1.Prediction of Suitable Site to Measure Abdominal Skin Fold Thickness and Correlation among Obesity Indicators in Patients with Type 2 Diabetes Mellitus

Purpose:
The purpose of this study was to predict measuring site suited for abdominal skin fold thickness (ASFT) by measuring the distribution of abdominal subcutaneous fat thickness (AScFT) and ascertain the correlations among obesity indicators.
Methods:
The size of analysis materials was 124 secondary data measured by ultrasonic device, bioelectrical impedance analyzer and caliper. Data were analyzed using t-test, and Pearson’s correlation.
Results:
The average of AScFT was 10.63± 6.79mm with its range 1.39-36.16 mm, and AScFT of female and of central parts were thicker than those of male and outer parts in the abdomen. The average of ASFT was 29.26± 12.59 mm. Site 5 on Figure 1 was most similar to the average of AScFT in both sexes. Body mass index (BMI) and waist hip ratio (WHR) were 23.65± 3.98 and 0.88± 0.05 respectively. The body weight, BMI, WHR, visceral fat, ASFT vs AScFT revealed in significant correlation (r= .29, r= .55, r= .39, r= .33. r= .29).
Conclusion
BMI and WHR seem more useful than other obesity indicators, when obesity control is necessary for Type 2 diabetes patients. Site 5 on Figure 1 is most suitable site to measure ASFT.

5.Increased Serum Level of Inhibin in Oligo-amenorrheic Women with Polycystic Ovaries.

Normal and abnormal follicular growth and steroidogenesis depend on gonadotropins as well as intraovarian peptides, which may mediate or potentiate gonadotropin action. Inhibin also affect follicular development and steroidogenesis and may play a role in dominant follicle selection and follicular atresia. Therefore, we studied the differences of serum inhibin, gonadotropin and androgen levels in the women with only the ultrasound findings and no disorder, and polycystic ovary (PCO) with ovulatory disturbance. We prospectively analysed forty-three women with PCO. The diagnosis of PCO was based on typical appearance of the ovaries on TVS. Twelve women with regular menstrual cycle and normal ovarian morphology were selected as control. Basal levels of inhibin, luteinizing hormone(LH), follicle stimulating hormone(FSH), estradiol(E2), testosterone(T), androstenedione(ADD), dehydroepiandrosterone-sulfate(DS), prolactin and TSH in serum were determined. There were significant differences in basal LH levels and LH/FSH ratio between the control and the women with PCO. The basal levels of inhibin and E2 in the oligo-amenorrheic PCO (N=34) were significantly higher than those in the control. There was higher negative correlation between the inhibin and T levels in the oligo-amenorrheic PCO, but, not in the regular cycling PCO. Also, there was higher positive correlation between the LH and T levels in the oligo-amenorrheic PCO, but not in the regular cycling PCO. These data presume that the initial event of PCO is elevated pituitary LH secretion. Elevated levels of LH may down-regulate LH receptors on granulosa cells and also cause hypertrophy of the thecal layer. High level of androgen secreted by the hypertrophied thermal layer may stimulate inhibin secretion from granulosa cells and can be converted to estrogen by extraovarian tissues and could serve to augment pituitary sensitivity to GnRH with a resultant secretion of more LH follicular development and dominant follicle selection resulted in ovulatory disturbance.

7.Impediment in Activity of Daily Living and Social Support for Rural Elderly Farmers Undergoing Nerve Block due to Low Back Pain

PURPOSE: This study is to investigate the low back pain, social support, impediment in daily living activities and to identify factors affecting impediment in elderly farmer' daily living activities. METHODS: The participants were 128 elderly farmers who had received nerve block. Data were collected using a structured questionnaire from February to March, 2018. They were analyzed using t-test, ANOVA, pearson's correlation coefficient, and linear multiple regression. RESULTS: The score of low back pain was 6.27±1.69 (10 points), that of social support 2.92±0.76 (1~5 points), and that of impediment in activity of daily living 2.01±0.82 (0~5 points). Factors affecting impediment in activity of daily living were found to include age (p=.017), daily hours of farm work (p<.001), fear for the nerve block (p<.001), low back pain (p<.001), and social support (p<.001); the explanatory power of these variables was 58.8%. CONCLUSION: This study has found the controllable factors affecting impediment in activity of daily living among the rural elderly engaging in farm work include low back pain, social support, and daily farming hours. Therefore, to reduce impediment in activity of daily living among them, it is necessary to develop nursing interventions that can improve impediment in activity of daily living through reduction of daily farming hours using local resources. It is also desirable to improve their health status by reducing low back pain, and develop and apply social supports with health education programs that fit the local resources and the needs of the rural elderly.

8.Lesional location of intractable hiccups in acute pure lateral medullary infarction

Background & Objective: Hiccups is a disabling condition of lateral medullary infarction (LMI).
Unlike other symptoms of LMI, the anatomical lesions of hiccups are not well known. Few studies
have evaluated the relationship between the lesional location of LMI and hiccups. We performed this
study to correlate hiccups and magnetic resonance imaging (MRI)-based lesional location in pure
LMI. Methods: Between January 1997 and February 2013, we identified 24 patients with pure LMI
who presented with hiccups in addition to typical lateral medullary syndrome. Sixty six pure LMI
patients without hiccups were included as a control group. Clinical and radiologic findings were
compared between the two groups. MRI-identified lesions were classified rostrocaudally as rostral,
middle and caudal, and horizontally as typical, ventral, large, lateral and dorsal. Results: The pure
LMI patients with hiccups had significantly more frequent aspiration pneumonia (P = 0.001) and
longer hospital stay (P = 0.03). The patients with hiccups significantly more often had dorsal rather
than ventral lesion at horizontal levels (P = 0.012). But, there were no rostro-caudal differences at
vertical levels (P = 0.162).
Conclusions: We suggest that pure LMI associated with hiccups often locates in the dorsal medulla
at horizontal correlation. This MRI-based comparative study has advanced the understanding of the
neural substrate for hiccups in LMI, and indicates that hiccups become predictable when specific
lesional locations in the lateral medulla are considered.
9.Difficulties and Coping Experienced by Advanced Practice Nurses in Home Health Nursing Field

Purpose:
This study aimed to explore the difficulties and coping that advanced practice nurses (APNs) experience in home health nursing field.
Methods:
The participants were 12 APNs who had experience in home health nursing for over 5 years. Data were collected through two focus group interviews between August and September, 2017. The results were analyzed using a qualitative content analysis.
Results:
Out of 10 sub-categories and 29 codes, 4 categories emerged: “crushed by the responsibilities as the only healthcare professional”, “worn out by family who interfere with work”, “anxiety about my safety being threatened” and “using self-learned know-hows”.
Conclusion
APNs face difficulties due to the environmental characteristics of the homes, the characteristics of chronically critically ill (CCI) patients and their families, and legal or systematical limitations when dealing with problems based on their personal competence. In order to ensure that professional nursing care is more readily provided to CCI patients through home health nursing services in local communities, it is imperative that support systems at the organizational and institutional levels be established in a systematic way to resolve the issues faced by APNs.
