Poland's syndrome which is associated with thoracic anomaly and ipsilateral syndactyly is not hereditary and is of unknown origin. It was described first Alfred Poland in 1841 year. This syndrome affects males more frequently than females. We have experienced two cases of Polands syndrome who were all male and each case revealed thoracic anomalies; abscent pectoral muscles and anterior axillary fold, pectus excavatum, and ipsilateral syndactyly with aplasia or hypoplasla of the middle phalanx.

3.Comparison of Mepivacaine and Bupivacaine as an Adjuvant of Morphine for Benign Anorectal Surgery under Caudal Anesthesia.

BACKGROUND/AIMS: The caudal anesthsia is most commonly used for benign anorectal surgery, The combination of long-acting anesthetics and opiates has been used for longer duration and successful control of postoperative pain. But the side effects of peridural anesthesics and morphine have commonly occured in caudal anesthesia. This study was performed to assess the difference in clinical effects between peridural mepivacaine and bupivacaine with morphine. METHODS: We evaluated the clinical effects in 60 patients who had anal operation with Jack-Knife position under caudal anesthesia. We divided randomly these 60 patients into two groups, M and B groups (in each group, 30 patients included). Group M (n=30) was given 2% mepivacaine 20 ml with morphine 2 mg caudally, and Group B (n=30) was given 0.5% bupivacaine 20 ml with morphine 2 mg in the same manner. We measured the onset time, duration, postoperative analgesia, and side effects including urinary retention. RESULTS: The onset time for analgesia was significantly shorter in group M than in group B. The duration of postoperative pain complaints was significantly longer in group M than in group B. The postoperative analgesic effects and side effects were not significantly different between two groups. CONCLUSIONS: Caudal mepivacaine and morphine mixture is effective for control of postoperative pain without significant side effects.

4.Ureteroappendicocutaneostomy(Appendix conduit): 4 Cases.

Cutaneous urinary diversion with intestinal conduits after cystectomy in bladder cancer has been popular. However, because of loss of continence and storage capacity of the lower urinary tract, the situation of patients with an ileal conduit was far from satisfactory with respect to quality of life. Recently, continent urinary diversion has begun to achieve worldwide popularity as Kock, Mainz, or Indiana pouch techniques have been more feasible and accessible, but these techniques have been known as time consuming procedures. In high risk patients with bladder carcinoma more simple techniques of urinary diversion is needed. Herein, we report the outcome of ureteroappendicocutaneostomy (appendix conduit) with radical cystectomy performed in 4 bladder carcinoma patients in whom unilateral nephroureterectomy was necessary or previously done.

5.A Case of Transient Acantholytic Dermatosis.

We present a case of transient acantholytic dermatosis which developed on the face of a 19-year-old Korean girl. Asyrnptomatic pinhead to miliary-grain sized brownish papules arranged in a somewhat band-like outbreak along the left side of the nose for about 3 months. Histopathologic findings were similar to Dariers disease. Immunoperoxidase staining showed that IgG was deposited on the intercellular area of keratinocytes around the suprabasal clefts. Skin lesions disappeared spontaneously in 5 weeks after a skin biopsy.

6.Embryologic Discission of the Median Raphe Cyst: Two Cases Report.

Median raphe cyst is known as congenital lesion of the perineum and genitalia, but its etiology is unclear. Most investigators believe that the median raphe cyst represent defects in the embryologic developenient bf the male genitalia. Simple surgical excision is effective in most cases. We report our experience with two cases of median raphe cyst without specific symptoms. Even though median raphe cyst is asymptomatic, surgical therapy is worth applicable because it relieve a patient from cosmetic and psychotic problem.

7.Too many ducts sign: a characteristic cholangiographic finding of clonorchiasis?.

Clonorchiasis procucts diffuse dilatation of the small and medium sized intrahepatic bile ducts and its cholangiogram shows visualization of many bile ducts, especially, tertiary, quaternary, and more peripheral tributaries up to the 6th tributaries. In an attempt to clarify this cholangiographic sign quantitively, we counted the visualized smaller bile ducts in clonorchiasis and compared the number of visualized ducts in normal cholangiogram, recurrent pyogenic chlangitis and carcinoma of the extrahepatic ducts. In clonorchiasis the number of visualized smaller bile ducts was considerably geater than in normal subjects and recurrent pyogenic cholangitis, but there was no singnificant statistical differences in the number of visualized bile duct tributaries between clonorchiasis and carcinoma of the bile ducts. Thus it is considered that too many ducts sign is not a unique cholangiographic finding of clonorchiasis, but we believe that in the presence of this sign with other we l known cholangiographic findings, diagnosis of clonorchiasis is very easy.

8.Histiocytosis X Case Report.

Eosinophilic granuloma, Hand-Schuller-Christian and Abt-Letterer-Siewe syndrome are known to be the same disease entity involving the reticulohistiocytic system of the body, although their clinical manifestations are different. So they were included under the term of Histiocytosis X and this concept has been generally accepted. The authors have experienced one case of Hand-Schuller-Christian disease and that of eosinophilic granuloma. The former was a 23 years old man with complaints of exophthalmos, polyuria and a tender palpable mass on the right parietal area and in the latter a soft pulastile mase located on the left parietal area in a 3 year old boy. There was no neurological deficits on their admission. We managed them with radiation and steroid therapy following surgical curettage.

9.A Case of Traumatic Spinal Epidural Hematoma in Ankylosing Spondylitis.

A case of traumatic thoracolumbar epidural hemorrhage is presented in a 37-year-old man with ankylosing spondylitis. The patient complained neck and back pain right after the accident but paralegia and difficult urination followed 2 days later. Spine CT showed thoracolumbar epidural mass, which was confirmed to be hemorrhage associated with spine fracture and was solely responsible for paraplegia.
