2.Pheochromocytoma found during Transurethral Resection for Bladder Tumor - A case report .

Unsuspected pheochromocytoma in a surgical patients may lead to serial complications. These include extreme hypertension, taehycardia and cardiac arrythmias which lead to cardiac arrest, cerebrovascular accident and renal failure in the postoperative period. Pheochromocytoma arising in the urinary bladder is a rare tumor. We recently experienced a case of pheochromocytoma which was found during transurethral resection for biopsy of bladder tumor under spinal anesthesia. The blood pressure suddenly rose during resection. of the tumor. It was difficult to arrive at a correct diagnosis in this case because of the predominant bladder symptams without hypertenaive attack. Therefore, final diagnosis was made as the pheochromocytoms after histological examination. The resection of the tumor was performed under general anesthesia after careful studies. The patient was not treated during the preoperative period as she had no particular symptoms, but Regitine was glven during surgery. Regitine was used twice to control the transient hypertension during manipulation of the tumor. After removal of the tumor, the blood pressure was well regulated without vasopressor drugs. We report a case of pheochromocytoma of urinary bladder with successful anesthesia management and the use of neuroleptanesthetics.

3.Health Locus of Control and Eating Behavior of Obese High School Girls.

The purpose of this study was to investigate the relationship between health locus of control (HLOC) and the eating behaviors in obese high school girls. The sample consisted of 262 obese high school girls in Seoul and Kangwon-Do. The results of this study were as follows: 1. The average scores of HLOC were HLOC- Internal; 4.06, HLOC-External; 2.47, and HLOC-Chance; 2.15. 2. The average scores of eating behavior factors were Disinhibition ; 2.91, Hunger ; 2.73, Dietary Restraint ; 2.55. 3. The HLOC-Internal and eating behavior (dietary restraint, disinhibition and hunger) were correlated positively. The HLOC- Chance and hunger was correlated positively. But HLOC-Chance and other eating behavior factors (dietary restraint & disinhibition) were not correlated in the level of statistical significances. The HLOC-External and eating behavior (dietary restraint, disinhibition and hunger) were correlated positively. 4. There was not a significant difference in HLOC depending on the existence of an obese sibling. There were significant differences in HLOC-Internal and HLOC- External depending on the evaluation of one's body figure. 5. There were significant differences in disinhibition and hunger depending on the existence of obese sibling. Also, there were significant difference in dietary restraint according to self perception of who is obese or not (t=3.342, p=.001). This study has shown a correlation between HLOC and eating behavior. As a result of these findings, clinical and school nurses should be involved in management and counselling obese girls concerning individual health locus of control and eating behavior.

4.The Severity of the Pediatric Patients Visiting Emergency Center.

This study was attempted to help in explore new direction about classification of the severity of the pediatric patients visiting emergency center. Data were collected from 276 patients who visited emergency center of E University Hospital during 3 months period from March 1, to May 31,1999. The results were as follows: 1. The degree of severity of the pediatric patients visiting emergency center shown ranged 0-18 and averaged .87. 2. With the respect to the severity of the pediatric patients visiting emergency center, there were statiscally significant difference in patients' visiting time(F=2.607, p=.025), disease classification(F=9.606, p=.000), consciousness level(F=71.499, p=.000), period of symptom manifestation (F=2.262, p=.030), pediatric patients protector's thinking about pediatric patients state (F=16.833, p=.000), treatment outcome (t=5.362, p=.000), duration of stay at emergency center(F=23.944, p=.000).

6.A Cytogenetic Study of Recurrent Spontaneous Abortion.

OBJECTIVE: The purpose of this investigation is to determine the frequency of chromosomal or genetic causes of recurrent spontaneous abortion. METHODS : A cytogenetic study was made in of 921 couples for 13 years from January 1984 to December 1997 in which the woman was ascertained to have had two or more spontaneous abortions at our Cytogenetic Laboratory, Institute of Reproductive Medicine and Population, Seoul National University. RESULTS : The overall incidence of chromosome anomaly was 80 out of 921 (8.7%). There were 34 cases (3.69%) of reciprocal balanced translocation and 13 cases (1.41%) of Robertsonian translocation. Also 17 cases (1.85%) of inversion and 5 cases (0.54%) of X chromosome mosiacism was observed. In the case of reciprocal balanced translocation, chromosome 8,6,7,13 were preferentially involved over others. And in the case of Robertsonian translocation, chromosome 13 was preferentially involved. CONCLUSION: Our study demonstrates that cytogenetic analysis is indicated in couples with 2 or more spontaneous abortion and about half of these disorders are reciprocal balanced or Robertsonian translocations.

8.Pure Motor quadriplegia in Bilateral Medial Medullary Syndrome.

Medullary pyramid is the one place where corticospinal fibers are isolated as the pyramidal tract, and the result of such lesions has been a flaccid hemiplegia. Medial medullary syndrome may occur bilaterally, resulting in flaccid quadriplegia with facial sparing, bilateral lower motor neuron signs of the tongue, and complete loss of position and vibratory sensation affecting all for extremities. Occasionally, only the pyramid is damaged, resulting in a pure motor quadriplegia without other medullary signs. We report a 78-year-old man who suddenly developed flaccid quadriplegia without any other medullary signs. Brain MRI showed bilateral medial medullary infarctions that was probably due to anterior spinal artery occlusion.

9.The degree of Satisfaction with Nursing Care of Pediatric patients visiting Emergency Center.

The purpose of this study was to provide qualified nursing care to examine the degree of satisfaction with nursing care of pediatric patients visiting emergency center. Data were collected from March, 8 to June, 5, 1999 from 304 subjects including 2 University hospitals located in Seoul. Using the 23 item questionnaire, which made out by Davis CHECSS tool through modification and supplement by researchers. The collected data were scored by use of means and standard deviations according to the degree of satisfaction with nursing care of pediatric patients visiting emergency center and each item as an independent variable was analysed by t-test and ANOVA test. The results are as follows. 1. The degree of satisfaction with the nursing care of pediatric patients visiting emergency center showed as an average of 3.69. 2. With the resept of general characteristics related to pediatric patients visiting emergency center, there were statiscally significant difference in duration of stay at emergency center(F=2.908, p=.014), type of disease(F=1.777, p=.046). The degree of satisfaction with the nursing care of pediatric patients of examiners showed relatively high, but emotional care and information supply are required.

10.A Clinical Study of the Pediatric Patients Visiting Emergency Center.

The purpose of this study was to provide basic data of nursing intervention through understanding disease pattern of pediatric patients. Data were collected from 3,016 patients who visited emergency center of E university Hospital during one year period from January to December, 1997. The results were as follows : 1. The most common type of disease was respiratory disease(26.7%). 2. According to the developmental age, toddler (35%) ranked first followed by preschooler(22.1%), infant(18.5%), schooler(16.0%), adolescent (5.6%), neonate(2.7%). 3. The proportion of males to females, male patients(59.8%) outnumbered female(40.2%) by the ratio of 1.48 : 1. 4. Visiting patients were seasonally more frequent in spring(29.8%) and summer(29.3%) than fall or winter. 5. Comparing weekly distribution, week end involing holiday outnumbered week day(48.5%). 6. The visiting time predominattly high from 8 : 00 PM to 0 : 00 AM (29.8%) during a day. 7. The majority duration of stay at Emergency center was 1-6 hours. 8. Most patients discharged from the emergency center in good condition(76.5%).
