This is a case report of subcutaneous mycosis presenting
as a non-healing left calf ulcer in an immunocompromised
patient. Traumatic inoculation of the causative agent is the
most likely route of infection. The diagnosis requires a
detailed history and high clinical suspicion, confirmed by
histopathological examination. The management requires a
multidisciplinary team approach involving surgeon,
pathologist, physician sub-specialised in infectious disease,
wound care nursing team as well as social support services.
The literature review recommended that the treatment of
choice for such infection is surgical debridement in addition
to optimal antifungal therapy.

6.Mistaken identity: Eumycetoma masquerading as Squamous cell carcinoma

We report a case of eumycetoma in a Filipino patient who presented with a solitary reddish brown, moist, multinodular tumor on the
dorsum of the left foot of 2 years duration. Biopsy with Periodic acid Schiff (PAS) & Gomori methenamine silver (GMS) staining, fungal
culture, ultrasound and X-ray of the foot were done in our institution which confirmed the diagnosis of eumycetoma. The patient was
successfully treated with itraconazole 400/day for 3 months, followed by 200mg/day for the succeeding 9 months, leading to complete
resolution of the lesion leaving an atrophic hypopigmented scar. A high index of suspicion supported by diagnostic tests aided in the
early detection of the disease which also resulted to complete resolution of the disease.

Actinomycosis is an infrequent chronic infection regarded as the most misdiagnosed disease by experienced clinicians. The Office of Rare Diseases at the National Institute of Health has also listed this disease as a “rare disease.” This article presents a case report of actinomycosis of the alveolus with unusual clinical features but a successful resolution. It also states the importance of biopsy of deceptive inflammatory lesions that do not respond or recur after conventional treatment modalities.
