The idiopathic hypereosinophilic syndrome is a disorder marked by the sustained overproduction of eosinophils. The disease is characterized clinically by damage to multiple organs due to eosinophilic infiltration and mediator release rather than by the level of the absolute blood eosinophil count. We report a case of hypereosinophilic syndrome presenting multiple organ damages with eyeball involvement.

To optimize results for polytrauma patients, prompt evacuation and early management are critical. These patients also require a multidisciplinary team effort, involving multiple departments, as they are likely to have not only musculoskeletal injuries but also specific organ injuries or compromised general status. In the 1980s, the goal was definitive fracture fixation in the early stages after injury (early total care). Since the 1990s, however, the goal has shifted to temporary fixation, with a delay of several days prior to definitive fixation (damage control orthopedics). Recently, the timing and extent of treatment have been determined by the condition of patients based on objective indicators. Because surgery may result in secondary damage, it is desirable to minimize potential tissue injury with either temporary fixation or a delay in definitive fixation.

Fractures of the scapula are relatively uncommon injuries and most can be treated satisfactorily with non-operative methods. But scapular fractures are being seen with increasing frequency in our mechanized society, specially in patients who have multiple injuries. So most injuries were related high energy, that residual deformities were high and related to the residual symptoms. Authors had done open reduction and internal fixation with plate in the four cases of the scapular fracture and analyzed that results.

4.The relationship of blood glucose to severity of injury in multiple trauma patients before operation

Background:Recent international researches in trauma patients have shown that hyperglycemia usually goes along with increased mortality, ventilator time and post-operative complications. The role of blood glucose in trauma prognosis and treatment is a current concern. Objective: To evaluate blood glucose change and the relationship of hyperglycemia to severity of injury in the clinical in multiple trauma patients before operation. Subjects and method: A prospective, descriptive study was conducted at the Emergency Surgery Unit and Post-anesthesia Care Unit, Department of Anesthesia and Recovery, Viet Duc hospital, from March to September/2006. The participants were over 15 years old, multiple trauma patients who were operated within 48 hours after the accident, but they had not been used inotrope, sedatives and operated in other clinical. Results: Hypoglycemia (under 3.5 mmol/l) was seen in 4/926 multiple trauma patients. Three of four were in shock due to severe blood loss, of which 2 patients had breath and cardiac arrest. Both patients died from multi-organ dysfunction immediately and one week after operation. The average blood glucose level of patients with ISS 25-40 and over 40 was significantly higher than those with ISS 16-24. Hyperglycemia had a closely positive association with ISS (r=0.48, p< 0.01). Conclusion: Most of multiple trauma patients have hyperglycemia before operation. Hyperglycemia has a relationship with severity of injury, especially with severity of anatomical injury. 4.1% patients have hypoglycemia which is mainly associative with prolonged shock.

Blunt diaphragmatic rupture (BDR) is a relatively rare injury and occurs in 0.8% to 7% of all thorocoabdominal blunt trauma. Especially right diaphragmatic rupture after blunt abdominal trauma is a rarer than left. The diagnosis of BDR can be missed while evaluating the multiple trauma patient. Other severe injuries may mask BDR during the primary resuscitation and survey. We experienced two cases of traumatic rupture of right diaphragm, one diagnosed immediately and the other diagnosed delayed. In this paper we present two cases of traumatic diaphragmatic rupture.

6.Simultaneous anterior and posterior dislocation of hips: a case report and review of literature.

The presence of anterior hip dislocation along with contralateral posterior hip dislocation in the absence of other major traumas is a distinctly rare injury pattern. We report such a case, along with a review of previous cases. A 40-year-old male patient after motorcycle skidding had posterior dislocation of the left hip and anterior dislocation of the right one without other associated injuries. The patient underwent successful closed reduction of both hips. The clinical course and follow-up assessment of the patient was uneventful.

7.Problems in Humeral Interlocking with Seidel Nail

Seidel nail is one of locked humeral nail and Seidel himself reported excellent result in his preliminary report using his nail. But our early experience showed disappointing result with high rate of complications, So we reviewed our 16 cases of humeral shaft fracture with polytrauma treated with Seidel nail and studied anatomical fitness of the nail within the distal medullary space using CT scan. The most frequent complication was intraoperative fracture of the proximal humerus < 4 cases> and this seemed to be due to lateral placement of pilot hole. There were 3 cases of nonunion and these cases showed insufficient distal anchorage, either primarily or postoperatively. Anatomical fitness study showed insufficient spreading of nail flanges in 5 cases of distal canal width below 9mm and nonunion cases were in this group. Other problems of Seidel nail were technical difficulties due to occasionally incorrect proximal targeting device and frequent breakage of adaptor screw. Study of more cases may be needed to confirm the design problem of Seidel nail.

8.A Case of Anti-Xga in a Young Man without Previous Transfusion.

We report a case of naturally occuring anti-Xga in a 22-year-old man suffering from multiple traumas, who had no past history of transfusion or transplantation. The positive reaction was detected at 37 degrees Cin the antibody screening test with LISS/Coombs card (DiaMed AG, Cressier, Morat, Switzerland), which disappeared at the enzyme phase with the NaCl/Enzyme card (DiaMed AG). The Xga antigen was negative on his red cell surfaces. The unexpected antibody was finally identified as anti-Xga. We found four of twelve units of packed red blood cells that were compatible with the patient's serum. Anti-Xga is a very rare unexpected antibody in Korea, so we need more study in order to determine incidence and the significance of this antibody.

9.Treatment of Renal Injury: Practical Approach.

Traumatic injuries are the leading cause of death in young people, and the renal injury is one of the major abdominal trauma. Hematuria is the most common sign of renal trauma, however the degree of hematuria and the severity of renal injury do not correlate consistently. Criteria for radiographic investigation is somewhat different according to the injury mechanism and an age. Excellent imaging modality such as computerized tomograph has made it possible to delineate not only detailed anatomic configuration for renal laceration itself but the presence of associated intra- abdominal organ injury. Renal injury may be presented as one of the multiple trauma, thus full evaluation for the overall injury severity of the patient is mandatory. Advanced supportive care techniques and awareness of the kidney's capacity for healing have facilitated non-operative management. Nowadays, operative exploration is indicated only in selected patients with major renal injury even in the patients with penetrating trauma, and high rates of renal reconstruction is possible through early vascular control. Urologic surgeon, as a major member of the trauma team, should be expertized in the surgical skills for reconstruction as well as acquiring up-to-date knowledge on diagnosis and management of renal trauma.

10.Neglected Type IV Acromioclavicular Joint Injury: 2 Cases Report.

Acromioclavicular joint injuries usually occur as a result of direct trauma to the superolateral aspect of the shoulder. Roockwood Type IV injuries are relatively uncommon, and they are easily misdiagnosed or neglected in patients who have suffered multiple traumas. Therefore, to correctly treat a patient with type IV injury, we need to take a careful physical examination and conduct proper radiologic evaluation for the acromioclavicular joint. We report here on two cases of modified Weaver-Dunn reconstruction for neglected type IV acromioclavicular joint injuries that were associated with multiple rib fractures.
