1.Clinical Application of a New Balloon Dissector.

A new balloon dissector, a modification of a Foley catheter, was devised and it can be used either as a dissector or a tissue expander. Since most operating time was spent in balloon manipulation (inflation/deflation), the duct for saline injection was made to be wider than the Foley catheter. As a result, the balloon could be inflated faster than before. In order to reduce the scar at the donor site, harvest of the sural nerve using endoscopic technique is currently applied, but utilization of this method is technically difficult and requires a long operating time. For these reasons, new our method of using a balloon dissector was devised. The balloon dissector can also be used for immediate intraoperative tissue expansion for the reconstruction of small skin defects without distortion. We have found that the advantages of using the new balloon dissector include a reduction in operating time, preservation of the perforating vessels, and primary closure with less tension. In addition, this simple and inexpensive instrument is cost-beneficial to patients.

2.Relationship between stress amounts and health habits about nurses in one hospital.

BACKGROUND: Stress is found to have influence on physical and mental disorders, and also to healthy life habits related to physical health mental stress. Job holders are groups that have suffered from chronic stressful condition. Consider medical people of job holders to received exess stress. So, we tested this investigation to know the relationship between health habits and stress amounts about nurse. METHODS: This study was performed on nurses who worked on one hosptal in Pusan Metropolitan city for one months(from April, 1, to April, 30, 2000). We used 128 results. Health habits were investigated about 5 articles out of "Breslow's 7 health habits" and stress amounts were measured by Korean translated BEPSI(Brief Encounter Psychosocial Instrument) and Bae Jong Myun's 30 items SRRQ(Social Readjustment Rating Questionnaire). RESULTS: There were differences in amounts of stress of subjects according to marital state(P<0.05), job department(P<0.05), BMI(Body Mass Index)(P<0.05), sleeping time(P<0.05). CONCLUSION: This investigation has showed that having excess stress in nurses. Body Mass Index, sleeping time appeared to be health habits related to amounts of stress.

3.A Clinical and Serologic Study of 21 Cases of Tsutsugamushi Disease Confirmed by Serologic test.

Tsutsugamushi disease is an acute febrile disease caused by Rickettsia tsutsugamushi, and which has been reported with increasing frequency thorough the nation since 1986. We experienced 21 cases of Tsutsugamushi Disease diagnosed with serologic test occurring in Taegu city and Kyungpook province during October-November, 1989. The results of survey are as follow. 1) Of 21 cases, 12 (57%) were males and 9 (43%) were females, and the peak incidence was the 4th decade. 2) The outbreak was in October to November and the peak incidence was in October. 3) The most frequent symptoms were fever and chill (100%), myalgia (95%), headache (90%), Eschar and rash were observed in 18 patients (86%) and the eschar was detected in all over the body, especially thorax (33%) and lower extremity (22%). 4) Laboratory features were SGOT elevation (83%), SGPT elevation (61%), LDH elevation (67%), leukocytosis (38%). 5) Indirect immunofluorescent antibody test was done in 18 patients and the antibody titer was above 1:320 in all patients. 6) The chloramphenicol, tetracycline or doxycycline regimens were very effective and mean duration of defervescence from initiation of therapy was 1.3 days. 7) The complication such as meningitis or shock, was not seen.

4.Carcinosarcoma and Multiple Early Gastric Carcinomas of the Stomach.

We report a rare case of carcinosarcoma arising in the stomach. The tumor is presented in the posterior wall of the gastric lower body and antrum of a 56-year-old man. Grossly the tumor has polypoid appearance with diffuse surface ulceration and measures 5.5 cm in diameter. The tumor is accompanied with three separate well differentiated early gastric adenocarcinomas (two EGC type IIc & IIa). Microscopically, the tumor consists of moderately well differentiated adenocarcinoma in the periphery, and lobulated sarcomatous areas in the center, which shows chondroid differentiation. Transitional areas between adenocarcinoma and chondrosarcoma are evident. Immunohistochemical studies show positivity for cytokeratin and carcinoembryonic antigen in the epithelial component, and for vimentin and S-100 in the sarcomatous component. The transitional areas are positive in carcinoembryonic antigen, vimentin, S-100, and cytokeratin. The tumor extended to the subserosa and showed metastasis of only adenocarcinomatous component in six out of 47 dissected perigastric lymph nodes.

5.Study of Cancer Cases by Industry in Kwangju-Chonnam Area: Based on Industrial Medical Insurance Record.

In order to investigate the occurrence characteristics of cancer in terms of industry in Kwangju-Chonnam area, medical utilization records of industrial medical insurance corporations during the period of 1987 to 1988 were reviewed for the identification of neoplastic disease. The cases obtained from the medical records were followed up for the verification and to get additional information. Standardized incidence data were compared by occupational characteristics. Multiple logistic regression analysis was applied to analyze the difference of incidence or distribution of cancer as a whole or of some selected cancer. Total cases of cancer identified were 242 during the study period. Annual incidence rate was calculated as 123.1 per 100,000 person. The frequent types of cancer were cancers of stomach, liver, lung, colon and rectum, bladder and lymphoma in descending order. Employees of mine and other sand handling industries showed significantly higher risks for cancer of stomach and cancer as a whole. Employees of the transportation industry showed the higher risk for cancer of liver. Workers in small-sized industry (< or = 100) had a higher risk for cancer than who in large-sized industry (>100). These findings suggested the effect of occupational environmental exposure to cancer development.

6.Ganglion Impar Block With Botulinum Toxin Type A for Chronic Perineal Pain -A Case Report-.

Chronic perineal pain is an often encountered problem, which produces a great degree of functional impairment and frustration to the patient and a challenge to the treating physician. The reason for this problem is that the region contains diverse anatomic structures with mixed somatic, visceral and autonomic innervations affecting bladder and bowel control and sexual function. A blockade of nociceptive and sympathetic supply to the perineal region, supplied through the ganglion impar has been shown to benefit patients with chronic perineal pain. Several options to this block have been described that chemical neurolysis, radiofrequency ablation etc. Although the analgesic effect of Botulinum toxin type A (BoNT-A) has long been considered secondary to its action for muscle relaxation, BoNT-A also affects the release of the neurotransmitters that are involved in pain perception. We describe a patient who was successfully given ganglion impar block with BoNT-A.

7.The Diagnostic Value of Segmental Nerve Conduction Study in Diabetics.

Nerve conduction study has been an important part of electrodiagnosis and has been utilized as a clinical diagnostic technique in diabetic neuropathy. But conduction study of the more proximal segment has been a few reports. This study demonstrated segmental method for obtaining orthodromic motor nerve conduction and mixed nerve conduction, and antidromic distal sensory nerve conduction in diabetic group and normal group. The purpose of the study was to determine diagnostic value of segmental nerve conduction in diabetic neuropathy. The results were as follows: 1. Significant difference at P<0.01 between diabetic group and normal group; in median motor terminal latency, median motor nerve conduction velocity at axilla-elbow segment and elbow-wrist segment, median sensory nerve conduction velocity at wrist-elbow segment, ulnar motor and sensory conduction velocity at wrist-elbow segment, posterior tibial motor nerve conduction velocity at popliteal fossa-ankle segment, peroneal motor nerve conduction velocity at fibula head-ankle segment. 2. No significant difference at P>0.05 between diabetic group and normal group; in ulnar motor terminal letency, ulnar sensory nerve conduction velocity at axilla-Erb's point segment and finger-wrist segment, peroneal motor terminal latency and peroneal motor nerve conduction velocity at popliteal fossa-fibula head segment, posterior tibial motor terminal latency, sural sensory nerve conduction velocity. 3. The nerve conduction of proximal segment is more statistically significant than that of distal segment.

9.Improvement of Pregnancy Rate by the Selection of Early Cleavage Embryos to 2-cell Stage in Human IVF.

OBJECTIVE: Evaluation of embryos using early cleavage to 2-cell stage has been proposed, but a critical time-point for selecting embryos is unclear. The aim of the present study is to provide a guideline including critical time-point in the selection of early cleaving embryo for the reduction of multiple pregnancies as well as the increase of pregnancy rate in human IVF. METHODS: This prospective study was performed in 116 cycles from 85 patients who underwent conventional IVF or ICSI at the infertility clinic of Good Moonhwa Hospital from January 2002 to December 2003. Early cleavage (EC) of embryos to 2-cell stage was assessed at 25 h and 27 h postinsemination/microinjection. Embryos that had early cleaved at each time point were designated as EC-1 and EC-2, respectively, while others were designated as non-early cleavage (NEC). RESULTS: At least one early cleavage embryo was observed in 54 (46.6%) for the EC-1 and 84 (72.4%) for the EC-2 of the 116 cycles assessed. Clinical pregnancy rates (PR) were significantly higher in the EC-1 group (66.7%) compared to the EC-2 group (53.6%) or the NEC group (31.2%) (p<0.05). Significant improvement of the pregnancy rate was found when at least two or more embryos were early cleaved at 25 h postinsemination or when the proportion of early cleavage embryo at 25 h postinsemination was higher than 20% (p<0.05). CONCLUSIONS: The critical time-point for the selection of early cleavage embryos with high implantation potential is more effective in 25 h postinsemination/microinjection compared to 27 h. The proportion as well as number of early cleavage embryos is also an important factor for the prediction of pregnancy outcome and the chance of multiple pregnancies. These results demonstrated that the evaluation of early cleavage embryos to 2-cell stage is an easy, simple, and objective method for the selection of good quality embryos suitable for embryo transfer.

10.Apoptosis and Cell Cycle Arrest with EGF, TGF- a and TGF- 8 in Cervical Cancer Cell Lines .

BACKGROUND: EGF and TGF-a are ligands for the EGF-receptor and act as mitogens for a variety of tissues. TGF-a, in particular, has been implicated as an autocrine growth factor for several cancer cell lines. TGF-B exerts an inhibitory effect on the growth of most epithelial cell types, and the loss of responsiveness to this growth inhibition has been implicated in the development of a variety of human cancers. In the present study, we evaluate whether EGF, TGF-a and TGF-B modulate apoptosis and cell cycle progression in cervical cancer cell lines. MATERIALS & METHODS: The effect of EGF, TGF-a and TGF-B on apoptosis and cell cycle such as CaSki and HeLa cell lines was analysed by flow cytometry RESULTS: 1. TGF-B did not induce apoptosis in CaSki and HeLa cell lines. 2. TGF-B as well as EGF, TGF-a, did not affect the process of apoptosis significantly. 3. The time to occur apoptosis was different between CaSki and HeLa cells treated by growth factots. 4. G1 phase was the checkpoint in CaSki and HeLa cells treated with TGF-B. CONCLUSION: These results suggest that TGF-B as well as EGF, TGF-a does not induce apoptosis and cell growth inhibition.
