3.Long-term Clinical Follow-up in Patients with Left Main Coronary Disease According to Treatment Strategies.

BACKGROUND AND OBJECTIVES: Many studies have shown that coronary artery bypass graft (CABG) surgery prolongs the life of patients with left main coronary artery disease (LMCD). Recently, percutaneous coronary intervention (PCI) has been applied to treat LMCD, with good clinical results. However, a significant portion of patients decline any revascularization therapy, so receive medical treatment only. The aim of this study was to evaluate the long term clinical outcome in these patients with LMCD, according to the treatment strategies. SUBJECTS AND MEHTODS: The clinical outcomes of 281 consecutive patients, with significant LMCD, between January 1997 and December 2000, were evaluated. The patients were divided into three groups, according to their initial treatment strategies;1) CABG, 2) PCI and 3) medical treatment. The mean follow-up duration was 37.4+/-14.9 months. RESULTS: The 1- and 3-year survival rates in the CABG group (97.4+/-1.5% and 95.6+/-1.9%) were significantly higher than those of the medical group (89.8+/-3.9% and 76.1+/-5.9%;p=0.03). The survival rates in the PCI group (one year and 3-year survival rate, 98.1+/-1.3% and 93.8+/-2.5%) were similar to those of the CABG group (p=0.93). The incidence of 3-year MACE in the medical group (40.7%) was higher than those of the CABG (10.5%, p<0.001) and PCI groups (20.4%, p=0.007). There was no significant difference between the CABG and PCI groups (p=0.06). CONCLUSION: In patients with LMCD, a CABG remains the standard therapy for prolonging survival and lowering the incidence of MACE. PCI offers similar survival benefits in selected patients. Medical treatment is associated with a significantly higher mortality and MACE. Active revascularization therapy should be the treatment of choice for the patients with LMCD.

4.Nonleukemic Granulocytic Sarcoma of the Ileum: A Case Report.

Granulocytic sarcoma is an uncommon clinical condition that usually occurs in the bone, periosteum, soft tissue, lymph node and skin, and this is rarely seen in the gastrointestinal tract. To the best of our knowledge, few cases of granulocytic sarcoma in the small bowel have been reported in the English literature. We present here a case of nonleukemic granulocytic sarcoma of the ileum in a 42-year-old patient who had no evidence of blood or bone marrow involvement that would have been suggestive of acute leukemia or myeloproliferative disorders.

Fibrolamellar hepatocelluar carcinoma is a distinct clinicopathologic variant of hepatocellular carcinoma. We describe here the sonographic and CT findings of fibrolamellar hepatocellular carcinoma in a 17-year-old patient that mimicked hepatic malignant lymphoma due to the multiple small hypoattenuating nodules and extensive lymphadenopathy that we observed. We also include a review of the relevant literatures.

7.Study for the Ureteral Reconstruction with Tissue Engineering Using Poly (glycolide/epsilon-caprolactone) Scaffold-1.

PURPOSE: Transplantation is one modality saving human life. But not only lack of the living or cadaveric human organs but also immunologic problems or some ethical situations limit transplantation in terminal stage patients. Recent research for escaping from those problems resulted in the reconstruction of the artificial organ using patients' own cells with tissue engineering. The goal of this study is, for the better reconstruction of urinary system using tissue engineering, to perform basic researches on techniques related with seeding and viability of cells. MATERIALS AND METHODS: We used 16 adult dogs, 4 female and 12 male, for primary culture of the dog bladder mucosal cell and muscle cell. The scaffold we used was made of absorbable substance polyglycolide/epsilon-caprolactone (GL/CL) in thin sponge like shape. Fibroblast 3T3 cell was used for control and 16 primary cultured mucosal cell and smooth muscle cells were used. For dynamic culture, rocker was adapted with for 5 hours. Attached cells were evaluated by 562nm ELISA reader using BCA method and scanning electron microscope. RESULTS: Successful primary culture was achieved with cells from dog bladder, and results were much better by using male dog. The dynamic culture increased attachment of the cell in scaffold and the cell attached at deeper portion of the scaffold. Long term culture showed formation of the cellular sheets on the surface of scaffold preventing inner passage of the suggesting disadvantageous condition for cells in core of the scaffold. CONCLUSIONS: We conclude that for the better attachment of the cultured cells on scaffolds, dynamic culture would be desirable. And for the better in vivo reconstruction of the organ with primary cultured cell attached scaffold, evaluation of culture state with repeated in vitro experiments are necessary.

10.Pharmacological Interventions on Surgical Intensive Care Units Initiated by Pharmacists

Purpose:
The severity of a patient’s medical condition, changing pharmacodynamics and pharmacokinetics, and variability in medication highlight the importance of pharmacological intervention by intensive care unit (ICU) specialized pharmacists.
Methods:
Retrospective observations of ICU interventions (omission, changes in medicine, side effects, changes in administration route and dosage, redundancy, and nutritional care) performed between April 2017 and March 2018, determined by an interdisciplinary team (including a specialized ICU pharmacist and a surgical intensivist) on their surgical ICU round, were analyzed. Medicinal prescriptions were screened weekly during the surgical ICU round, and interventions were made if any corrections were necessary. Two days later another team including a surgical intensivist, a pharmacist, and a nutritionist evaluated the patients’ nutritional status (performed weekly).
Results:
In the 23-bed ICU, the average number of patients whose prescriptions were examined was 22.38 per surgical round. There were 382 interventions made over 1 year, which was 9.68 interventions per day. The interventions were for nutritional care (161 cases, 42.2%), followed by changes in administration route and dosage (94 cases, 24.6%), omission (59 cases, 15.5%), redundancy (40 cases, 10.4%), changes in medicine (15 cases, 3.9%), and side effects (13 cases, 3.4%).
Conclusion
The conditions of patients admitted to ICU are typically unstable. Pharmacological interventions suggested by a specialized pharmacist may help control the changing medical condition of patients in ICU. A higher participation of pharmacists specialized in working in an interdisciplinary ICU team-based system could lead to safer treatments.
