1.Relations between peri and postmenopausal symptoms and vertebral bone mineral density.

BACKGROUND: Menopause leads to rapid bone loss, mainly as a result of estrogen deficiency and peri and postmenopausal symptoms are related with estrogen deficiency. We investigated the relationship between peri and postmenopausal symptoms and vertebral bone mineral density(BMD). METHODS: Study subjects were women aged above 40 years underwent a vertebral bone density measurement with dual energy x-ray absorptiometry(DEXA). We examined the relationship between the menopausal symptoms by Kupperman index including vasomotor symptom and vertebral BMD in 74 perimenopausal and 205 postmenopausal women. RESULTS: On the basis of vertebral fracture threshold, 169 subjects were osteoporotic group and the others were nonosteoporotic group. Menopausal index was significantly high in osteoporotic group. And the osteoporotic one also complained of vasomotor symptom more severely. These findings were more definite during perimenopausal and early postmenopausal periods than late period. CONCLUSIONS: The results suggest that menopausal symptoms of peri and early menopausal period are significantly related with vertebral bone loss and helpful in predicting the vertebral osteoporosis.

4.CT Findings of Bowel and Mesenteric Injury.

PURPOSE: To evaluate the role of CT in the diagnosis of bowel and mesenteric injury we studied the CT findings and its usefulness in patients of abdominal trauma. MATERIALS AND METHODS: CT scans of 27 patients who were confirmed to have bowel and/or mesenteric injury due to abdominal trauma were analyzed retrospectively. Of these 27 patients 15 had bowel injury only and 12 had both bowel and mesenteric injury. CT findings analysed were bowel wall thickening, presence or absence of highly attenuated bowel wall, sentinel clot, mesenteric infiltration, peritoneal fluid collection and free intraabdominal air in cases with bowel injury only and with both bowel and mesenteric injury respectively. Ten patients had other accompanying abdominal injuries, such as liver, spleen, pancreas, kidney, bladder injuries, intraperitoneal abscess or retroperitoneal hemorrhage. RESULTS: Findings observed were bowel wall thickening in 23 cases(85%), peritoneal fluid collection in 21 (78%), highly attenuated bowel wall in 19(70%), mesenteric infiltration in 17(63%), free intraperitoneal air in 10 (37%) and sentinel clot in 7(26%). Pneumoperioneum were observed in 10 of 24 patients(41.7%) having bowel perforation. Two cases did not show any CT findings suggesting bowel and/or mesenteric injury. There was no significant difference in the prevalence of the CT findings between the patient group with bowel injury only and the patient group with both bowel and meseneric injury. CONCLUSION: CT scan is a useful tool in evaluating the degree and extent of bowel and/or mesenteric injury as well as in planning the patient's management.

6.Single Centre Experience on Decision Making for Mechanical Thrombectomy Based on Single-Phase CT Angiography by Including NCCT and Maximum Intensity Projection Images – A Comparison with Magnetic Resonance Imaging after Non-Contrast CT

Objective:
: The purpose of this study was to suggest that computed tomography angiography (CTA) is valuable as the only preliminary examination for mechanical thrombectomy (MT). MT after single examination of CTA including non-contrast computed tomography (NCCT) and maximum intensity projection (MIP) improves door-to-puncture time as well as results in favorable outcomes.
Methods:
: A total of 157 patients who underwent MT at Dong Kang Medical Center from April 2015 to March 2019 were divided into two groups based on the examination performed prior to MT : CTA group who underwent CTA with NCCT and MIP, and NCCT+magnetic resonance image (MRi) group who underwent MRI including perfusion images after NCCT. In the two groups, time to CTA imaging or NCCT+MRi imaging after symptom onset, and time to arterial puncture and reperfusion were characterized as time-related outcomes. The evaluation of vascular recanalization after MT was defined as a modified thrombolysis in cerebral infarction (mTICI) scale. National Institutes of Health Stroke Scale (NIHSS) was assessed at the time of the visit to the emergency room and modified Rankin Scale (mRS) was assessed after 90 days.
Results:
: Typically, there were 34 patients in the CTA group and 33 patients in the NCCT+MRi group. A significantly shorter delay for door-to-puncture time was observed (mean, 86±22.1 vs. 176±47.5 minutes; p<0.01). Also, a significantly shorter door-to-imege time in the CTA group was observed (mean, 13±6.8 vs. 93±30.8 minutes; p<0.01). Moreover, a significantly shorter onset-to-puncture time was observed (mean, 195±128.0 vs. 314±157.6 minutes; p<0.01). Reperfusion result of mTICI ≥2b was 100% (34/34) in the CTA group and 94% (31/33) in the NCCT+MRi group, and mTICI 3 in 74% (25/34) in the CTA group and 73% (24/33) in the NCCT+MRi group. Favorable functional outcomes (mRS score ≤2 at 90 days) were 68% (23/34) in the CTA group and 60% (20/33) in the NCCT+MRi group.
Conclusion
: A single-phase CTA including NCCT and MIP images was performed as a single preliminary examination, which led to a reduction in the time of the procedure and resulted in good results of prognosis. Consequently, it is concluded that this method is of sufficient value as the only preliminary examination for decision making.
7.The effects of different exercises on regional bone density in young adult female athletes.

BACKGROUND: Previous studies have suggested that regular exercise may increase bone mineral density. However, the effects on bone mineral density are different depending on the types of exercises and recent studies on the effects of different exercises on bone mineral density are insufficient. This study was conducted to assess the effects of 2 different exercises(Judo, Taekwondo) and physical fitness(back muscle strength, grip strength, broad jump, vertical jump, flexibility, sit ups) on regional BMD. METHODS: Subjects were healthy adults female volunteers aged 19 to 22 years and consisted of eight Judo athletes, twelve Taekwondo athletes, and eight nonathletic controls. We measured their bone mineral density of lumbar spine, femoral neck, femoral greater trochanter, Wards triangle, femoral shaft and distal radius using dual energy X ray absorptiometry and assessed their physical fitness. We investigated the mean differences of regional bone mineral density between the groups and the relationships between physical fitness and regional bone mineral density. RESULTS: ANCOVA revealed that there were significant differences in the regional bone mineral density between the groups. The Judo athletes group had significantly higher bone density of greater trochanter, femoral shaft, distal radius, femoral neck and lumbar spine than the control group and had significantly higher bone density of greater trochanter, femoral shaft and distal radius.than the Taekwondo atheletes group. There were no significant differences of bone density in all the regions between the Taekwondo atheletes and the control groups. Partial correlation coefficients between the back muscle strength and the bone density of Wards triangle, greater trochanter, distal radius, femoral neck, femoral shaft, and lumbar were 0.581, 0.570, 0.526, 0.502, 0.424, and 0.418, Respectively (P<0.05). Correlation coefficients between the broad jump and the bone density of femoral neck, greater trochanter, and femoral shaft were 0.577, 0.539, and 0.457, respectively(P<0.05). Correlations of the grip strength, flexibility, sit ups and vertical jump with bone density of all regions were not high(r< or=0.5). CONCLUSIONS: Different types of exercises showed different effects of bone density. The back muscle strength among the physical fitness factors was considered to be the most important predictor of bone density. In the future, further studies are necessary for the effects of other exercises on bone density.

8.Folliculosebaceous Cystic Hamartoma of the Eyelid.

PURPOSE: Folliculosebaceous cystic hamartoma is a rare cutaneous hamartoma consisting of dilated folliculosebaceous units invested in mesenchymal elements. There is no report of folliculosebaceous cystic hamartoma case occurred in the eyelid. We report here on this case along with a review of the relevant literature. CASE SUMMARY: 72-year-old female visited for the complaint of a mass in right upper eyelid. The mass was 1.9 × 1.2 cm sized and palpated in the subcutaneous level of right upper eyelid. The mass was not tender and had hardness like rubber. It was covered by skin without adhesion but fixed on the upper tarsal plate. Turning the eyelid inside out, it was found that the upper tarsal plate was penetrated by the mass. There was no specific finding except both cataract by other ophthalmic examination. The paranasal sinus computed tomography finding was well demarcated 0.9 cm sized mass with calcification. The excisional biopsy was performed for diagnosis and treatment. In pathologic finding, various sized normal sebaceous lobules were connected with the dilated follicles through the sebaceous canal and formed infundibular structure. There were sclerosing collagen, adipose cells and vessels between follicles and sebaceous lobules. So it was compatible with folliculosebaceous cystic hamartoma. 18 months later, there was no recurrence and wound was clear. CONCLUSIONS: Folliculosebaceous cystic hamartoma of the eye lid is rare disease, and differential diagnosis is necessary in patient with mass of eyelid.

9.Characteristics of Potassium Channel in the Isolated Rat Detrusor Muscle.

The purpose of this study was to investigate the characteristics or the potassium channels existing in the rat urinary bladders. Smooth muscle strips of rat detrusor urinae were examined by isometric myography. Relaxation responses of detrusor muscle strips to the three potassium channel openers pinacidil, a cyanoguanidine derivative, BRL 38227, a benzopyran derivative and RP 52891, a tertrahydrothiopyran derivative were examined. The potassium channel openers reduced the basal tone, and the rank order of potency was RP 52891>pincidil>BRL 38227. Procaine, an inhibitor of the voltage-sensitive potassium channel tended to increase the basal tone, but it did not affect the relaxant effects of the calcium-activated potassium channel opener did not antagonize the relaxant effects, but it reduced the Emax of RP 52891 and BRL 38227. Glibenclamide, an inhibitor of the ATP-sensitive potassium channel, antagonized the relaxant effects of pinacidil, RP 52891 and BRL 38227 reducing the Emax of RP 52891 and BRl 38227. Galanin which inhibits secretion of insulin through opening the ATP-sensitive potassium channels in pancreatic β-cells rather increased the basal tone of the isolated detrusor strips. These results suggest that the urinary bladder of the rat has mainly the ATP-sensitive, glibenclamide sensitive potassium channel, which is a different type from that in the pancreatic β-islet cells.

10.Endovascular Coil Trapping of a Ruptured Dissecting Aneurysm of the Vertebral Artery Using Detachable Coils and Micro-Tornado(R) Coils.

We experienced a patient with a ruptured dissecting aneurysm of the vertebral artery who was treated by trapping of the lesion using Guglielmi detachable coils (GDCs) with micro-tornado(R) coils (MTCs). An 80-year-old male was transferred with a ruptured left vertebral artery dissecting aneurysm (VADA). The dissected portion of the vertebral artery was effectively trapped using GDCs and MTCs. The MTCs used for neurointervention were comprised of various types of coils and we successfully placed them into the parent artery of the dissected segment. The author suggests that this case demonstrates the usefulness of endovascular coil trapping of VADAs using MTCs in achievement of embolization.
