We report very rare case of concurrent bronchial atresia and bronchogenic cyst. Morphologic apical segment of right upper lobe directly stemmed from right main bronchus. Bronchogenic cyst was communicating with atretic segmental bronchus and both were filled with mucus. The etiology is not well known, however it is likely that a single insult arround the 5th~6th week causes both malformations.

2.A Case of Bronchial Obstruction with Mucus Plug following Endotracheal Intubation .

To keep the patent airway during a general anesthetic procedure or when handling an unconscious patient is the most important concern as well as responsibility and/or duty of the anesthesiologist. Many delicate operations could and can be performed because of the development of anesthetic techniques aad methods, especially through the introduction of endotracheal intubations. However, at times, an endotracheal tube can unfortunately be a hazardous instrument-lethal instead of lifesaving. The authors experienced a case of left main bronchial obstruction with a thick mucus plug immediately following the completion of oro-endotracheal intubation. This was successfully remedied by direct bronchoscopic aspiration under transtracheal spray. In such a circumstance as this, the newly designed instrument, namely; a bronchofiberscope, is a real necessity.

6.The wormicidal substance of fresh water fishes on Clonorchis sinensis V. Purification and chemical characterization of clonorchicidal substance from epidermal mucus of Cyprinus carpio.

As a series of studies to clarify clonorchicidal substances in body surface mucus of some fresh-water fishes, the substance in the epidermal mucus of Cyprinus carpio was isolated by silica gel column and thin layer chromatography and analyzed for its chemical nature. Wormicidal trial was done in vitro, and the results obtained are summarized as follows: The mucus was extracted by ethyl ether and separated into 4 fractions by column chromatography using benzene as solvent. The second fraction with yellowish red colour among them showed the strongest clonorchicidal effect. The yellowish red fraction obtained by column chromatography was then fractionated into 6 spots by thin layer chromatography with petrol. ether/chloroform(30/70, v/v), and the Rf. 0.714 spot among the 6 spots showed the strongest effect. The Rf. 0.714 spot was further fractionated into 6 spots by thin layer chromatography with benzene/acetone (90/10, v/v), and the Rf. 0.800 spot among the later 6 spots revealed the strongest effect. The Rf. 0.800 spot was chromatographed on column with benzene and 2 fractions were obtained. The second fraction of light brown colour represented the final purified fraction. By these purification procedures, clonorchicidal substance was purified 15-fold with 0.03 percent yield from the mucus of C. carpio, and 10mg of the final fraction killed the cercaria in 26 min, the metacercaria in 115 min, and the adult in 443 min. Infra red and nuclear magnetic resonance spectrometric analysis of the purified substance revealed that the substance belongs to an ethyl ester of unsaturated fatty acid with 2 double bonds, 15 methylene groups and l methyl group.

7.Clinical Application and Limit of Magnifying Colonoscopy.

BACKGROUND/AIMS: Magnifying colonoscopy was developed for detailed examination of the surface of colorectal neoplastic lesions. While magnifying colonoscopy is useful for differentiating neoplastic lesions from nonneoplastic ones, for evaluating early colorectal cancers, it still has limits in practice. This study was designed to clarify the usefulness and the limits of magnifying colonoscopy. METHODS: Three hundred and fifty-two lesions, which were observed using magnifying colonoscopy from July to August 1999 and whose histologies were proven, were analyzed according to their pit patterns. The pit patterns are I, normal round pits; II, large starry-shaped pits; IIIs, small round pits; IIIL, large round or rod-shaped pits; IV, branched or gyrus-like pits; and V, irregularly shaped or nonstructural pits. In cases where a pit pattern was hard to read, the pattern was classified as 'D'. RESULTS: The dominant pit pattern for protruded lesions was IIIL, accounting for 44.6%. In diminutive lesions (< or =5 mm), II and IIIL were equally common, 40% of the total for those lesions, respectively. In medium-sized lesions (from 6 to 10 mm), IIIL was the major pit pattern, 45.6% of the total for that type of lesion. In lesions larger than 10 mm, IIIL and IV were the most common pit patterns, each accounting for 26.7% of the total. The overall accuracy ratio of pit pattern diagnoses was 79.5%. The frequency of difficult pit patterns to read was 6.3%. Among them, 77.3% were due to difficulty in interpreting the pit patterns, and 22.7% were due to an inability to clarify the pit pattern because of poor staining. Ninety-one percent of the difficult cases to read involved diminutive lesions, and 86% of those difficult cases involved tubular adenomas. CONCLUSIONS: The 80% accuracy rate for pit pattern diagnosis suggests that magnifying colonoscopy is probably useful, but problems, such as poor staining due to mucus and difficulty with interpretation, still exist.

8.Re-discussion on "Fenglong (ST 40) for phlegm".

After reading the article, Tracing Origin and Development of "Fenglong (ST 40) for Phlegm": Examination and Verification published in Chinese Acupuncture & Moxibustion, the authors collected the original evidences also for the effect of Fenglong (ST 40) on phlegm disorders by tracing from 9 ancient books of acupuncture and moxibustion, explored the standardized expressions of its indications and analyzed the changes of its indications. In that article, it was viewed that the effect of Fenglong (ST 40) started to be generalized in treatment of phlegm disorders since the Ming Dynasty. On contrary, we believes that Fenglong (ST 40) works on strengthening the spleen and resolving dampness for the disorders caused by the broad meaning of "phlegm/damp". Based on it, since the Ming Dynasty, the effect of Fenglong (ST 40) is extended specifically for the disorders caused by the visible sputum from the lung, the narrowing mean of "phlegm". Hence, if "Fenglong (ST 40) for phlegm" is considered as an academic point of view, it needs an adequate evidences to expound and prove.

9.Effect of umbilical moxibustion on phlegm damp constitution and intestinal flora.

OBJECTIVE:
To observe the effect of umbilical moxibustion on phlegm damp constitution and intestinal flora, and explore the mechanism of umbilical moxibustion in improving phlegm damp constitution.
METHODS:
A total of 60 subjects with phlegm damp constitution were randomly divided into an observation group and a control group, 30 cases in each group. The control group was given TCM health guidance, such as diet and exercise; on the basis of the control group, umbilical moxibustion was applied in the observation group, 7 moxa cones each time, 2 times a week for 8 weeks. Before and after intervention, the indexes (transformation score of phlegm damp constitution, body mass, body mass index [BMI], waist circumference, hip circumference, heart rate and blood pressure) related to phlegm damp constitution were recorded in the two groups. The intestinal flora was detected with 16S rDNA sequencing technology in the two groups.
RESULTS:
After intervention, the transformation score of phlegm damp constitution, body mass, BMI, waist circumference and hip circumference were decreased compared before intervention in the observation group (

10.Research progress of intestinal mucus barrier and the repair of intestinal damage after burn injury.

The intestinal mucus layer is the first line of innate host defense and provides energy for the growth of the intestinal flora. In addition, it plays an important role in regulating intestinal microecology and maintaining intestinal homeostasis. This paper reviews the synthesis, secretion, processing, and modification of intestinal mucus, and focuses on the effect of intestinal mucus barrier on intestinal injury repair and the regulation of intestinal mucus barrier by special nutrients after burn injury.
