Differential diagnosis of Alzheimer's disease (AD) and vascular dementia(VaD) has an important bearing on the diagnosis and management of patients with dementia. This article provides a guideline for the differential diagnosis through 1) history taking, 2) neurological examination, 3) neuropsychological tests, and 4) neuroimaging studies. VaD consists of etiologically and clinically heterogeneous subtypes that include multi-infarct dementia (MID), single strategic infarct dementia, and subcortical vascular dementia. Patients with MID and single infarct dementia con be easily differentiated from patients with AD. However, clinical manifestations of subcortical vascular dementia can mimic those of AD, which may lead primary physicians to misdiagnose subcortical vascular dementia as AD. The issue of differential diagnosis is further complicated by the fact that many patients may have AD with concomitant VaD (mixed dementia).

BACKGROUND: This study aimed to identify the association of compliance with sick role behavior with satisfaction of basic psychological needs, and to assess the influence of compliance with sick role behavior among hemodialysis patients. METHODS: Participants included 109 patients from outpatient dialysis clinics at 6 major general hospitals located in urban areas in Korea. Data for compliance with sick role behavior and basic psychological needs were collected through a self-reported questionnaire and was analyzed with independent t-test, one-way analysis of variance, Pearson' correlation analysis, and multiple stepwise linear regression analysis. RESULTS: This study showed positive correlations between compliance with sick role behavior and satisfaction of basic psychological needs (r=0.59, P<0.001). The variables predicting compliance with sick role behavior were relatedness (beta=0.28), autonomy (beta=0.29), competency (beta=0.30), and age (beta=0.24). These variables accounted for 38.2% of the variance of compliance with sick role behavior in hemodialysis patients. CONCLUSIONS: Our results indicated that it is necessary to improve compliance with sick role behaviorin order to increase satisfaction of basic psychological needs in hemodialysis patients.

It is well known the early diagnosis and early treatment is very important for the patient with congenital dislocation of the hip(CDH) to provide a favorable function in the whole life. However, despite screening programmes for the detection of CDH, children with dislocated hip continue to present late. While there has been general agreement as to the need for treatment of persistent acetabular dysplasia secondary to CDH, the appropriate treatment has been a matter of controversy for many years, and the surgical treatment of CDH in old children presents a more formidable technicalchallenge than in younger child. The author studied 26 cases in 23 patients of CHD aged between 6 years and 13 years at the Department of Orthopedic Surgery of Yonsei University during a 6 year period from January, 1979 to December, 1984. The aim of this study was to find out available surgical treatment for the old childhood with CDH and to assess the comparative results of age, degree of acetabular dysplasia and the station of femoral head displacement. The results of this study are are as follows; l. Among the 23 patients, female was 20 patients, male was 3 patients and 3 female patients show bilateral involvement. And among 26 cases, left hip involvement was 21 cases and right hip involvement was 5 cases. 2. Age at initial operation was 8 years and 10 months in average ranging from 6 years and 2 months to 13 years and 4 months, and average follow-up time was 3 years and 11 months ranging from 1 year 6 months to 5 years 2 months. 3. Attended type of surgery of the CDH in old childhood were 4 cases of Salter's innominate osteotomy, 5 cases of Sutherland's double innominate osteotomy, 5 cases of Salter's innominate osteotomy with femoral shortening and 12 cases of Klisic procedure. 4. Age of the patient as well as degree of acetabular dysplasia and station of femoral head displacement may influence on determination of type of the surgical procedure. 5. The older the age at initial operation and the more the displacement of femoral head, the results was less favorable. 6. As in young childhood, the clinical results of treatment are invariably better than the radiological results. 7. Overall final results of leg length discrepancy was 1.0cm in length and range of leg length discrepancy differs from type to type of surgical procedure. 8. Complications developed after treatment were avascular necrosis of femoral head in one case, fracture of the ipsilateral femure in 3 cases and wound infection in 1 case.

A benign chondroblastoma of bone which involved the proximal end of the right tibia of a 14 year old girl was presented. Clinical symptoms, signs, roentgenographic findings, and bone biopsy led to the diagnosis. The lytic lesion of bone, which was 4cm×3cm×2.5cm in size, was curetted and autogenous bone graft was done. Most of the lesion was composed of a dark bluegray substance, which was either friable or even hemorrhagic, and the other areas were more grayish, gritty and flecked with yellowish calcified material. The postoperative course has been satisfactory, showing no recurrence for a period of 10 months after surgery.

Alveolar soft-part sarcoma has been originally described as a unique pathological entitiy by Christopherson, Foote, and Stewart in 1952, who have chosen its name because of their uncertainty in determining from which cells the tumor arose. The tumor is generally considered to be malignant and originates in soft tissue, the most commonly in the deep tissues of the extremities. The case, reporting here, illustrated several of the recognized characteristics of alveolar soft-part. sarcoma, both clinically and pathologically and is described in view of its rarity of its kind and difficulty of accurate diagnosis. The patient was a 29 year old male who has complained of a adult. fist-sized mass on posterolateral aspect of left arm for one year. Roentgenographic findings showed multiple metastatic lesions on both lung fields, left humerus, skull and cervical spine. The mass on the same site recurred after excision and hemoptysis probably due to aggravation of the metastatic lesions of the both lungs developed 4 months later. He died 8 months after surgery.

PURPOSE: The purpose of this study was to identify the factors influencing drinking problems in female university students to provide basic data for a nursing intervention program to improve health and prevent drinking problems. METHOD: Data were collected from September 17 to 21, 2007 through a questionnaire survey of 325 female college students in G City. Collected data were analyzed using descriptive statistics, Pearson correlation coefficients and stepwise multiple regression. RESULT: The most powerful predictor of drinking problems was refusal self efficacy. The combination of the influence of friends, perceived stress, personality of novelty seeking, and alcohol expectancy accounted for 23.7% of the variance in drinking problems. CONCLUSION: From the results, I recommend that refusal self efficacy, influence of friends, perceived stress, personality of novelty seeking, and alcohol expectancy should be contained in developing nursing intervention programs for preventing drinking problems in female university students.

Pancreatic Pseudocyst*

The first myelography with air was carried out by Walter Dandy in 1919, and the myelography with positive contrast medium was done by Sicard and Forestier in 1922. Since Pantopaque was adopted as a contrast material by University of Rochester group in 1944, it has been rapidly popularized. Although higher diagnostic accuracy has been made with a development of the diagnostic aids such as electromyography, discography, epidural venography and computerized tomography, myelography has been popularized in our country with electromyography. The authars have seen and studied 107 cases of herniated lumbar disc with respect to the diagnostic accuracy of myelographic examination from January, 1976 to December, 1981 at the department of Orthopedic Surgery, Yonsei University College of Medicine. The results were as follows: l. Of the 107 cases, 59 were found in the age of 20 to 40 years and the ratio of male and female was 2:l. 2. Fifty eight cases (54.2%) had no history of back injury, but 40 cases (37.4%) had sprain of the back and 9 cases had direct trauma history. 3. In all cases, plain X-ray and myelogram were taken. Among them 55 cases showed narrowing of intervertebral space in plain film, and 100 cases showed positive myelographic findings such as lateral indentation of dural sac (41 cases), hour-glass defect (30 cases), root sleeve defect (12 cases), block defect (10 cases) and combined defects (7 cases). 4. Sixty one cases which confirmed by operation, the most frequent level of disc herniation was in L4-5 space. Clinical diagnosis was correct in 57.4%, and myelographic accuracy was coincided with operative finding in 80.3%. 5. Twelve (19.7%) of the 61 cases showed discrepancy between myelographic and operative findings, and these 12 cases consisted of 5 cases of level discrepancy, 3 cases of false positive and 4 cases of false negative.
