of hCG secretion during pregnancy. These data provide a morphological understanding that placental GnRH has a paracrine/autocrine role through its receptor from 6 ~7 weeks gestation to term placenta.

10.Sensitivity of Two-Time Immunochemical Fecal Occult-Blood Testing with Sigmoidoscopy for the Detection of Advanced Colon Adenoma.

BACKGROUND/AIMS: Because detection and removal of colonic adenoma provided an opportunity to prevent colorectal cancer, advanced adenoma (>10 mm, villous or high grade dysplasia) should be the major target of screening. In this study, we assessed the diagnostic sensitivity of one- or two-time immunochemical fecal occult blood test (i-FOBT), flexible sigmoidoscopy and their combination in patients with advanced adenoma or non-advanced adenoma. MEHTODS: From January to October 2002, we performed colonoscopy with i-FOBT using latex agglutination method in 879 individuals. Among these, we diagnosed 234 polyps in 93 patients with advanced adenoma and 179 polyps in 109 patients with non-advanced adenoma. After the diagnosis of adenoma, second i-FOBT was done before polypectomy. Based on these data, we evaluated the diagnostic sensitivities of i-FOBT, flexible sigmoidoscopy and their combination for patients with advanced adenoma or non-advanced adenoma. RESULTS: The diagnostic sensitivity of one- or two-time i-FOBT, flexible sigmoidoscopy and flexible sigmoidoscopy with two-time i-FOBT in patients with advanced adenoma vs.non-advanced adenoma were 17.2% vs. 18.3%, 28.0% vs. 29.4%, 70.1% vs. 66.1% and 81.7% vs. 78.0%. Although repeated application of i-FOBT enhanced diagnostic sensitivity for colon adenoma, this test or combination with flexible sigmoidoscopy did not differentiate advanced adenoma from non-advanced adenoma. CONCLUSIONS: Although it fails to detect one fifth of colon adenoma, combined two-time i-FOBT testing with flexible sigmoidoscopy is an effective and feasible screening modality for advanced colon adenoma.
