1.A Case of Liposarcoma

A case of soft tissue liposarcoma developed on the calf region of right lower leg is reported. The case was a 2 year-old girl who complained marked swelling, pain and tenderness over the posterior aspect of the right leg for 4 months. Roentgenological study revealed marked enlargement of the soft tissue in calf region with marked periosteal elevation and pressure erosion of tibia and fibula. Above knee amputation of the affected leg was performed. The mass measured 10 cm. × 9 cm. in size and cut surface appeared to be white gray colored and central necrosis which was surrounded with fatty tissue. The diagnosis was confirmed as soft tissue liposarcoma (Pleomorphic type) by microscopic examination.

2.Congenital Coxa Vara: Report of Two Cases

Congenital coxa vara is an uncommon dysplasia, not evident at brith, which first noticed when the child begins walking. Radiographs of the proximal femur include a decreased neck shaft angle, a wide and vertically aligned physis, an irregular metaphyseal ossification, a shortened femoral neck, a triangular osseous fragment adjacent to the inferior margin of the physis, a normal but osteoporotic femoral head, and secondary degenerative changes of the hip joint in neglected case. Prompt diagnosis and early management can reduce severe deformity and degenerative changes of the hip. Authors had been treated two cases of congenital coxa vara with valgus osteotomy followed by good results.

6.Triple Arthrodesis: Review of 92 feet

Triple arthrodesis is performed to obtain stable stature and walking, to eliminate pain, to correct the deformities of the foot. This is the result of 92 feet triple arthrodesis over a period from 1976 to 1991 at the Department of Orthopaedic Surgery, Yonsei University College of Medicine. The results are as follows 1. Mean age was 17.6 years, ranged from 10 to 51 years. 2. Of the total of 92 feet, sequelae of poliomyelitis was the most common etiology with 58 feet(63%). 3. Most common type of foot deformity was talipes equinovarus with 52 feet(56%). 4. Internal fixation was done with staple or Steinmann pin combined with autogenous bone graft. Among them, using of 2 staples was the most common fixation method, 47 cases(51%). 5. As necessary, triple arthrodesis was combined with tendo-calcaneus lengthening, plantar fasciotomy, posterior capsulotomy, modified Jones operation and tibiotalar arthrodesis. 6. Final results were ideal. in 22 cases(24%), good in 51 cases(56%), fair in 16 cases(17%) and fail in 3 cases(3%). 7. There were 2 cases of pseudoarthrosis and 1 case of residual equinus deformity after operation. In conclusion, triple arthrodesis was the efficient method of treatment for the deformed feet in skeletally matured patients.

7.Fractures of Lumbar Ring Apophysis

Fracture of the posterior lumbar ring apophysis or vertebral cartilaginous end plate is unusual, having previously been described only in adolescent males. In this report, we present 3 such cases involving 2 adolescent males and 1 adolescent female. All patients had a bony ridge or fragment projecting into the spinal canal, usually from the lower border of L4 vertebral body; a defect in the posteroinferior aspect of the vertebral body. Preoperative myelogram and computerized tomogram including plain X-ray appear to be helpful in demonstrating bone within the canal and preoperative diagnosis can facilitate selection of the appropriate surgical approach. The radiographic findings, including the myelogram and computerized tomographic appearance are detailed and the previous literatures are reviewed.

10.Neurologic Complication after Spine Surgery

We examined the rates of postoperative neurologic complications, as recorded in the medical record department in Severance Hospital from 1989 through 1991 for patients who had had operations on the spine. There were 538 hospitalizations for procedures on the spine; 327(60.78%) of herniated disc, 120(22.30%) of spondylolisthesis, 64(11.90%) of infection, 27(5.02%) of spinal stenosis were included. Neurologic complications oecurred in 12(2.23%) patients; 3 cases in herniated disc, 4 in spinal stenosis, 4 in spondylolisthesis, and 1 in infection. Regarding techniques of the operation, 8 complications occurred in arthrodesis, among these, 2 were in anterior approach and 6 in posterior approach. Four cases of the complications occurred in laminectomy and discectomy procedure. Complications were more frequent in the patients who had had spinal stenosis when arthrodesis was performed than decompression procedure only. Possible causes of neurologic complication were postoperative hematoma formation, translation of the vertebral column due to instability after anterior debridement and forceful traction of the nerve root during the operation.
