3.A Clinical Study on the Diaphyseal Fractures Treated with Compression Plate

Most orthopedic surgeons were aimed at the rigid internal fixation on the surgical treatment of the fracture. And the compression plate was developed and regarded as an excellent rigid internal fixation device by its advantages of accurate anatomical reduction, early joint mobilization and shortening of the duration of the fracture healing, which are chief aim of A.S.I.F. method. During the period of January 1970 to December 1979, 67 cases of diaphyseal fractures of the long bones were treated with compression plate according to A.S.I.F. method at the Department of Orthopedic Surgery, College of Medicine, Seoul National University and results were obtained as follows: 1. The age of patient was ranged from 15 to 77, and the highest incidence was in the 3rd decade male group (31.4%). 2. Old fracture cases were slightly more than fresh cases, and the fresh femur fracture was the commonest. 3. The periods of the postop immobilization of fresh fractures were 3.7 weeks in humerun fracture, 6.3 weeks in forearm fractures, 8.7 weeks in femur fracture and 7.0 week in tibia fracture, and in old fractures about 4 weeks was longer than fresh fractures. 4. The bony union was accomplished in all cases between 8 week and 24 week. 5. The complication were 4 superficial infections, 4 deep infections, 1 delayed union and 9 joint stiffnesses. 6. Compression plate fixation is considered as an excellent method for the treatment of the fractures of the long bones.

5.A case of successfully resected retroperitoneal hemangiopericytoma.

Hemangiopericytoma is a rare mesenchymal neoplasm, accounting for about 1% of vascular tumor. Hemangiopericytoma is known to be derived from the vascular pericyte and occurs most commonly lower extremities, pelvis and retroperitoneum. Surgical radical excision is the treatment of choice. Because of malignant features of hemangiopericytoma, long term and close follow-up is important. We report a case of successfully resected retroperitoneal hemangiopericytoma.

6.Effects of Insulin-like Growth Factor in Peritoneal Fluid of Patients with Endometriosis on the Proliferation of Endometrial Stromal Cells.

The purposes of this study were to evaluate the effects of insulin-like growth factor (IGF)s in peritoneal fluid (PF) from patients with and without endometriosis on the proliferation of endometrial stromal cells and to investigate the effects of type I IGF receptor antibody on the response of endometrial stromal cells to PF from patients with endometriosis. IGFs in PF from patients with endometriosis (n=14) and without endometriosis (n=10) were measured by immunoradiometric assay and PF samples were divided into low IGF-I PF group (less than 85 ng/ml) and high IGF-I PF group (more than 85 ng/ml). Endometrial stromal cells from patients without endometriosis were cultured in serum free media in the presence or absence of 1% PF and thymidine incorporation test were used to evaluate the proliferation of endometrial stromal cells. Also cultures were incubated with type I IGF receptor monoclonal antibody (alpha IR3) before adding PF. PF from patients with endometriosis and without endometriosis increased thymidine incorporation in endometrial stromal cells. In patients with endometriosis, high IGF-I PF group had high IGF-II levels and resulted in higher thymidine incorporation than low IGF-I PE and low IGF-I PF group was noted in patients without endometriosis. There was not a significant correlation between increase in thymidine incorporation and IGF-I levels in PF from patients without endometriosis but in PF from patients with endometriosis. Preincubation with alphaIR3 significantly inhibited the mitogenic response of endometrial stromal cells to PF. Our data indicate that IGF-I in PF may be involved in the growth of ectopic endometrium in patients with endometriosis.

7.The Effect of Sex Steroid Hormone on the Expression of Insulin-Like Growth Factor Binding Preteins mRNA in the Explant Cultured from Human Uterine Myoma and Adjacent Normal Myometrium.

BACKGROUND: Sex steroid hormones are believed to play an important role in the genesis and growth of uterine myoma. Several studies suggest a possible role of insulin-like growth factor(IGF) as a mediator of estradiol in uterine myama. We have recently demonstrated that some IGF binding proteins(IGFRPs) messenger ribonucleic acid(mRNA) expressions in myoma are dependent on the in vivo esttogen status. The purposes of this study are to evaluate the in vitro effects of sex steroid hormones including estrogen on the IGFBPs gene expression in tissues from uterine myoma and adjacent normal myometrium. METHODS: Tissues from myoma and adjacent normal myometrium of patients with uterine myoma during early proliferative phase of menstrual cycle were cultured in the absence(control) and presence of 17b-estradiol(10M/L) or/and progesterone(10M/L) for 3 days. The IGFBPs mRNA expressions in these explants were analyzed by Nothern blot using specific human complementary deoxyribonucleic acid(cDNA) probes. RESULTS: The addition of 17b-estradiol, progesterone alone and in combination to conditioned media of explants from myoma and adjacent normal myornetrium did not result in any changes in the expression of IGFBP-2, IGFBP-4, IGFBP-5, and IGFBP-6 mRNA. With progesterone addtion, lGFBP-3 rnRNA expression was significantly reduced in myoma explant but not in adjacent ncemal myometrium explant. There was no significant change in the IGFBP-3 mRNA expression with 17b-estradiol and with the combination of both 17b-estradiol and progesterone. CONCLUSION: 17b-estradiol does not affect IGFBPs gene expression in the myoma and adjacent normal myometrium explant regardless of the presence of progesterone in vitro. However progesterone alone induces a decrease in IGFBP-3 synthesis in myoma explant.

8.The Effect of Finasteride, Tamsulosin and Doxazosin Therapy on Sexual Function in Patients with Benign Prostatic Hyperplasia.

Purpose: The aim of this study was to evaluate the effect on sexual function after finasteride, tamsulosin, doxazosin single or combination therapy according the sexual function state before these therapies. Materials and Methods: This study included 192 men with benign prostatic hyperplasia (BPH) who had neither diabetes mellitus nor hypertension. All patients were classified into 2 groups according to their erectile dysfunction (ED) severity based on IIEF-5 before treatment; the above mild ED group (severe + moderate + mild to moderate) and mild or no ED group. The patients were assessed by IIEF inventory at the baseline and 1, 3 and 6 months after finasteride, tamsulosin, doxazosin single or combination therapies, respectively. Results: In the above mild erectile dysfunction group the mean IIEF score was significantly decreased at 3 and 6 months with finasteride single therapy, at 6 months with finasteride-tamsulosin combination therapy and at 3 and 6 months with finasteride-doxazosin combination therapy (p<0.05). In the mild or no erectile dysfunction group the mean IIEF score did not change significantly with any of the medications. Conclusions: Sexual function tended to decrease in the above mild ED group with finasteride single or combination therapy. Therefore, the patients in this group should be treated carefully at the beginning of these therapies.

10.Three Cases of Neutrophilic Eccrine Hidradenitis.

Neutrophilic eccrine hidradenitis(NEH) was originally described in 1982 by Harrist et al. in a patient with myelogenous leukemia receiving chemotherapy. Clinically NEH represents various cutaneous manifestations with or without tenderness and pruritus. Histologic examination demonstrates a neutrophilic infiltrate within and around the eccrine gland and degeneration of the eccrine gland structures. Although the pathogenesis and possible cause of NEH remain unknown, it is probably an unusual cutaneous reaction to chemotherapeutic agents. A few cases of infection associated eccrine hidradenitis are found in the literature. We report three cases of neutrophilic eccrine hidradenitis. Two cases were associated with hematologic malignancy. The third case was associated with an infection of Vibrio vulnificus.
