1.Assessment of Posterior Capsular Opacification of Korean Using Straylight and Glare Sensitivity Meter.

PURPOSE: To evaluate posterior capsular opacity (PCO) using straylight and glare sensitivity meter and to compare availability of straylight and glare sensitivity with known methods for PCO evaluation. METHODS: Thirty-six pseudophakic eyes with PCO were selected for this study. Best-corrected visual acuity (BCVA), straylight (C-quant, Oculus GmbH, Wetzlar, Germany) and glare sensitivity (Binoptometer, Oculus GmbH, Wetzlar, Germany) were measured before mydriasis. After mydriasis, PCO images were captured with a slit-lamp and analyzed using the Evaluation of Posterior Capsular Opacification (EPCO) program (EPCO software, University of Heidelberg, Heidelberg, Germany). The same measurements were taken after capsulotomy and compared with pre-capsulotomy data. RESULTS: After capsulotomy, BCVA, EPCO score and straylight were improved with statistical significance (p < 0.05). Cases of PCO with mildly decreased visual acuity showed statistically significantly improved EPCO score and straylight (p < 0.05). Glare sensitivity did not show significant improvement but was statistically significantly correlated with straylight (p = 0.023, Rho = 0.732). CONCLUSIONS: Straylight is an available measurement for evaluation of PCO. Glare sensitivity meter which correlates with straylight can be used as a supportive measurement.

3.Effect of topical ketorolac tromethamine and topical nepafenac on maintaining pupillary dilation during phacoemulsification

Objective:
This study compared the effectiveness of prophylactic administration of topical ketorolac tromethamine 0.5% and nepafenac 0.1% on maintaining mydriasis during phacomulsification.
Methods:
This is a prospective, randomized, double-masked comparative study involving adult cataract patients given topical NSAIDs (ketorolac or nepafenac) or balanced salt solution (control) prior to phacoemulsification and capsular bag intraocular-lens (IOL) implantation at a tertiary hospital. Horizontal and vertical diameters of pupil were measured at different stages of cataract surgery and the mean values were compared across the three groups.
Results:
A total of 47 eyes of 44 cataract surgery patients, 13 males and 34 females, with a mean age of 66.04 ± 8.87 years, were included in the study. The mean horizontal and vertical diameters of the three groups were similar at the start of surgery. Significant differences were seen after IOL implantation, with the nepafenac group having the largest mean diameters in both horizontal (p = 0.012) and vertical (p = 0.012) pupil measurements.
Conclusion
Topical nepafenac has been shown to be a more effective inhibitor of miosis during phacoemulsification and provides a more stable mydriatic effect throughout the surgical procedure compared to topical ketorolac and placebo.

6.The Effect of Topical Dapiprazole on Phenylephrine and Tropicamide-induced Mydriasis.

In order to evaluate the effect of topical dapiprazole, an alpha-adrenergic receptor blocker, in reversing mydrisis by 1% tropicamide and 2.5% phenylephrine, we studied 89 subjects. 0.5% dapiprazole in one eye of each subject after full dilatation of both pupils. The pupillary diameter was calibrated, and thereafter statistical analysis was performed with student paired t-test. Thirty minutes after instillation of dapiprazole, there was a highly significant difference of the decreasing rate in pupil size between the dapiprazole treated eyes and the control eyes(p<0.001). The time for complete reversal of mydriasis was also at least 4 hours shorter in the dapiprazole treated eyes than the control eyes(p<0.006). This study suggests that 0.5% dapiprazole is effective in reversing mydriasis caused by 1% tropicamide and 2.5% phenylephrine, and also effective in making the eyes comfortable in a shorter time.

7.Preoperative Maximal Mydriasis Test in Cataract Patients.

To predict the mydriatic status of the cataract patients preoperatively, we performed maximal mydriasis test with stepwise instillations of 1% tropicamide and 2.5% phenylephrine or 10% phenylephrine to the out-patients who was scheduled to have cataract operation. Among 210 eyes of 113 patients, the pupillary size of 167 eyes was dilated to 8mm or more and the other 43 eyes were insufficiently dilated to less than 8mm diameter. In the insufficiently dilated group, we have observed the presumable causes of insufficient mydriasis as follows: posterior synechiae, diabetic autonomic pupillopathy, age-related miosis and unknown. By this test, we could predict the mydriatic status of the cataract patient preoperatively and could prepare the appropriate operative method and equipment effectively.

8.Mydriatic Action of Phenylephrine Hydrochloride.

Phenylephrine hydrochloride has been widely used in ocular theraphy and examination, such as the treatment of uveitis and glaucoma and examination of the eye ground. In this study, mydriatic action of single instillation of 10% phenylephrine hydrochloride solution was compared with that of combined instillation of 10% phenylephrine hydrochloride solution and 0.5% tetracaine drops in 110 brown eyes. The following results were obtained; 1) In 55 eyes, the diameter of the pupil reached more than 7 mm about 45 minutes after instillation of 10% phenylephrine hydrochloride solution. 2) In case of combined instillation of 0.5% tetracaine solution and 10% phenylephrine hydrochloride solution, the onset of mydriasis was delayed. 3) In the case of combined instillation of 0.5% tetracaine and 10% phenylephrine hydrochloride solution with 5 and 10 minutes interval, the action of mydriasis was acceralated. 4) After tonometry, the action of mydriasis was more acceralated.

9.The Effects of Pharmacologic Pupil Dilatation on Ocular, Corneal, and Internal Aberrations.

PURPOSE: The present study investigates the effects of pharmacologic pupil dilatation on ocular, corneal and internal aberrations. METHODS: Sixty-two right eyes of 62 healthy participants were included in the present study. Ocular, corneal and internal aberrations were measured with a KR-1W wavefront aberrometer (Topcon Corp., Tokyo, Japan) before mydriasis in mesopic conditions. After pupil dilatation with a mydriatic drug (phenylephrine chloride 0.5% + tropicamide 0.5%) (Mydrin-P, Santen, Osaka, Japan), the measurements were repeated. The wavefront data of 4-mm and 6-mm diameter zones were analyzed. The changes of aberrations before and after mydriasis were evaluated by paired t-test. RESULTS: The values of ocular, corneal and internal spherical aberrations before and after mydriasis on the 4-mm diameter pupil zone were not statistically significantly different. On the 6-mm diameter zone, the ocular and internal spherical aberrations were statistically significantly different (p = 0.025, p = 0.002, respectively, paired t-test). However, the corneal aberrations did not show significant changes. The internal aberrations average before mydriasis was -0.043 (+/-0.21) microm and was shifted in a negative direction to -0.093 (+/-0.17) microm after mydriasis. The ocular aberrations average also changed toward negative after mydriasis. The high-order aberrations and astigmatism did not change significantly. CONCLUSIONS: The ocular and internal spherical aberrations changed toward negative with mydriasis in the participants' eyes suggesting the change of the ocular spherical aberration to be attributed to internal changes.

Adie's tonic pupil usually consists of mydriasis, defect in accommodation. Poor or absent light reflex, and tonic near reflex, It is usually unilateral, but rarelY observed bilaterally. The pupillary abnormalities often accompany areflexia, which is called Adie syndrome collechvely, Other autonomic dysfunction especially hypohidrosis. Are rarely reported in association with Adie syndrome. We recently experienced 2 cases of bilateral Adie's pupils in 2 women, one of whom had hypohidrosis, and the other had areflexia. So we report 2 cases with bilateral Adie's pupils with a review of literature.
