1.Clinical Study of Hypertention.

A retrospecive clinical observation was done on 325 cases of hypertension admitted to Department of internal Medicine of Seoul Red Cross Hospital from January, 1979 to December, 1982. The following results have been obtained. 1) The observed patients were 157 males and 168 females, and the prevalence rate of malignant hypertension among 325 hypertensions was 16 cases (4.9 percent). Among 16 cases of malignant hypertension 9 cases were male and 7 cases were female. Most frequently in 50, 60 and 40 years of age in this order each sex. 2) The average admitted dates are 7 days. 27 cases (8.3percent) were readmitted above 2 times after one discharged. 3) The most freqeuntly observed duration of Known hypertension 37.9 percent was 6-10 years and the next frequently observed duration 37.4 percent was 3-5 years. 4) The highest percentage of malignant hypertension had systolic blood pressure between 190-209mmHg and diastolic blood pressure between 150-169mmHg. 5) The highest percentage of seasonal number was spring (31.1 percent) and the next autumn, summer, winter in this order and the prevalence rates of hypertention are increased average 1.9% of each year. 6) The 78.5 percent of the patients were found that their blood pressure were abnormaly high when they were examined with related symptoms on admission, and they major symptorms were headache and dizziness. 7) The physical examination on admission, tachycardia (56percent), tachypnea (15.1percent), semicoma to coma (12.9 percent) in this order. 8) The status of treatment in hypertension on admission was as follows; imtermittent treatment 54.5 percent, no treatment 31 percent and continuous treatment 14.5 percent in this order. 9) In the chest X-ray on admission, the most frequent finding was cardiomegaly, and normal finding was the next frequency. The pulmonary Tbc was noted in 12.6 percent. 10) According to the degree of fundoscopic abnormality by K-W classification stage III was 68.8 percent and stage IV was 31.2 percent. 11) Electrocardiogram abnormalities were 44.9 percent, and 87.5 percent of malignant hypertension revealed electrocardiographic abnormalities and left ventricle hypertrophy was observed in 70.5 percent. 12) Hypercholesterolemia 78 percent, Hypernatremia 69.8 percent, Hypokalemia 22.8 percent, serum chloride 34.5 percent (<95m Eg/l), BUN 53.8 percent (>20mg/dl) and creatinine 45.8 percent (>3mg/dl) on admission. 13) Proteinuria was observed in 87.5 percent of malignant hypertension. 14) Marked improved for clinically was 24 percent and mortality rate was 5.2 percent, most common cause of death are cardiovascular accident (64.7percent) and renal failure (23.5percent).

2.Effect of Nicardipine on Left Ventricular Mass in Hypertensive Patients.

BACKGROUND: Systemic hypertension produces varying degree of LVH which is associated with increased cardiovascular morbidiity. Previous studies have documented regression of LVH with various antihypertensives including calcium channel blockers, except diuretics and vasodilators. Recently echocadiographic assessment of the change of left ventricular mass(LVM) after antihyertensive therapy have been reported to offer prognostic cardiovascular information. The aim of this echocardiographic study is determining the influence of nicardipine, a calcium channel blocker, on the change of LVM in patients with essential hypertenison. METHODS: Left ventricular mass(LVM) and left ventricular mass index(LVMI) were measured by M-mode echocardiography in 15 patients with essential hypertension. Nicardipine, a calcium channel blocker, was administered for 6 months and two echocardiographies were done before and after administering, respectively. RESULTS: In the 15 patients treated for 6 months, systolic and diastolic pressure remained very significantly decreased compared with pressure before before therapy(135+/-15mmHg vs 168+/-26mmHg, and 86+/-7mmHg vs 105+/-16mmHg, both p<0.01). Concomitantly both LVM and LVMI decreased significantly(209+/-49g vs 235+/-71g, and 116+/-6g/m2 vs 131+/-38g/m2,both p<0.05). And no change was noted in left ventricular cavity size, demonstration that LVM reduction was due to regression of hypertrophy. CONCLUSION: This study showed that nicardipine produced a significant decrease in blood pressure, LVM, and LVMI over the 6 months period. And large and longterm controlled studies are needed for the clarification of the association between nicardipine and regression of LVH in hypertensive patients.

3.A case of angioedema associated with eosinophilia induced by bee sting.

There have been few reports of manifestations such as vasculitis, nephrosis, neuritis, encephalitis, and serum sickness occuring in a temporal relation to insect stings. Symptoms usually start several days to several weeks after the sting and may last for a long time. Angioedema with eosinophilia induced by bee sting has not reported in medical literature. We report a case of eosinophilia with angioedema induced by bee venom in a 30-year-old woman whom presented with edema of extremities and peripheral blood eosinophilia. The patient had high titer of specific IgE to yellow jacket venom.

4.A Study on the Positivities of Anti - HCV by Second - Generation ELISA in Rural Area of Chonnam Province.

BACKGROUND: Hepatitis C virus(HCV) is well known major cause of posttransfusion hepatitis. There were a lot of studies about the prevalence of anti-HCV. But, most of these focused on healthy blood donors or patients with liver diseases in urban areas. So they may not represent the prevalence of anti-HCV in the entire Korean population. This study focused on people of a rural area which consisted of six towns near Reservoir Juam in the Chonnam province in Korea. METHODS: Nine hundred and sixty three persons were selected by multi-stage cluster sampling method from January to February in 19%, Anti-HCV and HBsAg were examined by microparticle enzyme immunoassy(MELA; Abbott Co., USA). Alanine aminotransferase(ALT) was examined by enzyme kinetic method. RESULTS: 1) The positivity of anti-HCV in all subjects was 0.9%. 2) The positivity of anti-HCV in male(1.1%) was not significantly higher than in female(0.9%). 3) The positivity of anti-HCV was 0% below the 5th decade, 0.7% in the 6th decade, 1.4% in the 7th decade, 1.7% in 8th decade, 0% above the 9th decade. There was no significant relationship between the positivity of anti-HCV and age. 4) The positivity of anti-HCV was 0.8% in normal ALT(

5.Clinical Usefulness of Perfusion CT in Acute Ischemic Stroke.

BACKGROUND: In recent years, thrombolytic therapy has been used with success to reduce the infarction area in patients with acute cerebral infarction, It is very important to identify ischemic penumbra in the treatment of stroke. The aim of this study is detect ischemic penumbra and border between ischemic penumbra and infarction area using perfusion CT and diffusion-weighted MR images (DW-MRI) in patient 's with acute ischemic stroke. METHODS: Thirteen inpatients with acute ischemic stroke at the Department of Neurology at Chonnam University Hospital were selected for this study. They had the discrepancy of lesion between perfusion CT and DW-MRI. Perfusion CT was done on admission and DW-MRI within 24 hours after stroke in all patients. Ischemic penumbra was assessed by comparing the defect on perfusion CT with the infarction area on DW-MRI. Clinical outcome was measured with the NIHSS on day 1, 3, and 7. RESULTS: The subjects (perfusion defect on the perfusion CT> infarction area on DW-MRI) achieved significant improvement at NIHSS. There was a significant difference between infarct and non-infarct tissue for both rCBF and rCBV but not for MTT. It could be possible not to define the tissue outcome but to estimate it by this study. CONCLUSIONS: Perfusion CT with DW-MRI is a useful tool for the detection of ischemic penumbra which may be the main target of active treatment.

6.Comparison of Suicide Attempt by Generation During the COVID-19 Pandemic: Focusing on the Younger Generation

Objectives:
This study aimed to comparatively analyze the demographic characteristics and various other factors influencing those attempting suicide from the younger and older generations during the COVID-19 pandemic.
Methods:
The study included subjects who visited the emergency room of the Gachon University Gil Medical Center in Incheon after attempting to cause self-harm or commit suicide. Of the total of 598 cases, the characteristics of 383 subjects belonging to the younger generation (39 years old or younger) and 215 to the older generation subject (40 years old or older) were retrospectively compared.
Results:
The younger generation subjects who attempted suicide had significantly higher rates of psychiatric disorders such as bipolar disorder and histories of past suicide attempts compared to the older generation. Those attempting suicide among the younger generation were less planned in their attempts to commit suicide than the older generation, and the authenticity of their suicide attempts was significantly lower.
Conclusion
The younger generation subjects who attempted suicide were more affected by mental illnesses and more impulsive than those of the older generation, and the authenticity and medical lethality of their suicide attempts were low. An individual-centric intervention strategy for suicide prevention is needed in consideration of these generational characteristics.
7.A Case of Hydronephrosis as a Complication of Pelvic Actinomycotic Abscess.

Pelvic actinomycosis is a chronic granulomatous suppurative disease caused by an anaerobic gram- positive organism Actinomyces israelii. It is commonly associated with an intrauterine device(IUD) and can mimick pelvic or intra-abdominal malignant neoplasm. Ureteral obstruction leading to hydronephrosis is a rare complication of tubo-ovarian abscess. We experienced a case of hydronephrosis as a complication of pelvic actinomycotic abscess. The patient was a 46-year-old women presenting with fever and right flank pain. Leukocytosis and pyuria were present and a hydronephrosis was diagnosed by intravenous pyelography. Ultrasonography and a computerised tomography revealed a mass in right adnexum compressing the right ureter. Removal of retroperitoneal abscess and salphingo-oophorectomy were done and the diagnosis of actinomycosis was made by pathologic finding of resected mass. Postoperatively, the patient was treated with second-generation cephalosporin successfully.

8.Factors Influencing Sexuality Knowledge and Gender Sensitivity During Adolescents and Early Adulthoods

Objectives:
:This study investigated the level of sexuality knowledge and gender sensitivity and factors influencing on them among Korean high school and college students.
Methods:
:129 questionnaires from high school and college students with consent forms, collected from July to December of 2022, were analyzed. After identification of participants’ gender, age, major, their report of the practicality of sex education, and their experience of violence or sexual assaults, participants’ level of sexualityknowledge and gender sensitivity were compared.
Results:
:Females showed higher gender sensitivity, regardless of age and group (p=0.004). The early adulthood group (≥19 years old) showed higher sexuality knowledge and gender sensitivity, compared to those adolescents (<19 years old). Among college students, those who majored in health science were identified with higher sexuality knowledge and gender sensitivity, compared to non-health science major students(p<0.001; p=0.005).
Conclusions
:This study revealed the significant differences in sexuality knowledge and gender sensitivity according to sex, age, and majors in adolescents and early adults. Therefore, it seems to be necessary to consider the differences in sexuality knowledge and gender sensitivity among age, sex, and majors when establishing publicpolicy or legislation for sex crimes, including digital sex crimes.
