1.Clinical Study on Early Diagnosis of Skeletal Disorders by Bone Scan with Tc-99m-MDP (Medronate)

Early detection of skeletal disorders was very important role in establishing therapeutic modality and prevention of complications. Since Tc-99m-phosphates derivatives used in bone scan agent, remarkable improvement was noted in effectiveness, cost, radiation hazard to patient and image quality than previously used agents such as strontium or fluoride. Bone scan with Tc-99m-MDP was done in 155 cases for early diagnosis of skeletal disorders at Severance Hospital, College of Medicine, Yonsei University from August, 1980 to May, 1982 were analyzed both clinically and radiologically, and the following results were obtained. l. Among 19 cases of avascular necrosis, positive cases were 17 (89.5%) in bone scan and 6(31.6%) in X-ray. Positive pattern could be classified into 3 types and the radionuclide uptake in the femoral head was decreased in 3 cases which was negative in X-ray and duration of their symptoms was within 4 months. 2. Fifteen among 17 hip fractures or dislocations were positive in bone scan. In 12 (80.0%) among 15 cases, complications were revealed and positive cases in X-ray were 7(58.3%), which were all positive in bone scan. 3. In 20 bone scans for early diagnosis of acute osteomyelitis, positive in bone scan were 17 and 14 (82.4%) of 17 cases were finally diagnosed as acute osteomyelitis. Among 15 cases diagnosed as acute osteomyelitis, positive in X-ray were 8(53.3%) and sensitivity of acute osteomyelitis in bone scan was 93.3%. 4. In 25 cases of the suspected infection of the joint, 15 were positive in bone scan and among them, cases diagnosed as pyogenic arthritis were 12 (80.0%) and positive in X-ray were 2 (16.7%). Only one of 10 cases of transient synovitis of the hip joint was positive in bone scan. 5. In rheumatoid arthritis, 13 of 15 cases were positive in bone scan and 10 among them were revealed the diffuse uptake of the radionuclide in the joint. In degenerative arthritis, 7 of 8 cases were positive in bone scan and they were revealed the focal uptake of the radionuclide in the same site where the bone was changed in x-ray. 6. Among 28 cases showed spine fracture in X-ray after trauma, 15 (53.6%) were positive in bone scan. In spine fracture, it was difficult to differentiate fresh fracture from old fracture with bone scan. The positive duration of bone scen in fracture of weight bearing bone was longer than in spine fracture. Rib fracture which was not found in X-ray could be diagnosed with bone scan.

6.Transient Right-sided Heart Failure after Percutaneous Transluminal Angioplasty (PTA) of Membranous Obstruction of Inferior Vena Cava: A Case Report.

We experienced a case of transient right-sided heart failure after angioplasty of membranous obstruction of the inferior vena cava confirmed by sonography and an inferior vena cavogram. Angioplasty involved the use of a self-expandable metallic stent, but after successful recanalization of the obstruction , the patient became dyspneic. Chest radiography revealed mild cardiomegaly with pulmonary congestion, but this was resolved spontaneously. For the prevention of serious heart failure, we recommend preprocedural evaluation of cardiac function.

7.A Case of Open - lip Schizencephaly.

Schizencephaly, first described by Yakovlev and Wadsworth in 1946, is a rare congenital anomaly characterized by gray matter-lined clefts that extend through the entire cerebral hemisphere, from the ependymal lining of the lateral ventricle to the subarachnoid space. Clinically, patients with schizencephaly present motor dysfunction such as hemiparesis, seizures, and variable developmental delay. Absence of septum pellucidum, gray matter heterotopia, polymicrogyria, and hypoplasia of optic nerves were also frequently found to be associated with schizencephaly. We experienced a case of open-lip schizencephaly in a one-day-old male infant with wide, tense fontanel, which was confirmed by MRI.

8.Pulmonary Diffusing Capacity in Patients with Liver Cirrhosis.

About a third of the patients with decompensated liver cirrhosis have reduced arterial oxygen saturation and are sometimes cyanosed in the absence of any apparent lung or heart disease; There is a reduction of diffusing capacity without a restrictive ventilatory defect. The aim of this study was to determine diffusing capacities in patients with chronic liver- diseases. The diffusing capacities and arterial oxygen saturations were measured in 25 patients with chronic active hepatitis(CAH), 9 early cirrhotics (early LC), 36 cirrhotics(Child's class A) and 11 cirrhotics(Child's class B). The anterior tibial area was observed for pitting edema, and Thallium-201 test per rectum(shunt index) was done. Hypoxemia was not observed in all subjects. The number of cases with decreased pulmonary diffusing capacity (DLco) is 3/25(12.0%) for CAH, 3/9(33.3%) for CAH with early liver cirrhosis(LC), 17/36(47.2%) for LC(Child's class A) and 6/11(54.5%) for LC(Child's class B). The mean+/-standard deviation of Dlco(% predicred) are 93.1+/-12.1 for CAH, 85.7+/-12.3 for CAH with early LC, 82.2+/-14.7 for LC(Child's class A) and 80.4+/-6.9 for LC(Child's class B), There is a significant difference between DLco in CAH and that in LC(Child's class A)(p<0.01). Patients with higher shunt index(>0.3) had significantly lower DLco than these with lower shunt index(<0.3)(76.4+/-9.7% vs. 89.3+/-13.3%)(p<0.01). The DLco was also lower in patients with pitting edema(77.3+/-10.2%) than in those without pitting edema(85.5+/-13.8%) (p<0.01). These results summarized that the DLco was low in patients with cirrhosis and with higher shunt index(>0.3) or pitting edema. This may be due to an increased systemic blood flow shunt and an increased generalized interstitial edema. Pulmonary function tests including diffusing capacity may be useful as prognostic parameters in patients with chronic liver disease, especially in those with CAH or early LC.

9.The Results of Modified Brostrom Procedure for the Treatment of Chronic Lateral Ankle Instability.

PURPOSE: This study was performed to evaluate clinical and radiographical results of treatment of chronic lateral ankle instability with modified Brostrom procedure. MATERIALS AND METHODS: This is a retrospective study of 22 cases in 22 patients treated with modified Brostrom procedure under the diagnosis of chronic lateral ankle instability from May 2000 to August 2004. Average age was 32.3 years. Average follow-up period was 23 months. Preoperative and postoperative radiographs of ankle anteroposterior view, lateral view and varus stress view were analyzed. The clinical evaluation was performed according to the Americal Orthopaedic Foot and Ankle Society scales. RESULTS: After the modified Brostrom procedure, Americal Orthopaedic Foot and Ankle Society scales score was improved by average 28 points. Among the 22 cases, 13 cases were excellent, 3 cases good, 4 cases fair, and 2 cases poor with satisfaction of 73%. Five cases had intermittent ankle pain of whom 3 cases had difficulty during competitive sports activity. Lateral tilting of talus improved by average 5.4 degrees on varus stress view. CONCLUSIONS: The modified Brostrom procedure is one of the most effective methods for treating of chronic lateral ankle instability.

10.Benign Prostatic Hyperplasia: Its Feature and Impact on the Quality of Life in Rural Area of Kangwon Province.

PURPOSE: We studied 250 men who are over 50 years of age to evaluate the feature of benign prostatic hyperplasia and its impact on quality of life in rural areas of the Korea. MATERIALS AND METHODS: Two hundred fifty men participated in this study. Urinalysis, urine culture, uroflowmetry, digital rectal examination and serum PSA were examined on all the participants. Transrectal ultrasonography(TRUS) were performed in 234 men. RESULTS: Of these 250 men, symptom prevalence was 78.71%. The age distributions of 50-59, 60-69, 70-79 and > or =80 years of age were 91, 111, 43 and 5 men respectively(mean 62.4). The mean prostatic volumes for 50-59, 60-69, 70-79 and > or =80 years of age group were 27.33cm3, 29.95cm3, 28.89cm3 and 33.55cm3. Of these, its volume difference was not statistically significant. Of 250 men, mildly symptomatic group(IPSS< or =7) was 22.5%, moderately symptomatic group(8< or =IPSS< or =19) 43%, and severely symptomatic group(IPSS> or =20) 34.5%. Severely symptomatic group showed higher mean age(p=0.0478), lower maximal flow rate(MFR)(p=0.024) and average flow rate(AFR)(p=0.003). Mildly symptomatic group showed smaller prostatic volume(p=0.013). Mean quality of life score for mildly, moderately, and severely symptomatic group were 2.09, 3.50, and 4.51 points, the score of which showed statistical difference(p<0.001). On comparison between the group of MFR<15ml/sec and the group of MFR> or =15ml/sec, statistical difference for the mean age (p<0.001), voided volume (p<0.001), QOL (p=0.02) and symptom score (p=0.002) was found. But no difference was found in prostatic volume(p=0.261). MFR, AFR and voided volume did not show significant correlation with prostatic volume. But serum PSA level was strongly correlated with prostatic volume. The AFR was a greatest determinant to symptom score in multiple regression analysis. CONCLUSIONS: The symptom prevalence in rural areas of the Korea is not any less than that of western society. This study shows that it too has a great impact on the quality of life in rural Korea. The prostatic volume itself had no significance on symptom score nor uroflowmetric parameters. The uroflowmetric parameter such as MFR is more important in evaluating symptomatic patients than prostatic volume itself.
