1.Muscle Weakening and Change of Tension According to Degree of Superior Rectus Z-Myotomy in Rabbits.

PURPOSE: To identify the muscle weakening effect and the change of muscle tension according to degree of superior rectus Z-myotomy in rabbits. METHODS: After dissection of superior rectus in 15 rabbits (30 eyes), marking was done on 10 mm apart from muscle insertion. Then, Z-myotomy was done on 2 mm and 8 mm apart from muscle insertion which are two different positions on apposite sides. 25%, 50% and 75% Z-myotomy were defined as group 1 (10 eyes), group 2 (10 eyes) and group 3 (10 eyes). After Z-myotomy, the change of muscle lengthening between muscle insertion and marking, and muscle tension were measured. At 4 weeks after Z-myotomy, all parameters were re-measured. RESULTS: After Z-myotomy all markings moved posteriorly, and showed result as 10.83 +/- 0.13 mm in group 1, 11.02 +/- 0.17 mm in group 2 and 12.01 +/- 0.23 mm in group 3 respectively. Muscle tension result showed as 3.98 +/- 0.22 mm in group 1, 3.54 +/- 0.18 mm in group 2 and 2.87 +/- 0.25 mm in group 3. In comparison of three groups, group 2 and group 3 showed the significant results (p < 0.05). At 4 weeks after Z-myotomy the markings showed result as 10.55 +/- 0.14 mm, 10.85 +/- 0.20 mm, 11.91 +/- 0.14 mm respectively, and in group 2 and group 3 the significant changes were seen (p < 0.05). Muscle tensions were 4.01 +/- 0.31 mm, 3.88 +/- 0.53 mm, 3.12 +/- 0.42 mm respectively. There were significant results in group 2 and group 3 (p < 0.05). CONCLUSIONS: 75% Z-myotomy of superior rectus showed significant results in muscle weakening and decreasing muscle tension.

PURPOSE: To experimentally investigate the effect for muscle weakness after superior rectus Z-myotomy and histological changes. METHODS: Superior rectus muscle fibers of rabbits (16 rabbits, 32 eyes) were cut transversely with scissors across 75% of the muscle in two different positions on opposite sides. In group 1 (16 eyes), myotomies were performed at 2 and 7 mm from the muscle insertion (5 mm gap) and in group 2, performed at 2 and 5 mm (3 mm gap). The change of mark, eyeball position, and muscle tension after myotomy and 4 weeks postoperatively was evaluated, the location of the mark was examined, and muscle tissue biopsy was performed. RESULTS: After Z-myotomy, the marks of the two groups moved significantly posteriorly from insertion within groups (p<0.05), with no significant differences between groups (p=0.469). Eyeball positions of the two groups moved significantly inferiorly (p<0.05); the amount of position change of group 1 was greater than group 2 (p<0.05). When the globe was pulled in opposite directions for muscle action, the degree of change decreased with significant difference within groups (p<0.05), but there were no significant differences between groups (p=0.32). CONCLUSIONS: Z-myotomy of the superior rectus muscle affected the recession of eyeball position and weakened the muscle action. Muscle weakening affected by the different gaps between myotomies did not show consistent results.

3.Pharmacologic Effect of Phentolamine on Norepinephrine Induced Contraction of Corpus Cavernosum.

An in vitro pharmacologic study was conducted to investigate the effect of phentolamine on norepinephrine induced contraction of human corpus cavernosum. The isometric muscle tension of corpus cavernosum from 4 potent volunteers were recorded after stimulation with various concentrations of norepinephrine and phentolamine. The results are summarized as follows. Contractile response of corpus cavernosum was observed to begin in the concentration of 10(-6) M and to reach maximal level in the concentration of 10(-4)M norepinephrine. Compared to it, contractile activity of corpus cavernosum to norepinephrine was observed to be gradually decreased in response to pretreatment with phentolamine from 10(-6)M to 10(-4)M. These indicate that norepinephrine causes a dose dependent contraction of corpus cavernosum and phentolamine had a relaxant effect on norepinephrine-induced contraction of corpus cavernosum in dose dependent manner.

7.The Changes in the Cornea and Anterior Chamber after Lateral Rectus Muscle Recession in Intermittent Extropia.

PURPOSE: To investigate changes in corneal keratometry and the anterior chamber in intermittent exotropia after lateral rectus recession using Pentacam. METHODS: Corneal keratometry, anterior chamber depth, and anterior chamber volumes were evaluated preoperatively and at 1 week, 1 month, and 2 months postoperatively using Pentacam in 20 patients (40 eyes) who underwent lateral rectus muscle recession. The IOP was measured before and after lateral rectus muscle detachment during the surgery. According to the change in IOP, the patients were divided into 2 groups; Group 1 (DeltaIOP < 3 mm Hg) and Group 2 (DeltaIOP > or = 3 mm Hg), and the results of the 2 groups were compared. RESULTS: The horizontal keratometry and anterior chamber volumes significantly decreased 1 week postoperatively, although there were no significant differences in vertical keratometry, mean keratometry, or anterior chamber depth. Additionally, there were no significant changes in any measurement parameters at 1 month or 2 months postoperatively. In Group 2, horizontal keratometry, mean keratometry, and anterior chamber volumes showed significant reduction compared with those in Group 1. There were no significant differences between the 2 groups at 1 month postoperatively. CONCLUSIONS: Postoperative corneal keratometry and anterior chamber volumes did not change significantly compared to preoperative values in intermittent exotropia after performing lateral rectus muscle recession. However, the higher the ocular muscle tension, the greater were the changes in corneal astigmatism and anterior volumes.

8.Evaluation of Anterior Knee Pain.

Anterior knee pain is an enigmatic entity with multiple causes, and so the patients, who present with diverse symptoms, require different treatments. A combination of variables, including abnormal lower limb biomechanics, soft-tissue tightness, muscle weakness and excessive exercise, may result in increased cartilage and subchondral bone stress, patellofemoral pain and subtle or more overt patellar maltracking. Because of the multiple forces affecting the patellofemoral joint, the clinical evaluation and treatment of this disorder is quite a challenge. Our extensive search of the literature revealed no single gold-standard test/maneuver to diagnose this disorder, and the reliability of the maneuvers described was generally low or untested. Making a specific and accurate diagnosis of the etiology, based on a thorough physical exam and imaging studies, is essential to create an effective therapeutic approach for the patient's successful return to proper knee function.

9.A Psychologic Study in Tinnitus Disorder:CMI and SCL-90-R.

It has been assumed that tension and anxiety serve to induce or exacerbate tinnitus through increasing muscle tension or alteration in blood flow to the cochlear region. And it is also possible that psychological distress may be a result of tinnitus, or that awareness of tinnitus may be greater during environmental stress. So tinnitus patients need psychologic consideration in their diagnosis and treatment. Purpose of this study is to evaluate the degree and characteristic of the psychologic factors in tinnitus disorder. Cornell Medical Index(CMI), Fukamachi's Discriminative chart and Symptom Check List-90-Revision(SCL-90-R) were examed in tinnitus patients and control group. The results were as follows. 1) According to the Fukamachi's Discriminative Chart using CMI, the tinnitus group showed higher incidence than normal healthy adults group in class III or IV region which is regarded as neurosis. 2) The tinnitus group showed higher score than normal healthy group in all 9 sections of SCL-90-R. 3) The group which has long duration of tinnitus was related to high scores of interpersonal sensitivity, depression, anxiety, and the group which has not history of otologic surgery was related to high scores of depression, phobic anxiety(p<0.01).

10.A Case of Neurofibroma of the Vocal Cord

Solitary neurofibroma of the glottis is extremely rare and accounts for only 0.1–1.5% of benign laryngeal tumors. Aryepiglottic fold is the most frequent involved site followed by arytenoids and ventricular folds. There have been few reports of neurofibroma of the true vocal cord. We report a case of neurofibroma which was deeply embedded in the vocal cord and misdiagnosed as muscle tension dysphonia with a review of literatures.
