1.Middle pancreatectomy, a report of 19 cases

Objective To evaluate central pancreatectomy in pancreatic surgery. Methods The clinical data of 19 patients who received central pancreatectomy was retrospectively analyzed in the Provincial Hospital, Shandong University from Jan 2002 to Jun 2008. Results Among 19 patients, 9 patients were of islet cell tumor,4 patients were mucinous cystoadenoma,3 patients were solid pseudopapilloma, 2 patients were pancreatic carcinoid tumor, 1 patient was of cyst-solid pseudopapilloma. After resection all of them had pathological negative margin. Length of stay was ( 17. 7 ± 7. 9) days ( ranging from 10 to 40 days ). Incidence of pancreatic fistula after this pancreatectomy was 31.6%. The follow-up period was from 6 months to 6 years. All of the 19 patients were alive except 1 who was lost to follow-up after 1. 5 years. There was no recurrence nor new-onset diabetes mellitus. Conclusions Middle pancreatectomy was a proper operative approach for benign or low-malignant tumors located in the neck or the body of the pancreas. This pancreatectomy may preserve endocrine and exocrine function, and decrease the risk of postoperative new-onset diabetes mellitns.
2.Clinical efficacy and safety of montelukast combined with budesonide in the treatment of children

Objective To investigate the clinical efficacy and safety of montelukast combined with budesonide in the treatment of children with asthma. Methods 70 cases of children with asthma were randomly divided into group A and group B, included 35 cases in each group. Group A was treated with single budesonide and Group B was given montelukast combined with budesonide. The clinical efficacy of two groups of asthma was compared, the body temperature returned to normal time, the time of wheezing disappeared, the normal time of the laboratory index, the normal time of the chest radiograph, the inflammatory factors and the lung function indexes and the side effects. Results The clinical curative effect of group B was higher than that of group A(P<0.05); Body temperature in group B returned to normal time, the time of wheezing disappeared, the normal time of laboratory index was shorter than that of group A(P<0.05); Inflammatory factors and lung function were similar. After intervention, the inflammatory factors and lung function of group B were better than those of group A(P<0.05). There were no obvious side effects in the two groups. Conclusion The clinical efficacy and safety of montelukast combined with budesonide in the treatment of children with asthma can improve the symptoms of children, cause the symptoms to recite in a short time, reduce the inflammatory factors, improve lung function, no obvious adverse reactions, safety effective.
4.Postmortem Redistribution of Avermectin in Acute Poisoning death of Rabitts

Objective To investigate the postmortem redistribution of Avermectin in acute poisoning death of rabitts. Methods According to the minimum lethal dose intragastric of 250mg/kg avermectin, the avermectin contents in the heart blood and major organs and tissues of the rabitts after death of 0h~72h were assayed by HPLC method. Results Determination of clinical death time was 120.6±9.2min(x±s, n=10)by intragastric 250mg/kg avermectin to the rabitts; Lethal blood concentrations and lethal tissue concentrations of Avermectin were determined; Presence of postmortem redistribution of avermectin contents in the heart blood and major organs and tissues of the rabitts after death of 0h~72h were proved; Determination of the liver, kidney and lung were the best tissue samples for toxicological anaysis. Conclusion The data of Avermectin postmortem redistribution in the rabbits have important reference value for the forensic management of such cases.
5.Effect of curcumin on the activity and migration of as well as c-kit mRNA expression in melanocytes

Objective To explore the effect of curcumin on the activity and migration of as well as c-kit mRNA expression in melanocytes.Methods Human epidermal melanocytes were isolated from the prepuce in adolescents and subjected to primary culture.To estimate the effect of curcumin on the proliferative activity of melanocytes, some melanocytes were randomly divided into several groups to be cultured in the MelM-2 medium with or without the presence of 5, 10, 20 or 30 μmol/L curcumin, the MelM-2 medium containing curcumin of 5-30 μmol/L served as the drug control groups, and the MelM-2 medium without curcumin served as the blank control group.After 24 and 48 hours of culture, MTS assay was performed to evaluate the proliferative activity of melanocytes.Some cultured melanocytes were randomly divided into 4 groups to be cultured in the MelM-2 medium with 0, 5, 10 and 20 μmol/L curcumin respectively for 48hours.Then, wound scratch assay was conducted to estimate the migratory activity of melanocytes, and real-time fluorescence-based quantitative PCR to quantify the mRNA expression of c-kit in melanocytes.Statistical analysis was carried out by factorial design analysis of variance (ANOVA), one-way ANOVA and least significant difference (LSD)-t test.Results The proliferative activity of melanocytes was significantly decreased at 24 and 48 hours in the 30-μmol/L curcumin group compared with the negative control group (0.783 ± 0.053 vs.1.000 ± 0.018 at 24 hours, 0.637 ± 0.015 vs.0.993 ± 0.064 at 48 hours, both P < 0.05), while no significant differences were observed between the other curcumin groups and the negative control group (all P > 0.05).The 48-hour treatment with curcumin could significantly inhibit the migration of melanocytes in the 5-, 10-and 20-μmol/L curcumin groups compared with the control group (all P < 0.05).The mRNA expression level of c-kit was also significantly reduced at 48 hours in the 5-, 10-and 20-μmol/L curcumin groups compared with the control group (1.799 ± 0.372, 1.539 ± 0.224 and 1.026 ± 0.038 vs.3.371 ± 0.352, all P <0.05).Conclusion Curcumin at low concentrations (≤ 20 μmol/L) has no obvious cytotoxicity against melanocytes, but can inhibit the migration of and c-kit mRNA expression in melanocytes, while curcumin at 30 μmol/L can promote the apoptosis of melanocytes.
6.Neurodevelopmental outcome at 18 months of late preterm infants

Objective To study the neurodevelopmental outcome prospectively at 18 months of the late preterm infants.Methods Data from 7 584 live born neonates were collected between January and December.2009 in 3 hospitals located in the north of Chengdu City,Sichuan Province were collected,89 late preterm infants were brought into study ; 170 healthy full-term infants were chosen as the controls randomly.Neurodevelopment outcome was assessed by using neonatal behavioral neurological assessment(NBNA) at 40 weeks corrected gestational age,and Bayley scales of infant development was performed to obtain the physical development index (PDI) and mental development index (MDI) at 3,6,9,12 and 18 months corrected age.Neurodevelopmental outcome of late preterm infants was studied compared with that of the term infants.Results Sixty-three neonates born at the late preterm phase and 115 neonates born at the term phase were successfully followed up.The NBNA scores of the late preterm infants at 40 weeks corrected gestationa] age were significantly lower than those of the term infants.The proportion of the late preterm infants whose scores ≥37 was significantly lower than that of the term infants(82.5% vs 94.8%),the proportion of late preterm infants whose scores <35 was significantly higher than that of the term infants(4.8% vs 0),the proportion of the late preterm infants whose scores in 35-36 was significantly higher than that of the term infants (12.7% vs 5.2%,Z =-2.707,P < 0.05) ;At 3,6,9 and 12 months corrected age,the late preterm infants showed a significant lower PDI scores(t =-4.266,-4.594,-5.663,-2.584) and MDI scores (t =-7.121,-7.829,-7.038,-6.002) than those of the term infants(all P <0.05).Compared with the term infants,the late preterm infants still had lower MDI scores than the term infants at 18 months corrected age(t =-4.115,P <0.05),but no difference was observed in PDI scores between late preterm and the term infants (t =-0.957,P > 0.05).Conclusions Neurodevelopment outcome of the late preterm infants is delayed in the first year compared with term infants.At 18 months corrected age the mental development is still delayed compared with the term infants.Measures should be taken properly to improve the neurodevelopment of the late preterm infants in the early childhood.
7.The influencing factors of psychological health status and social disability in hashimoto thyroiditis

Objective To analyze of the influencing factors of mental health status and social disability for hashimoto thyroiditis(HT) patients.Methods The research employed Symptom Checklist 90 (SCL-90) and Social Disability Screening Schedule (SDSS) to evaluate the change and the influencing factors of mental health status and social disability of Hashimoto Thyroiditis patients.Result The scores of SCL-90 share,body symptom,sensitiveness relationship,depression,anxiety and social disability [(1.7±0.6),(2.0±0.7),(1.9±0.6),(2.0±0.60),(1.8±0.5) and (5.2±0.9),respective] in HT were increased compared with normal group[(1.4±O.4),(1.3±O.5),(1.6±O.5),(1.5±0.6),(1.4±0.4),(1.3±0.8) respective](P<0.01).In HT subjecte receiving levothyroxine(L-T4) therapy for 3 months,the scores of SCL-90 share score,body symptom,sensitiveness relationship.depression,anxiety and social disability were reduced respectively by(1.3±0.5),(1.5±0.5),(1.6±0.4),(1.6±0.4),(1.4±0.6) and(1.8±0.3)(all P<0.01).At the same time,the score of social disabilitv after therapy was increased compared with normal group (t=5.78,P<0.01).With the Logistic analyses.the score of social disability(F=1.16,P<0.01),TSH (F=2.05,P<0.01) and TP0(F=3.48.P<0.01)were risk factors of SCL-90 share score in HT subjects.Conclusion There were psychological health symptoms and social disability in HT case.TSH and TPO were risk factors of psychological health.L-T4Wag good for returning to normal mood.
8.Angiotensin Ⅱ AT1 receptor blockade modulates leptin,adiponectin and TNF-? in rat

Objective To investigate the effects of angiotensin Ⅱ AT1 receptor antagonist-Candesartan on production of leptin, adiponectin and TNF-? in rat. Methods 20 male Wistar Kyoto rats were randomly divided into vehicle and Candesartan treated groups. The dosage of Candesartan was 10mg?kg~ -1 ?d~ -1 . Food intake and bodyweight were recorded each week. Rats were sacrificed by decapitation after 17 weeks. Epididymal and perirenal fat were collected and weighted. Adipocytes were isolated from epididymal adipose tissue with collagenase. Diameter of adipocyte was evaluated. Blood chemistry, insulin, leptin, adiponectin and TNF-? were assayed. mRNA expressions of leptin, adiponectin and TNF-? of epididymal adipose tissue were assayed with RT-PCR. Results In Candesartan treated group, bodyweight and content of adipose tissue were significantly decreased, and adipocytes shrunk. Blood glucose and insulin levels showed no difference between two groups. Plasma leptin level and leptin mRNA expression in adipose tissue were lower, and plasma adiponectin level as well as adiponectin mRNA expression were higher in Candesartan treated group. Plasma TNF-? level was too low to detect with super sensitive kit, and mRNA expression of TNF? was decreased in Candesartan treated group. Conclusions Angiotensin Ⅱ AT1 receptor antagonist decreases the production of leptin and TNF-?, enhances synthesis as well as release of adiponectin in rat adipose tissue.
9.Treatment of unstable femoral fractures between rotor in elderly patients with bone cement longer handle ar-tificial femoral head replacement

Objective To evaluate bone cement longer handle artificial femoral head replacement treatment the clinical effect of the treatment of femoral fractures between rotor-instability.Methods There were 98 patients with fracture of instability-between the femur rotor ,women 60 cases,38 cases,male,aged 75 to 94 years old,average 83.5 years.Fractured by Tronzo-Evans type III 56 cases,type IV 42 cases.All use of bone cement longer handle arti-ficial femoral head replacement therapy ,to observe clinical therapeutic effect .Results All cases of postoperative X-ray showed both fracture reduction and fixation well between the rotor and prosthesis position is good .96 cases of post-operative for 6 to 24 months followed-up,the average 15 months.Harris score was 85.4%.Conclusion Bone cement longer handle artificial femoral head replacement has quick postoperative functional recovery ,weight-bearing activities earlier,which can avoid the advantages of long-term complications in bed ,instability is treatment of older femoral frac-tures between rotor reasonable operation method .

ObjectiveTo evaluate the performance of diagnosis-related groups,Beijing version (BJ-DRGs) which were locally developed in Beijing.MethodA total of 1.3 million inpatient records from 149 hospitals in Beijing in 2008 were drawn from Beijing Public Information Center.Coefficient of variation (CV) was used to measure the performance of DRGs system.ResultsBJ-DRGs produced the best CV results for expenditure.ConclusionsUnder Beijing's medical information condition,BJ-DRGs produced good performance.
