1.A Case fo Cheilitis Granulomatosa.

Cheilitis granulomatosa is a macrocheilia having a sudden onset and a progresaive eourse that terminates in chronic enlargement of the lips. Labial enlargement can occur alone or in combination with intermittent facial paralysis and fissured tongue. The traid of abnormalities has been termed the Melkersaon-Rosenthal syndrome. The term, cheilitis granulomatosa, is now generally accepted as the description of the labial conditian with or without the other components of the triad. We have experiegced a case of cheilitis granulomatosa in a 23-year-old man and excellent result were obtained after local triamcinolone acetonide injections. On the first examination, the upper lip was estimated at approximately twice the normal size and the tongue was moderatedly furrowed. The consistency was elastic, but firm due to the nan pitting edema of the entire upper lip. We have treated him with intralesional injections of triamcinolone acetonide. Total injeetions were fifteen times in 6 months period. After the treatment, climcally ohaerved ehanges eonsisted of a decrease in lip volume, diminished edema plus a gradual softening of lip texture.

2.A Case of Multiple Basal Cell Epitheliomas Arising in An Epidermal Nevi.

Basal cell epithelioma arising from epidermis and its appendages is a common skin tumor. But multiple occurence of basal cell epitheliomas and that of arising in an epidermal nevi are extremely rsre. Since the first report by Toyama and Kojima in 1937, only few cases were reported in the world literatures. We observed a case of multiple basal cell epitheliomas arising in an epidermal nevi in a 54-year-old female patient who had rhomboidal verrucous patch on the right side of the cheek and arciform erythematous verrucous eruption from the right side of the eyebrow to the lower eyelid. Clinical features were not compatible with any type of basal cell epithelioma. But histological finding revealed basal cell epithelioma with epidermal nevi. Literatures were briefly reviewed.

3.Clinical Study of Ku-54 (Aplace(R)) in Gastric Ulcer.

Although our present knowledge of the etiology of peptic ulcer is incomplete, the presence or absence of peptic ulcer is determined by the delicate interplay between aggressive factors (secreted gastric acid and pepsin) and defensive factors (mucosal resistance). Peptic ulcer is produced when the aggressive effects of acid-pepsin dominate the protective effects of gastric or duodenal mucosal resistance by predominance of aggressive factors or interruption of defensive factors. KU-54 enhances mucosal resistance to tissue injury by the increase of gastric mucosal blood flow, the stimulation of gastric mucosal metabolism, the increase of glycoprotein of gastric mucus, and the increase of ATP of gastric mucosa. We have treated 38 cases of gastric ulcers with KU-54 300 mg daily for 4-12 weeks for the evaluation of the therapeutic efficacy. Endoscopic, clinical, and laboratory assessments were undergone before and after 4 ~ 12 weeks of the treatment. Major symptoms of gastric ulcer have been improved in 83.3% after the medication with KU-54. The healing rate of gastric ulcer evaluated by endoscopy was observed in 33.3% after 4 weeks, 73.3% after 8 weeks, 76.6% after 12 weeks of the medication with KU-54. The utility rate of KU-54 was 86.7%. We could conclude that KU-54 is the utilizable drug for gastric ulcer.

4.Non-operative Dilatation of Corrosive Esophageal and Gastric Angular Stricture: A Case reoprt.

Acid ingestion causes a coagulative necrosis of the surface epithelium of the upper gastromtestinal tract. Its late sequence is luminal stenosis which frequently requires surgical repair. Nevertherless, in many cases, non-operative dilatation of luminal stenosis have been applicated, such as bouginations, balloon dilatations, endoscopic laser therapy, endoscopic electrocoagulation, endescopic microwave coagulation. These methods may give some considerable benefits in unoperable cases. Recently, we experienced a case of a 60-year-old man who had severe corrosive esophageal and gastric angular strictures by accidentally ingested hydrochloric acid and after many tiems of application of non-operative dilatation was able to have normal diet without dysphagia. So, we report this case with a review of literatures.

The nail has been considered to be the best human model to evaluate the general body state at all ages. The present study was disigned to measure the nail growth rate and to define the correlation of them with other values of nail size. The subjects were 373 males and 812 females whose ages from R months to 79 years. The results were as follows: 1. The linear elongation of the nails were most rapid in the 14 to 15 year-old age group. It decreased steadily as the subjects got older and olr1er. 2. Sex differences in the nail growth rate were not statistically significant. R. Where were no statistically significant differences in the linear growth rate of nails between right and left hands. 4. The middle finger nail grew more quickly than others, while the thumb and little finger nails grew more slowly. 5. Linear growth of nails were not significantly correlated to the thickness and length of nails and the length of lunulae.

7.A Review of the Endoscopic Extraction in 52 Cases of the Upper Gastrointestinal Foreign Bodies.

A clinical review was performed in 52 cases with the foreign body in the gastrointestinal tract who underwent the endoscopic extraction of fereign body at our hospital from June 1985 to October 1989. The results are as following. 1) The most prevalent age was under 10 years of age (21 cases, 40.4%), and male was more prevalent than female (M:F=1.6:1). 2) The most common site of foreign bodies was esophagus (35 cases, 67.3%), next stomach (15 cases, 28.9%). 3) The foreign bodies in esophagus were coins (18 cases, 51.4%), meats (7 cases, 20.0%), bones (5 cases, 14.3%), other food materials or metals (5 cases, 14.3%)in order of frequency, and those in the stomach wer bezoars (7 cases, 50.0%), coins (2 cases, 14.3%), suture materials (2 cases, 14.3%), others (baduk stone, pin, tack, cross). 4) The combined diseases with upper gastrointestinal foreign bodies were the passage disturbances (11 cases, 21.2%) due to stenosis, and schizophrenia (1 case), aleoholism (1 case). 5) Complications by foreign bodies were 8 cases (15.4%), such as eisophageal laceration or ulceration (4 cases), esophaigeal perforation (1 case), aspiration pneumonia(1 case), duodenal obstruction (2 cases). 6) Successful rate of the endoscopic extraction was 88.5%(46 in 52 cases). And 6 unsuccessful cases were 4 cases who have gastric bezoars too hard and large to extract, a case with esophageal perforation, and a case with bezoar in the third portion of duodenum.

8.Sequential Changes of Extracellular Matrix mRNA in Anti-GBM Antibody Induced Crescentic Glomerulonephritis in the Rabbit.

Progressive renal fibrosis is considered to be the final common pathway leading to chronic renal insufficiency, however, the mechanism regarding renal fibrosis in renal injury is not well understood. Recently, several kinds of cytokines have been known to be related to fibrosis after renal injury. The interaction between elements regulating fibrogenesis would be better understood by looking at the effect of TGF-beta1 on the synthesis and accumulation of extracellular matrix, especially collagenous proteins. Crescentic glomerulonephritis (CGN) was induced in New Zealand White rabbits by administration of guinea pig anti-GBM IgG after sensitization with guinea pig IgG; and their kidneys were analyzed for the development of crescents and fibrosis through sequential renal biopsies. Serum creatinine levels in a time course progressively increased until day 15. We semi-quantitatively assayed the levels of the expression of alpha1(I) collagen mRNA and TGF-beta1 mRNA factored for GAPDH mRNA using RT-PCR. We observed a progressive interstitial fibrosis and the expression of collagen I both in the cortex and medulla. The effect of repeated renal biopsy itself on pathology and on the expression of alpha1(I) collagen mRNA and TGF-beta1 mRNA in a time course were not significant, but a very mild increase of the expression of alpha1(I) collagen mRNA was noted at day 15. Histology showed a progressive crescent formation and interstitial fibrosis in a time course that roughly paralleled the expression of alpha1(I) collagen mRNA in both cortex and medulla. TGF-beta1 mRNA was hardly expressed at day 0 in cortex as well as in medulla. It was elevated from day 1, peaked at day 7, and then decreased. In medulla, TGF-beta1 mRNA was noticeably expressed at day 1, peaked at day 4, and then decreased. The expression of alpha1(I) collagen mRNA was seen even before inducing CGN. It was gradually and continuously increased until day 15 both in cortex and medulla. These results suggest that the expression of TGF-beta1 mRNA precedes that of alpha1(I) collagen mRNA in the early stage of CGN and has a central role for provoking the accumulation the collagen I, the most representative interstitial extracellular matrix, in the rabbit model CGN induced by anti-GBM antibody. We conclude that the measurement of the expression of TGF-beta1 mRNA and/or alpha1(I) collagen mRNA in a biopsy sample can be a useful predictor for renal outcome.

9.Endoscopic Enucleation of a Gastric Lipoma by Strip Biopsy.

Gastric lipomas are rare benign submucosal tumors which can present the diagnostic and therapeutic problems. The preferred treattment is observation or local excision. Since preoperative diagnosis and differentation from malignant tumors can be difficult by use of the conventional diagnostic modalities such as X-ray or endoscopic examination, patients are sometimes subjected to more extensive surgical procedures than warranted. As to the diagnosis of submucosal tumors of the upper digestive tract, a newly developed diagnostic modality of endoscopic ultrasonography(EUS) allows us to visuialize the structures underlying the gastrointestinal wall in a noninvasive maneuver, and can contribute to make differential diagnosis and decision of management. Strip biopsy is an endascopic tissue resection technique which permits resection of both mucosal and submucosal tissue regardless of the morphological type of the lesion, because the submucosal saline injections during the procedures make the flat or depressed lesions to be elevated. Now, we report a 56-yr-old female which had a Yamada type I polypoid gastric submucosal lipoma in the antrum, which could be performed the different diagnosis and successful endoscopic removal by endoscopic ultrasonography and strip biopsy.

10.Estimation of the Depth of Invasion in Depressed Type of Early Gastric Cancer by Endoscopic Ultrasonography.

A new method for combining ultrasonography snd endoscopy, endoscopic ultrasonography (EUS), was developed to improve the diagnostic accuracy of ultrasound by direct image of the target lesion via the gastrointestinal lumen. EUS has the unique ability to provide clear sonographic images of the wall structure of the stomach. This capability can be ultilized in assessing the depth of cancer invasion of the stomach by identifying which layer the ultrasonographic change extends to. However, the depressed types of early gastric cancer(EGC) are often accompanied by ulcerative change in the tumor foci. And the ultrasonographic picture is also influenced by the ulcerative change.
