1.A Clinical Study of the Carpal Tunnel Syndrome

Carpal tunnel syndrome is most commonly applied to compression neuropathy of the median nerve at the wrist, and is the most cause of buring pain, numbness and tingling in the thumb, index and long fingers, and in the lateral half of the palm, with or without progressive atrophy of the thenar muscles. Fifteen symptomatic hands of 13 patients of the carpal tunnel syndrome managed and operated at the Department of Orthopedic Surgery Severance Hospital from Jan. 1972 to Mar. 1982 were analized in clinical aspect. l. Among 13 patients, 3 were male and 10 were female. 2. Nine patients (69.2%) were affected on dominant hand. 3. Average duration of the symptoms was 17 months. 4. In electromyographic study, conduction delay of the motor fiber of the median nerve was found in 11 cases(73.3%) and sensory fiber in 13 cases(86.6%). Fibrillation of the thenar muscles was found in 11 cases(73.3%). 5. Ten patients, 11 cases were followed up from 11 months to 9 years and 11 months and 72.8% was improved.

2.A Case of Extra-Adrenal Pheochromocytoma with Abdominal Colic and Paroxysmal Hypertension.

Pheochromocytomas can arise wherever chromaffin cells are found, and most of them(90%) are in one or both adrenal glands. But they may be located anywhere along the sympathetic chain and rarely in aberrant sites. One of the common extra-adrenal sites from which these tumors can arise is a collection of para-aortic and para-ganglion cells around the origin of the inferior mesenteric artery. We experienced a case of extra-adrenal pheochromocytoma located at the organ of Zukerkandl in a 21-year-old female patient with abdominal colic and paroxysmal hypertension, whose symptoms and blood pressure returned to normal after successful surgical excision of the tumor.

7.Combined Chemoradiation of Advanced Pancreatic Cancer.

PURPOSE: This study was designed to evaluate the survival rate and prognostic factor of patients with advanced pancreatic cancer who received chemoirradiation. MATERIAL AND METHODS: From March 1993 to November 1995, twenty patients with unresectable pancreatic cancer were treated at the Department of Therapeutic Radiology, St Mary's Hospital, Catholic University Medical College. There were 11 men and 9 women. Age at diagnosis ranged from 34 to 75 years. All patient were treated according to a protocol consisting of 40 Gy external radiation by split course concomitant with intravenous 5-fluorouracil (5-FU) 500 mg/m2 given in a bolus injection 4 hours before radiatian on each of the first 3 days of each treatment course. Among them, 5 patients received incomplete radiotherapy. The follow-up period ranged from 1.3 to 29 months. RESULTS: In all the patients, median survival is 5.0 months and one and two-year overall survival rate was 34.3% and 25.8%, respectively. Median survival was 9.0 months and one-year survival rate was 33.3% in 15 patients with complete radiotherapy. The significant prognostic factors were stage, tumor location, and completion of chemoradio- therapy(p < 0.05). CONCLUSION: A combination of radiotherapy and chemotherapy resulted in improved median survival. However, the significant prognostic factars affecting survival rate in this analysis need to be verified further through randomized trial.

8.A Survey on the Interval of Post-polypectomy Surveillance Colonoscopy.

BACKGROUND/AIMS: Colonoscopy is the golden standard used as a surveillance test and screen for colon cancer, and the current demand for colonoscopy exceeds its availability. This study is an assessment of the colonoscopic surveillance intervals currently practiced. METHODS: A multiple choice survey of the colonoscopic surveillance interval used in six case scenarios [hyperplastic polyp; two 0.5 cm tubular adenomas (TAs); a 1.5 cm TA; 0.8 cm triple TAs; a 1.5 cm TA with high grade dysplasia; current normal exam after polypectomy of a <1 cm sized TA 3 years ago] was sent via e-mail to members of the KASID. RESULTS: A total of 131 colonoscopists (104 men, 27 women) replied, and the mean age of the respondents was 36 years (range 28~58). All respondents were board- certified in their respective specialties (internal medicine 75, general surgery 3, and GI subspecialty 53). When compared with the AGA guidelines, 90.1~99.2% of the respondents performed the first post-polypectomy surveillance colonoscopy prematurely, and 75.6% of respondents performed the second surveillance prematurely. CONCLUSIONS: Most post-polypectomy surveillance colonoscopies were performed prematurely. It is quite possible that unnecessary surveillance may account for a significant portion of the demand for colonoscopy.

10.Respiratory Mechanics of Young and Elderly under Enflurane Anesthesia.

BACKGREOUND: The aim of this study was to compare the respiratory mechanics between young and elderly during enflurane anesthesia using flow-interruption technique. METHODS: Fourteen patients were divided into 2 groups; for young aged group (Group Y)(24.9 4.9 years) and old aged group (Group O)(73.4 5.6 years). After anesthetic induction, endotracheal intubation was performed with Univent tube, and then tracheal pressure was measured at the distal end of the tube. 1.5 vol% of enflurane and 0.15 mg/kg of vecuronium were administered to maintain anesthesia. IMV (intermittent mandatory ventilation) was applied with Siemens Servo 900C ventilator and anesthetic gases were supplied via low pressure inlet of the ventilator. Tidal volume (10 ml/kg) was fixed during measurements for each patient. After stabilization of vital signs, respiratory parameters were measured before surgery. End-inspiratory occlusion was applied at least 3 seconds. Pressure, flow and volume were monitored and recorded with Bicore CP100 pulmonary monitor. The data were transfered to a PC and analyzed by a processing software. Total respiratory (Rrs), airway (Raw) and tissue (Rve) resistances, and dynamic (Cdyn) and static (Csta) compliances were calculated. Mann-Whitney U test was used for statistical analysis. RESULTS: There were no statistically significant differences for the Cdyn and Csta values between the two groups. But the values of Rrs, Raw and Rve were higher in the Group O than the Group Y. CONCLUSION: During enflurane anesthesia, the values of Rrs, Raw and Rve were higher in the Group O than the Group Y. To elucidate the exact causes of the above results, further studies are needed.
