1.Antioxidant and cytotoxic activity of Acanthus ilicifolius flower

Objective: To investigate the antioxidant and cytotoxic activity of the flower of Acanthus ilicifolius (A. ilicifolius). Methods: Antioxidant activity was determined as antiradical efficiency with diphenyl picrylhydrazil (DPPH) method and cytotoxic assay was undertaken using brine shrimp lethal toxicity test. Results: A. ilicifolius flower contained terpenoid, phenolic compounds, and alkaloid. The methanol extract of A. ilicifolius flower showed the highest antiradical efficiency (AE=1.41í10-3) against DPPH radicals and the highest cytotoxicity (LC50=22 μg/mL) against brine shrimp nauplii. Conclusions: It is suggested that active compounds of A. ilicifolius flower solved in methanol play a role to inhibit free radical activity and kill Artemia salina nauplii. The substances can be considered as potential antioxidant and cytotoxic agents as well as imminent candidate for cancer therapy.

3.The Impact of Dry Needling on Patients with Neck Pain in a Tertiary Hospital

Neck pain presents as a symptom of dull pain or discomfort mainly along the trapezius muscle. Dry needling is an
invasive procedure which uses acupuncture needle directed at myofascial trigger points. The aim of the study was to
evaluate the effectiveness of dry needling in managing patients with neck pain. A pre-test-post-test interventional
study design was used. Patient education package was provided to 32 respondents who fulfilled the inclusion criteria.
A 13-item Pain Catastrophizing Scale (PCS) questionnaire was used to assess Rumination, Magnification and
Helplessness. Subjective pain intensity was measured by Visual Analogue Scale (VAS). These questionnaires were
given before and after the dry needling intervention. The findings reported that respondents scored high in pre-test
total PCS score (27.41±13.652). Post-test result revealed a significant improvement in total PCS score
(23.06±13.938) (p = 0.000). Post-test VAS score (4.78±1.237) was also significantly better than pre-test
(6.47±1.414) (p = 0.000). There was no significant difference in pre-test PCS in terms of marital status (p > 0.05)
whereas there was significant difference between marital status and rumination in post-test (Z = -2.303, p = 0.021).
There was significant difference between pre-test magnification in terms of respondents’ occupation (p = 0.008) and
race (p = 0.035) but no significant difference in post-test. Respondents’ age group showed no significant differences
between pre-test and post-test PCS and VAS (p > 0.05). In conclusion, patients who received dry needling showed
improvement in pain intensity and catastrophizing towards neck pain.

4.The Uncharted Danger of Living in the Tropics: A Needlefish Impalement of the Sigmoid Colon

Penetrating injury to the abdominal viscera is not uncommon unless when it is caused by marine life. We present a 39-year-old fisherman from a tropical country who had sustained a penetrating injury from a stab wound to the abdomen due to needlefish impalement. He sustained a small perforation of the sigmoid colon during exploratory laparotomy and primary repair was done. Although this incident is rare, there are cases involving seawater activities either for leisure, sport or diving for fishing. We highlight this rare incident and discuss the management plan.
5.A SHORT-TERM LONGITUDINAL STUDY ON MULTIDIMENSIONAL OUTCOMES FOLLOWING MILD TRAUMATIC BRAIN INJURY IN MULTI-ETHNIC MALAYSIA

his is a short-term longitudinal study of physical, cognitive, psychological and functional outcomes following mild traumatic brain injury (mTBI) in adults, caused by road traffic accident (RTA). Outcome measures were the Montreal Cognitive Assessment (MoCA), the Neuropsychological Assessment Battery Screening Module (S-NAB), the Patient Health Questionnaire (PHQ-9) and the Generalized Anxiety Disorder 7 (GAD-7) scale. Functional outcomes included returning to working/academia and driving, lifestyle changes, financial claims and litigation status. A total of 285 individuals were diagnosed with mTBI, involving young male motorcyclists (90%): uncomplicated mTBI, n=201; complicated mTBI, n=84. Ethnic distribution consisted of 204 Malays, 58 Indians and 23 Chinese. MoCA detected cognitive deficits (mean=23.11, SD=3.41) within 72 hours of injury. At two weeks, somatic manifestations, physical injuries, cognitive deficits and psychological symptoms were detected. At three months of injury, the language domain was persistently impaired, with a lower score on most cognitive domains in the complicated mTBI category than the uncomplicated mTBI. Psychological and somatic symptoms had improved. Almost 50% of patients had returned to a functional baseline within two weeks of injury and a further 24% within three months. A small proportion of patients made active lifestyle changes (<25%), financial injury claims (38%) and were involved in litigation (11%). In conclusion, mTBI in multi-ethnic Malaysia has multifaceted deficits and outcomes. Early management of symptoms may promote maximum recovery.
