1.Detection of Babesia bovis in blood samples and its effect on the hematological and serum biochemical profile in large ruminants from Southern Punjab.

OBJECTIVETo determine the presence of Babesia bovis (B. bovis) in large ruminants in southern Punjab and its effect on hematological and serum biochemical profile of host animals.
METHODSBlood samples were collected from 144 large ruminants, including 105 cattle and 39 buffaloes, from six districts in southern Punjab including Multan, Layyah, Muzaffar Garh, Bhakar, Bahawalnagar and Vehari. Data on the characteristics of animals and herds were collected through questionnaires. Different blood (hemoglobin, glucose) and serum (ALT, AST, LDH, cholesterol) parameters of calves and cattle were measured and compared between parasite positive and negative samples to demonstrate the effect of B. bovis on the blood and serological profile of infected animals.
RESULTS27 out of 144 animals, from 5 out of 6 sampling districts, produced the 541-bp fragment specific for B. bovis. Age of animals (P=0.02), presence of ticks on animals (P=0.04) and presence of ticks on dogs associated with herds (P=0.5) were among the major risk factors involved in the spread of bovine babesiosis in the study area. ALT concentrations were the only serum biochemical values that significantly varied between parasite positive and negative cattle.
CONCLUSIONS: This study has reported for the first time the presence of B. bovis in large ruminant and the results can lead to the prevention of babesiosis in the region to increase the livestock output.

2.Limb Salvage Surgery in Chronic Osteomyelitis: A Case Report

Osteomyelitis refers to an infection of the bone characterized by progressive inflammatory destruction caused by infecting microorganism. Open fracture carries the risk of developing osteomyelitis from 3 to 50% with a high amputation rate. Salvage of the limb is always a challenge and needs perseverance. We report the case of a 44-year-old male with chronic osteomyelitis with successful multiple limb salvage surgeries. The curative approach to chronic osteomyelitis has the following goals including arrest of infection, pain reduction and salvage of limb and function. With regard to survival and function, the result obtained in this case is more promising compared to above knee amputation.
3.Vertebral Endplate Changes Correlate with Presence of Cartilaginous Endplate in the Herniated Disc Tissue: Factor Predicting Failure of Conservative Treatment

Methods:
Fifty-one discs were obtained from patients undergoing surgery for herniated disc. Their ages ranged from 19-65 years, and 31/51 were male. Pre-surgical T1 and T2 weighted lumbar-spine MRIs were analyzed to observe Pfirrmann grade, VEP defects, herniation type, Modic changes, and high-intensity zones (HIZ) at the affected level. Five-micron thick sections were stained with hematoxylin and eosin, Alcian blue periodic acid–Schiff stain; examined for histological degeneration scores (HDS; 0–15), inflammation (0 [absence]–3 [severe]), and presence of cartilaginous endplate (CEP). Three-micron thick sections were stained with protein-gene-product 9.5 and expression was counted/mm2. Data was analyzed, and p<0.05 was considered to indicate statistical significance.
Results:
VEP defects, Modic changes, and HIZ were respectively observed in 30/51, 16/51, and 6/51 of the samples. CEP was observed in 26/51 samples and in 23/51 with endplate defects. Discs with adjacent VEP defects showed increased HDS (p<0.001) and inflammation (p<0.001). Discs with adjacent Modic changes also revealed increased HDS (p=0.01). Histological sections with CEP showed increased HDS (p<0.001) and inflammation (p<0.001), and nerve density was significantly positively correlated with HDS (r=0.27, p=0.02).
Conclusions
VEP changes can modulate degeneration and inflammation of herniated discs. Presence of these changes is highly predictive of the occurrence of CEP in herniated discs, which leads to slow resorption and persistent clinical symptoms.
4.Posterior Polymorphous Dystrophy: An Unusual Presentation

Posterior Polymorphous Dystrophy (PPD) is a rare, innocuous and asymptomatic
condition in which corneal endothelial cells display characteristics similar to
epithelium. It is often bilateral and frequently asymmetric. We report a case of a
10-year-old girl with a family history of glaucoma who presented with right eye
blurring of vision since few years. She had frequent spectacle-prescription changing
due to unimproved visual acuity. Cycloplegic refraction revealed high astigmatism
and moderate amblyopia over the right eye. Her best corrected vision was 6/9 for
the right and left eyes. Slit-lamp examination showed a vesicular-like lesion at the
periphery of corneal endothelial layers in both eyes. Endothelial cell density was
much reduced on the right eye compared to the left, with more severe astigmatism.
Other ocular examinations were unremarkable. Patient was instructed to patch
her better left eye periodically. Although patients with Posterior Polymorphous
Dystrophy (PPD) present at the age of adulthood, the age at diagnosis is highly
variable. It is rare and inherited through an autosomal dominant pattern. PPD may
rarely lead to astigmatism, by which was present in the patient due to the features
of non-keratoconic and keratoconic cornea on the right and left eye, respectively.
Endothelial cell counts were reduced more on the right eye compared to the left,
which later may worsen her visual acuity thus indicating corneal transplant in the
future. Patient also had a strong family history of glaucoma. Hence, screening of
associated disease of corneal endothelial dystrophy in the family may be necessary.
5.Gastric Schwannoma in an elderly man: A case report

Schwannomas are mesenchymal tumors that are characteristically benign and slow growing, which
originate from any nerve with Schwann cell sheath. Gastrointestinal schwannomas are rare with
distinct morphologic features as compared to schwannomas of soft tissue or central nervous system.
A 77-year-old male patient was diagnosed with gastrointestinal stromal tumor based on radiological
findings and clinical impression when he presented with worsening abdominal discomfort and pain.
He underwent distal gastrectomy however histopathological examination of the tumour revealed
schwannoma. This case report presents a rare case of a symptomatic gastric schwannoma, whose
definitive diagnosis was established by histopathological and immunohistochemical findings
postoperatively.
6.Descemet Membrane Detachment Post-Phacoemulsification

We report the case series of phacoemulsification-related Descemet membrane detachment (DMD) encountered at a tertiary hospital in Kuala Lumpur. Case 1 was an iatrogenic DMD which was detected intraoperatively and managed early with good outcome. Case 2 and case 3 described unusual presentation of DMD which was initially undiagnosed. This report highlights the use of anterior segment optical coherence tomography (ASOCT) in detecting and confirming the correct diagnosis for DMD. With the aid of an experienced corneal specialist, the higher threshold for suspicious occurrence of DMD was confirmed using ASOCT. Treatment was tailored accordingly, with successful clearance of corneal oedema and visual recovery. This case series highlighted the importance of proper operative documentation and high threshold for suspicion for DMD in focal corneal oedema following an otherwise uneventful cataract surgery. It is concluded that ASOCT is an excellent tool to confirm diagnosis of DMD and success of treatment.
7.Observation of Auditory Perceptual and Visuo-Spatial Characteristic of a Patient with Hemangiopericytoma in Occipital Lobe: A Magnetoencephalography (MEG) Study

The present study discussed functional reorganization and alteration in respond to the slow-growing tumour,hemangiopericytoma in the occipital cortex. Visual evoked field (VEF) and auditory evoked field (AEF) usingmagnetoencephalography (MEG) was used to evaluate the source localization and brain activity. Results of VEF sourcelocalization show a typical brain waves. Brain activity of the occipital lobe demonstrate low activation in the ipsilateralto the tumour. However, result shows the activation on the contralateral hemisphere was high and bigger in activationvolume. AEF result shows an identical source localization and both side of the temporal lobe are activated. This resultsuggests that there is a positive plasticity in auditory cortex and slow-growing tumour can induce functional reorganizationand alteration to the brain.
8.Tissue-Engineered Hydroxyapatite Bone Scaffold Impregnated with Osteoprogenitor Cells Promotes Bone Regeneration in Sheep Model

BACKGROUND:
Managing massive bone defects, a great challenge to orthopaedics reconstructive surgery. The problem arise is the supply of suitable bone is limited with many complications. Tissue-engineered hydroxyapatite bone (TEHB) scaffold impregnated with osteoprogenitor cells developed as an alternative to promote bone regeneration.
METHODS:
This animal protocol has been approved by Universiti Kebangsaan Malaysia Animal Ethical Committee. The TEHB scaffold prepared from hydroxyapatite using gel casting method. A total of six adolescent female sheep were chosen for this study. Later, all the sheep were euthanized in a proper manner and the bone harvested for biomechanical study.Bone marrow was collected from iliac crest of the sheep and bone marrow stem cells (BMSCs) isolated and cultured. BMSCs then cultured in osteogenic medium for osteoprogenitor cells development and the plasma collected was seeded with osteoprogenitor cells mixed with calcium chloride. Bone defect of 3 cm length of tibia bone created from each sheep leg and implanted with autologous and TEHB scaffold in 2 different groups of sheep. Wound site was monitored weekly until the wound completely healed and conventional X-ray performed at week 1 and 24. Shear test was conducted to determine the shear force on the autologous bone and TEHB scaffold after implantation for 24 weeks.
RESULTS:
All of the sheep survived without any complications during the study period and radiograph showed new bone formation. Later, the bone harvested was for biomechanical study. The highest shear force for the autologous group was 13 MPa and the lowest was 5 MPa while for the scaffold group, the highest was 10 MPa and the lowest was 3 MPa.Although, proximal and distal interface of autologous bone graft shows higher shear strength compared to the TEHB scaffold but there is no significant difference in both groups, p value [ 0.05. Histologically in both proximal and distal interface in both arms shows bone healing and woven bone formation.
CONCLUSION
TEHB scaffold impregnated with osteoprogenitor cells has the potential to be developed as a bone substitute in view of its strength and capability to promote bone regeneration.

10.A Case Study on Sibling Relational Problem: Its Clinical Significance in Managing A Dyslexic Adolescent with Mild Depressive Disorder (Kajian Kes Mengenai Masalah Perhubungan Adik Beradik: Kepentingan Klinikalnya dalam Pengurusan Remaja Disleksia yang Mengalami Kemurungan Ringan )

The purpose of this case study was to see the impacts of managing sibling relational problem on mild depressive
disorder. This approach was applied on a 15-year-old girl, who has dyslexia. She was found to have mild depression for
one year that was precipitated by her poor relationship with her younger sibling. The intervention used in managing this
case was adapted from the Siblings Are Special Module, a special programme that helps to alleviate anger and frustration
that siblings had on each other. These 5 session interventions also included leisure activities that helped them to
overcome their ongoing conflicts and were conducted over 3 months period. Parents were involved in the last 2 sessions
in order to promote positive parenting. She showed a marked improvement in her psychological wellbeing and family
relationship especially with her siblings at the end of the intervention. In conclusion, the intervention used to improve
sibling relationship has shown to be effective in improving the mental health of the adolescents.\
