2.An Adult Case of Mumps Brainstem Meningoencephalitis with a Past Measles-Mumps-Rubella (MMR) Vaccination.

Severe complications such as meningoencephalitis are quite rare in the setting of mumps infection. We report here on a 24-year-old soldier with a past history of MMR vaccination who was admitted because of aseptic meningitis. His mental status rapidly deteriorated and swelling of both parotid glands was observed. The MR findings revealed discrete lesions in the brainstem and thalamus. He barely recovered within 2 months. Among the adults with failure to acquire immunity despite MMR vaccination, institutional crowding can lead to cases of potentially fatal CNS complication such as mumps meningoencephalitis.

OBJECTIVE:
To review and explore the clinical characteristics, acoustic features, early diagnosis and prognosis of epidemic parotitis being accompanied by acute pancreatitis and sensorineural hearing loss (SNHL).
METHOD:
Report and analysis the clinical data of a patient with epidemic parotitis being accompanied by acute pancreatitis and sensorineural hearing loss and review the relevant literatures.
RESULT:
Epidemic parotitis virus could produce lesion in more than one organ resulting in the corresponding complications. It was uncommon when a patient presented epidemic parotitis, acute pancreatitis and sensorineural hearing loss at the same time. Unilaterally total hearing loss was the most common type of the SNHL, and the prognosis was bad.
CONCLUSION
Epidemic parotitis can be accompanied by one or more complication, including SNHL. Auditory brainstem response (ABR) is of great value in early diagnosis of SNHL in patients with epidemic parotitis.

4.Clinical studies on the short-course and efficient treatment of mumps meningitis.

OBJECTIVETo investigate the short-course and efficient way to treat mumps meningitis.
METHODSTotally 155 cases of mumps meningitis treated by intrathecal injection with dexamethasone only once were enrolled as experimental group and 55 similar cases treated with the common therapy as control.
RESULTSThe time for recoveries of temperature, headache, the pathologic reflexes and the total time of treatment were 32 hours, 15 hours, 12 hours and 3.1 days, respectively, while those of the control group were 58 hours, 24 hours, 32 hours, 6.5 days respectively. There was statistical difference between the two groups (P<0.01 or 0.05).
CONCLUSIONSIntrathecal injection with dexamethasone only once is efficient and safe in the treatment of mumps meningitis.
