1.Insulin resistance and polycystic ovary syndrome

Insulin resistance likely plays a central pathogenic role in polycystic ovary syndrome and may explain the pleiotropie presentation and involvement of multiple organ systems.Hyperinsulinaemia leads to increased production of androgens resulting in anovulatory infertility.Women,especially those obese women with polycystic ovary syndrome,have an increased risk of developing type 2 diabetes and cardiovascular disease in their later life.Treatment of insulin resistance through lifestyle therapy or with a diabetes drug has become mainstream therapy to reduce the risk of diabetes,endometrial abnormalities,and cardiovascular disease events in women with polycystic ovary syndrome.
2.Warm Needling Combined with Iontophoresis of Chinese Medicine for Temporomandibular Joint Disorder

Objective: To observe the clinical efficacy of warm needling combined with iontophoresis of Chinese medicine for temporomandibular joint disorder (TMD). <br> Methods: A total of 100 TMD cases were randomly allocated into a treatment group and a control group, 50 in each group. Cases in the treatment group were treated with warm needling combined with iontophoresis of Chinese medicine, whereas cases in the control group were treated with Western medication. The therapeutic efficacies in two groups were assessed after three courses of treatment. <br> Results: The recovery rate and total effective rate were 54.0% and 98.0% respectively in the treatment group, versus 30.0% and 82.0% in the control group, showing significant differences (bothP<0.05). The recovery rates in the treatment group were higher than those in the control group respectively after 1, 2 and 3 courses of treatment, indicating that warm needling combined iontophoresis of Chinese medicine could substantially shorten the course of treatment. <br> Conclusion: Warm needling combined with iontophoresis of Chinese medicine is remarkably effective for TMD and deserves further clinical application.
3.Effect of Tripterygium hypoglaucum Hutch on MMP-13 protein expression and IL-12, IL-23 and IL-37 levels in serum and foot paws of rats with collagen-induced arthritis

[ ABSTRACT] AIM:To investigate the effect of tripterygium hypoglaucum Hutch ( THH) on collagen-induced ar-thritis ( CIA) in rats and its possible mechanism.METHODS:SD rats were randomly divided into normal group and CIA group.The rat model of type II CIA was successfully established and the model rats were randomly divided into 4 different groups:model group, dexamethasone group, THH (200 mg/kg) group, and THH (400 mg/kg) group.The contents of IL-12, IL-23 and IL-37 in the serum and foot paws of the CIA rats were detected by ELISA.The histopathological changes of the skin of the food paws were observed by HE staining.The protein expression of MMP-13 was determined by Western blot.The MMP-13 activity in the foot paws was detected by fluorescence labeling method.RESULTS:Compared with CIA group, THH at dose of 400 mg/kg significantly reduced the weight loss in typeⅡCIA rats ( P<0.01 ) .THH at dose of 400 mg/kg obviously decreased the contents of IL-12 by 28.31%, IL-23 by 41.57%in the serum and IL-12 by 30.78%, IL-23 by 39.46%in the foot paws, while IL-37 was significantly increased by 79.43% in the serum and 75.78% in the foot paws ( P<0.01) .The pathological changes of the subcutaneous tissues were improved by treating with THH (400 mg/kg).The protein expression of MMP-13 was significantly decreased by 31.82%(P<0.01), and the MMP-13 activity was also reduced.THH at dose of 200 mg/kg had no obvious improvement on the above indexes.CONCLUSION:THH has significant inhibitory effect on rat CIA by reducing the content of proinflammatory cytokines IL-12 and IL-23, increasing the content of anti-inflammatory factor IL-37, inhibiting inflammatory cell infiltration and vascular proliferation, and attenuating the protein expression of MMP-13 and MMP-13 activity in rats.
4.Study on Pharmacokinetics and in Vitro-in Vivo Correlation of Riluzole Tablets in Dogs

Objective To study the pharmacokinetic parameters of riluzole tablets and commercial riluzole capsules in Beagle dogs,and evaluate the correlation between the release rate in vitro and the absorption in vivo, and calculate the relative bioavailability of riluzole tablets. Methods Six Beagle dogs were treated with 50 mg of riluzole tablets or capsules,and then cross-treated by the other drug after 14-days wash out period. Blood concentration of riluzole was measured by high performance liquid chromatography ( HPLC ) . The atrioventricular model was used to calculate the pharmacokinetic parameters and Wanger-Nelson method was applied to assess the correlation between the release rate in vitro and the absorption in vivo. Results The t1/2 ,tmax ,Cmax ,AUC0-72 and AUC0-∞ in the tablet and capsule groups were (12. 43±3. 87) and (12. 57±3. 25) h,(6. 00±2. 60) and (6. 23±2. 72) h,(56. 24±16. 51) and (60. 82±18. 13) ng·mL-1 ,(1 255. 83±311. 39) and (1 283. 50±313. 81) ng·mL-1 ·h, (1 282. 57±322. 64) and (1 297. 22±297. 39) ng·mL-1 ·h,respectively. The relative bioavailability of capsules versus tablets was (105. 9±30. 6 )%. Conclusion HPLC method can be applied to measure the concentration of blood riluzole with little interference and high levels of repeatability and accuracy. The absorption in Beagle dogs between riluzole tablets and capsules was equal,and a good correlation between the in vivo absorption and in vitro release has been found in this study.

6.Resistant Bacteria of Nosocomial Infection in ICU:Distribution and Surveillance

OBJECTIVE To investigate the distribution and resistance of bacteria of nosocomial infection in ICU.METHODS A total of 128 strains were detected by double-disc synergy test.RESULTS Among 128 strains from ICU,Gram-negative bacilli,accounted for 61 strains(47.65%),fungi for 48(37.5%),and Gram-positive cocci for 19(14.84%).Escherichia coli and Klebsiella pneumoniae were 73.00% and 55.13%.CONCLUSIONS The strains isolated from ICU are antibiotic-resistant.It is very impotant to select antibiotics correctly for infection of the patients in ICU according to the results of antibiotic susceptibility tests.
7.Exploration of Optimal Scheme and Theoretic Mechanism for Treatment of Chronic Ulcerative Colitis

8.Application of computer-assisted telephone intervewing in the follow-up study of Keshan disease

Objective To explore the application of computer-assisted telephone interviewing in the follow-up study of endemic diseases,and to provide a reference and basis for wide range application of this method.Methods In this investigation,146 patients with Keshan disease from 2 provinces were recruited in.The study mainly included two questionnaire surveys:disease follow-up questionnaire survey and cardiac insufficiency quality of life scale survey.Survey methods included computer-assisted telephone survey and face-to-face questionnaire survey.In the 18-month of follow-up study,one computer-assisted telephone survey was done in the 3ed,6th,12th,15th month,and one face-to-face questionnaire survey in 9th and 18th month.Time-consuming and quality of the two survey methods were eompaired.The computer-assisted telephone survey database was designed using Epi Info 3.32software.Results The computer-assisted telephone survey took an average of (13.10 t 1.80)min,and face-to-facequestionnaire survey took an average of (22.04 ± 2.28)min,the difference was statistically significant (t =- 36.214,P < 0.05).The error rate of computer-assisted telephone survey was 6.80% (37/544),and that of face-to-face questionnaire survey was 11.07%(28/253),the difference was statistically significant(x2 =4.195,P < 0.05).The quality of computer-assisted telephone survey was better than that of face-to-face questionnaire survey.Conclusions Through the computer-assisted telephone interviewing,the work efficiency of investigators is improved during the follow-up study of Keshan disease.The validation function improves the accuracy rate during the process of data entry.Because of the application of telephone contact,a lot of research funding is saved.
9.Autologous fat transfer augmentation using external tissue expansion

Objective To observe the clinical effects of autologous fat injection augmentation with external tissue expander.Methods 26 cases wore the Brava device,a bra-like vacuum-based external tissue expander for 4 weeks before autologous fat injection augmentation.Patients resumed Brava wear for 14 more days after fat injection.Results 26 women had a mean augmentation volume at 3 months of 76 ml per breast after single fat injection.Follow-up ranged from 1 month to 12 months.There were no complications such as nodules,masses,or cysts occurred.Patient satisfaction was as sessed:25 cases (95.6 %) were satisfied with the cosmetic results.Conclusions The addition of Brava expansion before autologous fat grafting leads to significantly larger breast augmentations,with more fat graft placement,higher graft survival rates,and minimal graft necrosis or complications,demonstrating high safety and efficacy for the procedure.
10.Correction of secondary deformity after removal of polyacrylamide hydrogel in breast

Objective To evaluate the methods of correction for secondary deformity after removal of polyacrylamide hydrogel in breast.Methods From June 2006 to December 2007,the Center of Breast Plastic and Reconstruction at Chinese Academy of Medical Sciences admitted and treated 36 patients who experienced deformity after polyacrylamide hydrogel remoral in breasts.The average age of the patients was 27.5 years,and the time of consultation for the correction was from 6 months postoperatively.The patients who had preoperative MRI examinations showed that no visible polyacrylamide hydrogel remained in the breast were included in the study.The patients were classified according to the deformity of the breast and the chest wall tissue.Autologous fat injection grafting,silicon gel implant augmentation,and dermis grafting were performed for treating the deformity of the breasts after polyacrylamide hydrogel removal.Results During 3 to 18 months follow-up,the shape of the breast was improved and no complications such as infection,the sclerotic nodules,implant exposure occurred.35(97.2%) patients were satisfied with the result of the operations.Conclusions The correction for secondary deformity of breast after removal of PAHG should be performed at least 6 months after removal of polyacrylamide hydrogel.The optimal and nature contour of the breast may be recovered by combination of various surgical methods which are carefully selected according to the individual situation.
