1.A Clinical Study on Malignant Salivary Gland Tumors.

The authors analyzed retrospectively the 36 patients with malignant salivary gland tumors who were treated at Department of Oral and Maxillofacial Surgery, Pusan National University Hospital from February, 1989 to September, 1997. The results obtained were as follows: 1. There were 14 males( 38.9%) and 22 females(61.1%). The peak age of patients with major salivary gland tumors was the 6th decade, but with minor salivary gland was the 5th decade. 2. Of all salivary gland tumors, 5 cases arose in the parotid glands, 2 cases in the submanibular glands, 1 case in the sublingual gland and 28 cases in the minor salivary glands. 3. The incidence according to the anatomic primary site for minor salivary glands was 15 cases in the palate, 5 in the floor of mouth and 2 cases each arising in the tongue, lip, retromolar area and buccal mucosa. 4. Of all salivary gland tumors, adenoid cystic carcinoma was 5 cases in the major salivary glands and 8 in the minor salivary glands. mucoepidermoid carcinoma was 2 cases in the major salivary glands and 14 in the minor salivary glands and others were 2 cases of adenocarcinoma, 4 malignant mixed tumors and 1 undifferentiated carcinoma. 5. The incidence of cervical lymph node metastasis was 100%(2/2) in the submandibular glands, 80%(4/5) in the floor of mouth, 50%(1/2) in the tongue and 20%(1/5) in the parotid glands. The highest incidence of lymph node metastasis according to histopathological classfication was found in the high grade of mucoepidermoid carcinoma and tubular type of adenoid cystic carcinoma. 6. Nerve invasion was common in the adenoid cystic carcinoma. 7. The lung was the commonest site for distant metastasis comprising 7 cases among 7 cases in which distant spread occurred.

Most salivary gland calculi occur in the submandibular gland, but can also occur in the parotid gland and the sublingual gland. One giant sialolith is rarely reported, while the several cases of one or multiple sialolith in the submandibular gland have reported in the literatures. 3.A Case of Mucocele of the Submandibular Gland.

Cysts of the salivary gland are non-neoplastic, localized lesions of the duct system. They form about 6 % of lesions of the salivary glands. Mucoceles represent more than 70% of the salivary cysts and usually arise from minor salivary glands; however, major salivary glands, especially the sublingual glands, may also be involved. Characteristically, mucoceles consist of a soft fluctuant bluish or translucent swelling which contain mucus, varying in size. They are most commonly found on the lower lip, but may also occur on the buccal mucosa, floor of the mouth, palate, upper lip, and tongue. They are found in all age groups but are most frequent in children and young adults. There are two types of mucoceles-one in which mucus has extravasated into the tissues (extravasation cyst) and one which is lined with epithelium (retention cyst). Recently, we experienced an unusual case of huge extravasation mucocele arising from the submandibular gland. The mucocele and submandibular gland were completely excised surgically, which were confirmed by computed tomographic scanning and histopathologic examination.

4.Morphological evidences in circumvallate papilla and von Ebners' gland development in mice.

In rodents, the circumvallate papilla (CVP), with its underlying minor salivary gland, the von Ebners' gland (VEG), is located on the dorsal surface of the posterior tongue. Detailed morphological processes to form the proper structure of CVP and VEG have not been properly elucidated. In particular, the specific localization patterns of taste buds in CVP and the branching formation of VEG have not yet been elucidated. To understand the developmental mechanisms underlying CVP and VEG formation, detailed histological observations of CVP and VEG were examined using a three-dimensional computer-aided reconstruction method with serial histological sections and pan-Cytokeratins immunostainings. In addition, to define the developmental processes in CVP and VEG formation, we examined nerve innervations and cell proliferation using microinjections of AM1-43 and immunostainings with various markers, including phosphoinositide 3-kinase, Ki-67, PGP9.5, and Ulex europaeus agglutinin 1 (UEA1). Results revealed specific morphogenesis of CVP and VEG with nerve innervations patterns, evaluated by the coincided localization patterns of AM1-43 and UEA1. Based on these morphological and immunohistochemical results, we suggest that nerve innervations and cell proliferations play important roles in the positioning of taste buds in CVP and branching morphogenesis of VEG in tongue development.

5.A case of bilateral submandibular gland mucoceles in a 16-month-old child.

Mucoceles are common benign cystic lesions of the oral cavity that develop following extravasation or retention of mucous material from the major or minor salivary glands. Mucoceles are usually located in the lower lip (60 to 70% of cases), and the floor of the mouth is only involved in 6 to 15% of cases. Submandibular gland mucocele is extremely rare but should be considered in the differential diagnosis of swelling at the submandibular triangle in young children. We present the rare case of a 16-month-old child who was diagnosed with bilateral submandibular gland mucocele, presenting as serial swellings in both submandibular regions. We removed the cystic mass with the submandibular and sublingual glands to prevent recurrence.

7.The Effect of A Salivation Stimulation Method on Xerostomia in Colon Cancer Patients after Surgery.

PURPOSE: The aim of this study is to develop a salivation stimulation method and to verify the effect of a salivation stimulation method on xerostomia in colon cancer patients after surgery. METHODS: This study targeted 21 in the experimental group and 22 in the control group, who were diagnosed with colon cancer and underwent surgery at A university hospital. The salivation stimulation method consists of tongue movement for stimulating minor salivary glands in mouth and a parotid gland and submandibular gland massage with hands. The method was applied four times a day for 2 days the next day after surgery. RESULTS: On the first postoperative day, the experimental group showed a significantly larger amount of salivation than the control group (p=.010). On the second postoperative day, the experimental group showed a significantly larger amount of salivation than the control group (p<.001). On the first postoperative day, the subjective xerostomia scores in the experimental group and control group were not statistically significant (p=.165). On the second postoperative day, the experimental group showed a significantly lower subjective xerostomia score than the control group (p=.001). CONCLUSION: It is considered that providing cancer patients with this salivation stimulation method after surgery would reduce discomfort caused by xerostomia.

salivary gland tumors. Pleomorphic adenoma accounts for 60% of all parotid gland tumors, 50% of submandibular gland tumors, and only 25% of sublingual gland neoplasms. Fifty percent of all oral minor gland tumors are pleomorphic adenomas of which 55% arise in the palate, 25% in the lip, 10% in the buccal mucosa, and 10% from all other oral and oropharyngeal sites. The presence of tumors within the parapharyngeal space is rare. Some investigators have reported pleomorphic adenomas originating in the deep lobe of the parotid gland advancing to involve this region, as well as primary pleomorphic adenomas of nonparotid origin. We present a case report of pleomorphic adenoma in the parapharyngeal space with literature review. The tumor was completely removed under general anesthesia via submandibular incision with the division of mandible.]]>

10.A Case of Papillary Cystadenocarcinoma Arising from Submandibular Gland.

Papillary cystadenocarcinoma originated from salivary gland is a lower grade neoplasm that rises most commonly in major salivary glands, although involvement of lip, palate, buccal mucosa and tongue has also been described. Papillary cystadenocarcinoma was first defined in 1991 by WHO. Until then, it was classified as an atypical type of adenocarcinoma. We experienced a case of papillary cystadenocarcinoma of the submandibular gland, so we report this case with a review of the literature.
