3.COMPARISON OF WEAR RESISTANCE AMONG RESIN DENTURE TEETH OPPOSING VARIOUS RESTORATIVE MATERIALS.

The aim of this study was to compare wear resistance of resin denture teeth opposing various restorative materials. The wear resistance of conventional acrylic resin teeth (Trubyte Biotone) and three high-strength resin teeth(Bioform IPN, Endura, SR-Orthosit-PE) opposing different restorative materials (gold alloys, dental porcelain, composite resin) was compared. Wear tests were conducted with a sliding-induced wear testing apparatus which applied 100,000 strokes to the specimen in an mesio-distal direction under conditions of 100 stroke/min and constant loading of 1Kgf/tooth. Wear resistance of the resin denture teeth was evaluated by the following criteria : 1) wear depth, 2)weight loss, and 3) SEM observation. Results were as follows. 1. When opposed to gold alloys and composite resin, high-strength resin teeth showed superior wear resistance compared to acrylic resin teeth. But, in cases opposing dental porcelain, differences between the wear of the high-strength and acrylic resin teeth were not statistically significant (p<0.05). 2. When comparing wear resistance among high-strength resin teeth, opposing gold alloys, Endura was slightly more resistant and while in cases opposing dental porcelain, SR-Orthosit-PE was showed to be slightly resistant(p<0.05). 3. The wear of high-strength resin teeth was greater by 5 to 7 times when opposing porcelain and 2 to 3 times when opposing composite resin compared to gold alloys(p<0.05). 4. SEM observations of the wear surface showed that wear of resin teeth opposing gold alloys is a fatigue type of wear and wear of resin teeth opposing dental porcelain is fatigue and abrasion type of wear. Trubyte Biotone showed more severe fatigue type of wear than high-strength resin teeth. In conclusion, the use of dental porcelain should seriously be considered as restorative material in cases opposing resin denture teeth and improvement seems to be needed on resin teeth in the areas of wear resistance.

4.The Treatment of Fracture of Tibial Shaft with Rush Nailing and Early Weight Bearing

Twenty nine patients who had fracture of tibial shaft were treated with closed Rush nailing and early weight bearing. 1. The average age was 37.8 years (range from 20 to 65), and 21 were men and 8 women. Eighteen cases were open fractures and 11 closed. 2. The time for clinical union was averaging 15.4 weeks. In some cases callus appeared as early as 3 weeks after nailing and in most cases appeared at 8 weeks. 3. Mild angular and rotational deformity of the fractured shaft were developed as complication but did not present any clinical problems. No deep wound infection and pulmonary embolism were encountered. 4. The nails allowed a certain dynamic controlled motion at the fracture site which was probably beneficial to early callus formation. 5. This type of closed nailing method required no medullary reaming, further periosteal stripping and muscle detachment. The allowance of early knee and ankle movements and early weight bearing after surgery could be listed as the advantage.

5.Pulmonary Function in Kyphosis and Scoliosis of the Spine

Kyphosis or scoliosis are developed with various causes. Mild deformity of Spine does not change the cardiopulmonary function while the more aggravate the deformity of spine, the worse the cardiopulmonary function. In 1968, Nilsonne and Lundgren have reported that the mortality in patients with severe spinal deformity was twice as high as in normal person and heart or lung disease was the main cause of death. The problem of cardiopulmonary function should be considered in the treatment of pateints who have the severe deformity of the spine. The author studied the degree of curvature of spine, pulmonary function and its correlation in 18 cases of severe kyphosis or scolissis which deformities were caused from spinal tuberculosis poliomyelitis or idiopathic Scoliosis. The results of this study are as follows; 1. Over all average degree of kyphosis of spine was 101.4; vital capacity was 61. 1% of normal and maximum breating capacity was 84. 4% of normal. 2. Vital capacity and maxmium breating capacity have a significant negative correlation with the increase of kyphosis and scoliosis of the spine. 3. Vital capacity has a positive correlation with maximum breathing capacity in the increase of the kyphosis, but not significant. 4. Timed vital capacity was in normal limit in spite of the change of kyphosis and scoliosis of the spine. 5. Change of pulmonary function after surgical correction of the deformity of spine was discussed.

6.Effects of n-Hexane and Benzene on Tibial Nerve for Rats.

n-Hexane and benezene are organic compounds which have been widely used as industrial solvents. However, they are also increasingly recognized as important pollutants in working environment. The purpose of this study is to analyze neurotoxicity of benzene and n-Hexane. In this study, tibial nerve of Sprague-Dawley rats were observed after exposing them to two different concentrations of these compounds (6000 ppm of n-Hexane and 2000 ppm of benezene) which were known to be the levels to cause subacute toxicity for the three different periods; two weeks, four weeks, and sex weeks. The following results were obtained from the analysis of variance, Duncan's multiple comparison test, and regression analysis: 1) Myelin sheath thickness of nerve fiber for two n-Hexane exposed groups (four weeks and six weeks) were both reduced compared with the control group and the benezene exposed group. 2) There were positive relationships between nerve fiber diameter and myelin sheath thickness for both exposed and control groups. 3) There were significant difference in myelin sheath thickness from equal diameter nerve fibers between benzene exposed group and control group, but the greater number of thin myelin sheath were observed for n-Hexane exposed group compared with control group. Thus, it is concluded that n-Hexane tends to reduce the rate of growth of nerve fiber more than the benzene and control group. While these results shed light on understanding the effects of benzene and n-Hexane, the duration of exposure was not long enough to apply these results to real working environments. In addition, to further understand the mechanism of nerve degeneration caused by organic solvents, both epidemiological and biochemical studies should accompanied by this kind of study.

7.A Case of Autonomous Ovarian Cyst with Severe Vaginal Bleeding.

Small ovarian cysts(<0.7 cm in diameter) are common in prepubertal children. Occasionally larger cysts occur in young girls with isosexual incomplete precocious puberty in the absence of LH and FSH elevation. The mechanism of production of these autonomous functioning cysts is unknown. A 3 year 6 month-old girl was referred to our department of pediatrics because of severe vaginal bleeding. On examination, she was tall and had breast enlargement of Tanner stage II. On hormonal study, FSH(0.38mIU/ml) was suppressed and estradiol(62.2pg/ml) was markedly elevated, and GnRH stimulation failed to evoke an increase in gonadotropin. Bone age was advanced. Pelvic ultrasonography showed both huge ovarian cysts. Brain MRI showed no abnormalities. We underwent both ovarian cystectomy and histological examination showed follicular cyst in left ovary and luteal cyst in right ovary. After ovarian cystectomy vaginal bleeding stopped and hormonal abnormalities were normalized. We report a case of autonomous ovarian cysts presenting severe vaginal bleeding treated with ovarian cystectomy.

9.Treatment of PDA in premature newborns with mefenamic acid.

This study was conducted to examine the effect of mefenamic acid for treatment of PDA in premature newborns. Ductus arteriosus is reopened by locally produced prostaglandin E2 in a premature newborn during hypoxia. Mefenamic acid is one of non-steroidal antiinflammatory drugs acting by inhibition of cyclo-oxygenase in the prostaglandin synthesis pathway. For three premature newborns with PDA, we administered mefenamic acid and evaluated them with echocardiography to study the effect of mefenmic acid for closure of PDA. In all three babies, ductus arteriosus was closed successfully. We feel that mefenamic acid is safe and effective medication for treatment of PDA in premature newborns, but further-study need to be conducted with larger numbers of cases to confirm this effect.

10.A Case of Hyperkeratosis of the Nipple and Areola Aggravated During Pregnancy.

We report a case of hyperkeratosis of the nipple and areola occuing in 33 year old woman. She did not have a history of endocriruipathy, horrnonal therapy and other ceatoses, so this case seems to be the nevoid form by Levy-Franckel classification. Interestingly, however the lesion which had developed before marriage suddenly aggr avated during pregnancy. This may be a unique feature which has not been reported elsewhere. Histapathologic findings are compatible with breviously reported eases. There was no response to topical steroid and retinoic acid treatrnent.
