1.Application of Bayes Probability Model in Differentiation of Yin and Yang Jaundice Syndromes in Neonates.

OBJECTIVETo study the application of Bayes probability model in differentiating yin and yang jaundice syndromes in neonates.
METHODSTotally 107 jaundice neonates who admitted to hospital within 10 days after birth were assigned to two groups according to syndrome differentiation, 68 in the yang jaundice syndrome group and 39 in the yin jaundice syndrome group. Data collected for neonates were factors related to jaundice before, during and after birth. Blood routines, liver and renal functions, and myocardial enzymes were tested on the admission day or the next day. Logistic regression model and Bayes discriminating analysis were used to screen factors important for yin and yang jaundice syndrome differentiation. Finally, Bayes probability model for yin and yang jaundice syndromes was established and assessed.
RESULTSFactors important for yin and yang jaundice syndrome differentiation screened by Logistic regression model and Bayes discriminating analysis included mothers' age, mother with gestational diabetes mellitus (GDM), gestational age, asphyxia, or ABO hemolytic diseases, red blood cell distribution width (RDW-SD), platelet-large cell ratio (P-LCR), serum direct bilirubin (DBIL), alkaline phosphatase (ALP), cholinesterase (CHE). Bayes discriminating analysis was performed by SPSS to obtain Bayes discriminant function coefficient. Bayes discriminant function was established according to discriminant function coefficients. Yang jaundice syndrome: y1= -21. 701 +2. 589 x mother's age + 1. 037 x GDM-17. 175 x asphyxia + 13. 876 x gestational age + 6. 303 x ABO hemolytic disease + 2.116 x RDW-SD + 0. 831 x DBIL + 0. 012 x ALP + 1. 697 x LCR + 0. 001 x CHE; Yin jaundice syndrome: y2= -33. 511 + 2.991 x mother's age + 3.960 x GDM-12. 877 x asphyxia + 11. 848 x gestational age + 1. 820 x ABO hemolytic disease +2. 231 x RDW-SD +0. 999 x DBIL +0. 023 x ALP +1. 916 x LCR +0. 002 x CHE. Bayes discriminant function was hypothesis tested and got Wilks' λ =0. 393 (P =0. 000). So Bayes discriminant function was proved to be with statistical difference. To check Bayes probability model in discriminating yin and yang jaundice syndromes, coincidence rates for yin and yang jaundice syndromes were both 90% plus.
CONCLUSIONYin and yang jaundice syndromes in neonates could be accurately judged by Bayesian discriminating functions.

2.Cloning and expression of Rv2460 c gene in mycobacterium tuberculosis and analysis of immune characteristics of its coding product

Objective:To clone and express Mycobacterium tuberculosis(Mtb) Rv2460c gene(encoding ClpP2 protein),and evaluate the immunogenicity of its coding product. Methods: The recombinant plasmid of pET32a (+) vector-ClpP2 that H37Rv Rv2460c gene was cloned into the plasmid pET32a(+)vector,was transformed into E. coli BL21(DE3)and induced expression by IPTG,then purified by affinity chromatography. The recombinant protein was confirmed by SDS-PAGE and Western blot. The analysis of immunogenicity of Mtb ClpP2 and its epitope prediction were performed by bioinformatic methods. The antibody levels of polyclonal antibody titer against ClpP2 protein in rabbits and TB patients′ serum were detected by ELISA. Results: The recombinant ClpP2 protease was expressed as inclusion bodies in E. coli. The purity of purified protein was 93% by bandscan software analysis. The bioin-formatics analysis shows Mtb ClpP2 protein has multiple preponderant B cell and T cell epitopes. Rabbit antiserum titer was 1∶64 000;Serum anti-ClpP2 antibody levels in TB patients was higher than that in healthy control subjects. Conclusion:The recombinant ClpP2 protein was purified, and specific Rabbit anti-ClpP2 polyclonal antibody was prepared successfully. Experiment and bioinformatic information studies showed that Mtb ClpP2 protease has strong immunogenicity.
3.Shang Ring circumcision by transverse incision in the distal penis foreskin and pull-up of the interior board for short frenulum praeputii.

OBJECTIVETo investigate the effectiveness of surgical strategies for Shang Ring circumcision in the treatment of short frenulum praeputii in patients with redundant prepuce or phimosis.
METHODSTotally, 130 cases of short frenulum praeputii with redundant prepuce or phimosis were randomly assigned to an experimental group and a control group of equal number to receive Shang Ring circumcision, the former by transverse incision in the distal penis foreskin and pull-up of the interior board, and the latter by conventional transverse incision and longitudinal suture of the frenulum praeputii. Comparisons were made between the two groups in the surgical duration, intraoperative blood loss, 24 h postoperative pain visual analog score (VAS), postoperative complications, satisfaction with the penile appearance, and the quality of sexual life.
RESULTSThe surgical duration, intraoperative blood loss, 24 h postoperative VAS, postoperative sexual satisfaction, and satisfaction with penile appearance were (4.60 +/- 1.20) min, (2.61 +/- 1.81) ml, 1.73 +/- 0.76, 98.5%, and 98.5%, respectively, in the experimental group, as compared with (21.60 +/- 6.30) min, (11.10 +/- 3.40) ml, 5.37 +/- 1.84, 70.3% and 69.8% in the control, with statistically significant differences between the two groups (P < 0.05). The incidence rates of such major complications as wound dehiscence, infection, and moderate to severe edema were 1.5% (1/65), 3.1% (2/65), and 4.6% (3/65), respectively, in the experimental group in comparison with 12.3% (8/65), 15.3% (10/65), and 30.7% (20/65) in the control, with statistically significant differences between the two groups (P < 0.05). None of patients had any serious complications.
CONCLUSIONShang Ring circumcision by transverse incision in the distal penis foreskin and pull-up of the interior board, with its advantages of shorter operation time, less blood loss, mild pain, fewer complications, and higher satisfaction and acceptance of the patients, can be used as an safe and effective approach to the treatment of short frenulum praeputii.

4.Purification,antiserum preparation and antigenic characters of ClpC2 in Myco-bacterium tuberculosis

Objective:To investigate the antigenicity of ClpC 2 and the feasibility of polyclonal antibodies of ClpC 2 as detected antibody.Methods:rClpC2 was induced with IPTG.The rClpC2 was identified by SDS-PAGE and Western blot ,and purified by affinity chromatography ,with which rabbit were immunized and the specificity of rabbit antiserum was detected by Western blot , the titer of rabbit antiserum against ClpC2 was detected by double immunodiffusion and indirect enzyme-linked immunosorbent assay (ELISA).The antigenicity of the rClpC2 was detected by ELISA.The polyclonal antibodies of ClpC 2 were prepared to detect the ClpC 2 in clinical serum of TB patients by ELISA.Results:SDS-PAGE showed specific protein band with a relative molecular mass of 46 kD.The rClpC2 could bind with the antibody in the blood serum of the mouse immuned by MTB.By Bandscan analysis rClpC 2 accounted for about 58.7%of the total bacteria protein ,the purity of rClpC2 was 88.5% after purification.The ClpC2 of BCG could bind with the rabbit antiserum.The titer of antiserum were 1∶32 and 1∶320 000 by double immunodiffusion and ELISA detected respectively.ELISA results showed that clinical serum positive rate of rClpC 2 antigen was 46%in TB patients,the sensitivity of this protein was 46%,and the spe-cificity of this protein was 90%.ELISA results showed that the sensitivity of rabbit antiserum against ClpC 2 was 40%, and the specificity was 90%.Conclusion: Successfully expressed and purified rClpC 2 and high titer polyclonal antibody were successfully prepared,and these results will provide basements for further study on the biological functions of ClpC 2 and its candidate potentiality as serological diagnosis and drug-target and biological functions of antiserum against ClpC 2.
5.A qualitative research on the behaviors caused by fear of hypoglycemia in type 2 diabetic patients

Objective To explore the behaviors caused by fear of hypoglycemia in type 2 diabetic patients, and deeply analyze the causes of these behaviors, providing help for medical staff to fully understand the inner demands of diabetic patients with hypoglycemia, so as to manage the blood glucose effectively, laying the foundation of Hypoglycemia Fear Survey-behavior scale suitable for domestic type 2 diabetic patients. Methods The interpretative phenomenological analysis method was used guided by the knowledge-attitude-belief-practice and social cognitive theory, 33 patients with hypoglycemia from 5 hospitals with different levels were interviewed by semi-structured interview. Results Four themes were extracted, including excess sensitivity, caution, avoidance and compromise. The theme excess sensitivity contained three kinds of behaviors, that were meal with sensitive alertness, drug sensitivity and excessive response to the hypoglycemia, while the theme caution contained two kinds of behaviors, that were strive to blood sugar stable and must prepare food. Conclusions The results show that the fear of hypoglycemia is serious in type 2 diabetic patients, and they make many behavior change to avoid it, affecting their quality of life and glycemic management. Medical staff should attach importance to the inner feelings and behaviors of diabetic patients with hypoglycemia, and provide guidance and support after comprehensive assessment, working together with the patient to realize the effective management of blood glucose.
6.Effect of hsa-microRNA-218 on granulysin expression

Objective:To elucidate the effect of hsa-microRNA-218(hsa-mir-218)on exogenous granulysin (GLS) expression in 293T cells.Methods:Total RNA was extracted from THP-1 cells induced by phorbol 12-myristate 13-acetatefor (PMA), and GLS gene was amplified by RT-PCR, and then cloned into pDsRed-Express-C1 to construct the GLS expression vector pDsRed-GLS.Then 293T cells were co-transfected with pDsRed-GLS and pGenesil-mir-218 (pGenesil-mir-control) and laser confocal microscopy was per-formed 36 h later to detect their co-expression .Total RNA and protein were extracted 48 h post transfection , and RT-PCR and Western blot were performed to detect the effect of hsa-mir-218 on exogenous GLS expression .Results:The GLS expression vector pDsRed-GLS was constructed successfully and laser confocal microscopy indicated that it was co -expressed with the interference vector .Compared with that of cells transfected with pGenesil-mir-control, Western blot showed a markedly decrease of GLS protein expression (50%) in the cells transfected with pGenesil-mir-218.However, GLS mRNA expression remained unchanged .Conclusion: hsa-mir-218 nega-tively regulates GLS expression at a post-transcriptional level , and this provides an experimental basis for future study of mechanism of GLS expression regulated by mir-218 .
7.The effects of ultraviolet blood irradiation and oxygenation on oxygen free radical metabolism in rabbits with soman intoxication

Objective To investigate the effects of ultraviolet blood irradiation and oxygenation(UBIO)on oxygen free radical metabolism(OFRM)in rabbits with acute soman intoxication.Methods One hundred rabbits were randomly divided into five groups:a control group,a soman intoxication group(I),a soman intoxication plus routine therapy group(TR),a soman intoxication plus UBIO therapy group(UBIO)and a soman intoxication plus complex therapy group(CT).All the rabbits were intervened accordingly.Then the concentrations of malondiade- hyde(MDA)and the activities of superoxide dismutase(SOD),glutathionperoxidase(GSH Px)and catalase (CAT)in serum were determined at 14 d after various treatments.Results Compared with the control group,the concentration of MDA and the activity of CAT in the 1 group were significantly increased(P<0.01),while the activi- ties of SOD and GSH Px were obviously decreased(P<0.05).After UBIO or complex therapy,the serum level of MDA was significantly decreased in comparison with that in the I group(P<0.01),while the concentrations of SOD, GSH Px and CAT were enhanced(P<0.05).Conclusion UBIO therapy can improve antioxidation activity against the injury caused by free radicals and could be used to treat acute soman intoxication,which causes injury from in- creased oxygen free radical concentrations.
8.Observation on therapeutic effects of different methods on peripheral facial paralysis at different stages.

OBJECTIVETo observe therapeutic effects of different therapeutic methods for treatment of peripheral facial paralysis at different stages.
METHODSIn the treatment group, 26 cases at acute stage were treated by acupuncture at corresponding acupoints at the healthy side, 15 cases at resting stage by acupuncture at the affected side with shallow needling method, and 2 cases at restoration stage by routine acupuncture and moxibustion, and their therapeutic effects were compared with the control group of 36 cases who were treated by routine acupuncture and moxibustion. The acupoints selected in the two groups were same.
RESULTSThe cured-markedly effective rate was 97.7% in the treatment group and 75.0% in the control group with a very significant difference between the two groups (P < 0.01). The treatment group needed 2.7 therapeutic courses and the control group needed 4.0 courses in average with a very significant difference between the two groups (P < 0.01).
CONCLUSIONDifferent acupuncture methods at different stages are better than routine acupuncture method in treatment of peripheral facial paralysis.

10.Mass screening prostate cancer and clinical comparison.

OBJECTIVETo investigate the clinical value of the mass screening by analyzing the features of prostate cancer between mass screening patients and clinical patients.
METHODSFrom January 2000 to January 2008, 441 cases of prostate cancer (including 122 patients from clinical diagnosis and 319 patients from mass screening 23 183 men who were more than 50 years old) were analyzed from age, digital rectal examination (DRE), serum prostate specific antigen (PSA) levels and range, the Gleason's score and grade, clinical staging and therapy.
RESULTS42.0% of mass screening patients were inspected by DRE, it was lower than that (79.5%) in the clinical patients. The percent of patients with serum PSA levels of less than 10.0 microg/L in mass screening group was higher than in clinical group, while the percent of patients with serum PSA levels of more than 20.0 microg/L in mass screening group was lower than in clinical group. The percent of moderately differentiated degree of prostate cancer in mass screening group was higher than in clinical group, but it was on the contrary for poorly differentiated degree of prostate cancer. The percent of T1-2 prostate cancers in mass screening group was 56.1%, which was higher than 25.4% in clinical group. While, the percent of T3-4 tumors in mass screening group was lower than in clinical group. The percent of men undergoing radical prostatectomy in mass screening group was 18.2%, which was higher than 9.8% in clinical group. The percent of men of locally advanced and far metastasis in mass screening group was 26.0%, while was lower than 46.0% in clinical group.
CONCLUSIONGeneral investigation for prostate cancer is benefit to find asymptomatic cancer of early stage.
