The academic journal in health policy could be involved in all stages of the health policy process–agenda setting, policy making, policy implementing, and policy evaluating. ‘Health Policy and Management’ has been undertaking the role of an academic society in health policy. ‘Health Policy and Management’ will strengthen its role in health policy.

2.Surgical Treatment of Tennis Elbow

Five cases of tennis elbow seen during the time period from Jaunary 1983 to January 1985 had operative treatment. Of the five clinical cases, four elbows in three patients were treated with Nirschl and Pettrone operation. This operative technique included exposure of the extensor carpi radialis brevis, excision of the identified lesion, and repair. The following results were obtained. 1. Correct analysis of the patient's emotional stability was essential prior to undertaking surgical procedure for tennis elbow. 2. The results of Nirschl and Pettrone operation were graded as excellent in three elbows and fair in one.

3.Second-look Arthroscopy after Surgical Treatment for Osteochondral Lesion of Talus: Comparison of Mosaicplasty with Microfracture.

PURPOSE: To evaluate the results of mosaicplasty and microfracture after surgical treatments for symptomatic osteochondral lesion of talus (OLT) by second-look arthroscopy. MATERIALS AND METHODS: 7 cases of mosaicplasty and 7 cases of microfracture were reviewed who undertook second-look arthroscopy at 6 months or one year after undertaking mosaicplasty or microfracture for OLT between December 2004 and October 2005. The mean age at first operation was 43.6 years (Range, 20-59) (Mosaicplasty; 43.9 years, Microfracture; 43.4 years). The mean size of cartilage defect was 15.0 x 7.7 mm in mosaicplasty and 7.1 x 6.6 mm in microfracture. Clinical outcomes were evaluated by Freiburg ankle score. Cartilage healing state was evaluated by Insall's classification for chondromalasia during second-look arthroscopy. RESULTS: By the Freiburg ankle score, 9 ankles (6 in mosaicplasty, 3 in microfracture) had excellent and 5 (1 in mosaicplasty, 4 in microfracture) had good results at the times of second-look arthroscopy. By Insall's classification, consistency of the osteochondral grafts and congruity between grafts and native cartilage (Grade I) were shown in 9 (6 ankles in mosaicplasty, 3 ankles in microfracture), a fissuring (Grade II) in one ankle of mosaicplasty, a fasciculation (Grade III) in one ankle of microfracture, and partial exposure of subchondral bone (Grade IV) in 3 ankles of microfractures. CONCLUSION: Mosaicplasty was more excellent in consistency and hardness of cartilage than microfracture. In some cases of microfracture, cartilage healing was incomplete at 6 months postoperatively, so second-look arthroscopy is necessary to identify. If incomplete cartilage healing was shown, additional procedure such as microfracture or mosaicplasty was needed.

4.The Development of Quality Assurance Program for CyberKnife.

PURPOSE: Standardization quality assurance (QA) program of CyberKnife for suitable circumstances in Korea has not been established. In this research, we investigated the development of QA program for CyberKnife and evaluation of the feasibility under applications. MATERIALS AND METHODS: Considering the feature of constitution for systems and the therapeutic methodology of CyberKnife, the list of quality control (QC) was established and divided dependent on the each period of operations. And then all these developed QC lists were categorized into three groups such as basic QC, delivery specific QC, and patient specific QC based on the each purpose of QA. In order to verify the validity of the established QA program, this QC lists was applied to two CyberKnife centers. The acceptable tolerance was based on the undertaking inspection list from the CyberKnife manufacturer and the QC results during last three years of two CyberKnife centers in Korea. The acquired measurement results were evaluated for the analysis of the current QA status and the verification of the propriety for the developed QA program. RESULTS: The current QA status of two CyberKnife centers was evaluated from the accuracy of all measurements in relation with application of the established QA program. Each measurement result was verified having a good agreement within the acceptable tolerance limit of the developed QA program. CONCLUSION: It is considered that the developed QA program in this research could be established the standardization of QC methods for CyberKnife and confirmed the accuracy and stability for the image-guided stereotactic radiotherapy.

5.Effects of an Education Program for Effective Nursing Intershift Handoff Communication on Nurses' Intershift Performance, Self-Efficacy, and Interrelationship Stress among Clinical Nurses: A Pilot Study.

PURPOSE: The purpose of this study was to examine the effects of an education program for effective nursing intershift handoff communication (NIHC) on nursing intershift performance, self-efficacy, and interrelationship stress among clinical nurses. METHODS: The study was a pilot study using a one-group pretest-posttest design. Participants were 40 clinical nurses from Seoul/ Gyeonggi Province. The handoff dialogue pattern was developed by three professors and one doctoral student in 2014, and was based on an education program for effective (NIHC) from dialogue analysis. The education program as an experimental treatment was provided for 3 1/2 hoursto clinical nurses. Measures included nursing intershift performance scale, self-efficacy scale, and interrelationship stress scale. Collected data was processed and analyzed with SPSS PC+ Version 21. RESULTS: There were significant differences in nursing intershift performance (hand over: t=-12.18,p<.001; undertaking: t=-6.88, p<.001), self-efficacy (hand over: t=9.42, p<.001; undertaking: t=8.13, p<.001), and interrelationship stress (hand over: t=11.46, p<.001; undertaking: t=10.49, p<.001) between pre and post-test. CONCLUSION: Findings indicate that this education program can be utilized as a manual, and can be applied to improve nursing intershift performance and self-efficacy, and to decrease interrelationship stress for effective (NIHC) among clinical nurses. Findings will also help to reduce incorrect performances and increase work efficiency in clinical practice for nurses.

6.Effect-Evaluation of Nutritional Education Program Using Internet for School Children.

This study was carried out to evaluate the effects on elementary school children of Internet-based nutrition education programs. The subjects consisted of 80 boys and girls (book-loaming group : 42, Internet-based teaming group : 38) in the 5th grade of elementary school. The results were as follows : The evaluation of Internet-based education programs by Internet-teaming group showed above average in all domains. As the favorite and wholesome menu chose 'nutrition kingdom' (52.6%) made of animation. The educational effect of this program was evaluated by comparing the methods of the Internet-learning group with those of the book-learning group. For effect-evaluation were measured the degree of nutrition knowledge and diversity score of dietary intake from before and after education. Before undertaking the program, there was no significant difference between the two groups in terms of their nutrition knowledge. However, the nutrition knowledge of the Internet-based teaming group after the program had increased significantly (p<0.01) compared to the book-learning group. There was no significant difference between the two groups in terms of the diversity of dietary intake before the start of the program, but upon its completion, the Internet-based learning group showed a significant increase compared to the book-loaming group (p<0.05). This suggests that Internet-based educational programs could be effective in drawing the interest and boosting the concentration levels of schoolchildren. In conclusion, Internet-based nutrition education shows the potential to be an effective alternative education method.

7.TREATMENT OF MARCUS GUNN PHENOMENON.

Marcus Gunn described for the first time a syndrome consisting of unilateral, congenital ptosis and rapid, exaggerated elevation of the ptotic eyelid on movement of the mandible to the contralateral side. The clinical findings and management of 2 patients with Marcus Gunn phenomenon had been reviewed. Preoperative measurement of ptotic amount, levator function by Berke's method, and marginal limbal distance were evaluated. The amount winking of the upper eyelid on primary gaze was graded on as scale from I to III. The operation was performed two patients under local anesthesia, one by unilateral levator resection, one by orbicularis oculi muscle flap. As a result, the correction of blepharoptosis was possible without severe complications. However, moderate degree of jaw-winking, slight undercorrection, transient lagophthalmos was inevitable. The management of patients with Marcus Gunn phenomenon is a challenging endeavor. Therefore, a comprehensive medical and ophthalmological evaluation of the patient and a detailed history are mandatory before undertaking the successful treatment of patients with this syndrome

8.The Coping Process of Family Caregivers for Demented Elderly.

PURPOSE: The purpose of this study was to understand the coping process that family caregivers experience during the process of care, to grasp coping behaviors used to solve problems and, finally, to develop a substantive theory by analyzing the coping process. METHOD: The methodology of collecting and analyzing data used in this study followed the ground theory of Strauss and Corbin(1998). Data were collected through in-depth interviews with open-ended and descriptive questions about the subjects' coping experiences, which were audio-taped and transcribed. The survey was conducted between February 2000 and February 2001. The subjects of this study were 17 women. RESULT: The result of the study is as follows. The coping process of family caregivers while taking care of the demented elderly was found to have six stages: problem recognition: undertaking care: struggling: mental control: burden mediation; and acceptance. These stages proceed with reciprocal action and in cycle. CONCLUSION: The result of this study is helpful for developing effective and individual nursing strategies suitable for each coping stage. It is necessary to develop an assessment tool that can judge caregivers' coping stage based on the result of this study. Moreover, we need continuous study to practice nursing mediation and to analyze the change about nursing effect and family members' adaptation.

9.Airway fire injury during rigid bronchoscopy in a patient with a silicon stent: A case report.

Therapeutic bronchoscopy is widely employed as an effective first-line treatment for patients with central airway obstructions. Airway fires during rigid bronchoscopy are rare, but can have potentially devastating consequences. Pulmonologist and anesthesiologist undertaking this type of procedure should be aware of this serious problem and be familiar with measures to avoid this possibly fatal complication. We report the case of a 24-year-old patient with a silicone stent who experienced an electrocautery-induced airway fire during rigid bronchoscopy.

10.Development of a Five-Day Basic Microsurgery Simulation Training Course: A Cost Analysis.

The widespread use of microsurgery in numerous surgical fields has increased the need for basic microsurgical training outside of the operating room. The traditional start of microsurgical training has been in undertaking a 5-day basic microsurgery course. In an era characterised by financial constraints in academic and healthcare institutions as well as increasing emphasis on patient safety, there has been a shift in microsurgery training to simulation environments. This paper reviews the stepwise framework of microsurgical skill acquisition providing a cost analysis of basic microsurgery courses in order to aid planning and dissemination of microsurgical training worldwide.
