1.Two Case of Cutaneous Horn.

Cutaneous horn is a clinical decription for a cohesive mass of cornified mterial protruding from the surface of the skin. The incidence of cutancous horn is relaively rare and the lesion is usually single, The predilection sites are known to be the exposed area of the skin, that is, face, ear, dorsum cf hand etc. Soles are reported as the least common site of the cutaneous horn. Here, we report two cases of cutaneous horn. The one was a 49 year old schzophrenic male who had multiple cone shaped protrusions on hoth soles for 10 year. The other was 17 year old, boy who had a single cornified protrusion on the left forhead. The histopa.thologic findings from the base of two cases were consistent with benign hyperplastic epithelium.

Failure in the formation of a hymeneal orifice is a rare congenital malformation. There are many cases of imperforated hymen with hematocolpos in literature. However this condition associated with obstructive uropathy such as acute urinary retention is rare. We experienced a case of imperforated hymen with hematocolpos which caused acute urinary retention in a 16-year-old girl. Herein we report this case with brief review of literature.

5.A Case of Ureteral Fistula Misdiagnosed as Acute Rejection after Renal Transplantation.

The ureteral fistula is the most common urologic complication and it usually occurs within the first 3 weeks after renal transplantation. The dominant cause is damage to the arterial supply of the transplanted ureter by close dissection at time of donor nephrectomy. The signs and symptoms of ureteral fistula can mimmic acute rejection, which is oliguria, anuria, fever, wound tenderness, swelling and decreased renal function. It was well established that open intervention was standardized for such problem. Recently nonoperative percutaneous approaches to the placement of stents and tubes are used. Herein we report a case of ureteral fistula, misdiagnosed as acute rejection after renal transplantation, which was healed with Silastic drain insertion in a 22-year-old male recipient.

6.Control of Postpartum Bleeding by Rectal Misoprostols: A Report of 3 Cases.

Postpartum hemorrhage is an important cause of maternal mortality and morbidity. Especially uterine atony is the most common cause of postpartum hemorrhage. Conventional method to control postpartum uterine atonic bleeding is based on the use of oxytocin and ergot preparations. Prostaglandin F2alpha analogue such as carboprost can be used to promote contraction when these agents fail to produce uterine contraction. Prostaglandin E1 analogue, misoprostol has uterotonic effect by oral or vaginal administration. They are used to induce labor and first or mid trimester abortion. In postpartum uterine atonic bleeding, misoprostols cannot be used via oral or vaginal route. Recently we have experienced that postpartum uterine atonic bleedings unresponsive to conventional methods were controlled by rectal misoprostols. So we report these cases with a brief review of literatures.

7.A Case of Renal Hemangiopericytoma.

Hemangiopericytoma of the kidney is extremely rare, which is very much resembled a renal cell carcinoma. It tends to grow slowly but occasionally had demonstrated rapid infiltration and distant metastasis. The incidence and degree of its malignancy appears to be low. Herein we report a case of hemangiopericytoma which was developed from left kidney in 42 old man and treated with radical nephrectomy.

8.CT staging of real cell carcinoma:Emphasis on perinephric tumor extension.

A total of 47 preoperative CT scans in patients with renal cell carcinoma were retrospectively reviewed and compared with surgical findings to assess the accuracy of CT for determining the perinephric tumor extension. CT criteria for perinephric extension were hazy ill-defined tumor margin, perirenal nodule and fascial thickening. Regardless of the tumor stage, the accuracy of CT in detecting perinephric extension was 76.6%(36/47), with a sensitivity of 88.9% (16/18) and specificity of 68.9% (20/29). The cause of understaging (n=2) was microscopic infiltration of the perinephric space. The causes of overstaging were tumor infiltration to the renal capsule (n=5), partial adhesion with the perinephric fat (n=3) and renal vein thrombosis (n=1). A smooth clear tumor margin is highly reliable sign for stage I but infiltrative findings onto renal capsule and perirenal fat could be considered stage I.

10.Application of ABO genotyping in determination of ABO subgroups.

BACKGROUND: The knowledge about the nucleotides sequence of 9th chromosome that regulates the phenotype of ABO blood group has made the ABO genotyping possible. Since the genotyping can be done with only a small amount of DNA sample, it was primarily applied to the field of forensic medicine. When applied to the blood bank, it is useful in the resolution for ABO discrepancies between the cell and serum typing and determination of A and B subgroups. Rapid ABO genotyping using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method and its value in determination of ABO subgroups is presented. METHODS: ABO genotyping was performed in seven patients and three families, seven were the cases of ABO discrepancies in routine ABO grouping and three families were for the confirmation of the ABO group. To identify the 261th nucleotide, a 252 bp PCR amplifed fragment was amplified by PCR and digested with Kpn I. For 703th nucleotide, a 128 bp PCR amplified fragment was designed and digested with Alu I. To determine the ABO genotype, the patterns of digestion in DNA fragment were examined. RESULTS: Among the seven cases of ABO discrepancies, B3 and Ael were two cases each. Weakened B due to leukemia was the one, and the other two cases were cis-AB and Am. The three families for confirmation of the ABO group were acquired B due to infection one family, cis-AB two families. CONCLUSIONS: ABO genotyping is a rapid and reliable method that can be used in the case of ABO discrepancies and determination of ABO subgroups.
