1.Pulmonary Function in Adults with Tuberculous Kyphosis of the Spine

Kyphosis and scoliosis develops from various causes, Tuberculosis is the main cause of kyphosis and spinal deformity in Korea. Such spinal deformities are frequently accompanied by cardiac and respiratory changes. In 1969, Westgate and Moe reported that, when a kyphoscoliotic patient dies, he dies because of cardio-respiratory failure and not because of skeletal abnormality, and, in 1968, Nilsonne and Lundgren reported that the mortality in patients with severe spinal deformity was twice as high as in normal persons, and that heart or lung disease was the main cause of death. One must consider the cardiopulmonary status when treating patients with severe spinal deformities. While there are numerous reports on pulmonary dysfunction due to scoliosis or kyphoscoliosis, studies on pulmonary function of patients with tuberculous kyphosis are surprisingly few. The authors investigated the correlation of the degrees of curvature and pulmonary function on 17 cases of adults with tuberculous kyphosis and compared the results with those of the children reported in 1972 by Moon and Lee. The results of this study are as follows: 1. The over-all mean degree of kyphosis was 92.6 (Salters angle). Vital capacity was 63.1% and maximum breathing capacity 61.7% of the predicted normal value. 2. Vital capacity showed a negative correlation with the degree of kyphotic curvature; i. e. the greater the spinal curvature, the less the vital capacity and vice versa, R=0.45, P=<0.1, but there was no correlation between maximum breathing capacity and the degree of curvature. 3. Vital capacity showed a relatively significant positive correlation with maximum breathing capacity as kyphosis increased. 4. The over-all mean tidal volume increased 9.3% of the normal predicted value. 5. When comparing with the pulmonary function of the children with same level and same degree of kyphosis, the adults had significant decrease in vital capacity and maximum breathing capacity, but had increased tidal volume. Such disparity was more prominent in the thoracic spine than in the lumbar spine and when the kyphotic curvature was greater than when less. 6. Timed vital capacity was within normal limits in spite of kyphotic changes of the spine.

6.Full mouth rehabilitation of destroyed dentition with rotational path removable partial denture: a case report.

BACKGROUND: Though implant dentistry is very successful and predictable in treatment of patients with destroyed dentition, there are some cases with limitations to implant therapy. In these cases, alternative treatment modality should be considered. CASE DESCRIPTION: A patient with destroyed dentition was rehabilitated with a lateral rotational path removable partial denture. According to the diagnosis, we determined to raise vertical dimension for esthetic and functional restoration. The final restoration was performed after four months of provisional period. CLINICAL IMPLICATION: The edentulous patients with compromised esthetics and functions can be successfully treated with a rotational path removable partial denture through adequate treatment planning and precise laboratory procedure.

7.A Case of Cutaneous Metastasis from Pancreatic Adenocarcinoma.

Cutaneous metastases from internal carcinomas are relatively rare, especially from pancreatic carcinoma. Pancreatic carcinomas are usually adenocarcinomas which arise in the head of the gland, and are known to rapidly metastasize to the lymphatic system by permeation and embolization. We report a case of cutaneous metastasis from pancreatic adenocarcinoma. in 74-year-old male patient who have two pea sized, slight erythematous nodules on the lower abdomen and posterior side of the neck.

10.The Effect of Topical Indomethacin and Topical Corticosteroid on UVB Induced Erythema.

BACKGROUND: Indomethacin is a potent inhibitor of prostaglandins biosysnthesis. Sunburn erythema is mainly mediated by prostaglandins. OBJECTIVE: Our purpose was to compare objectively the effectiveness of topical indomethacin with topical corticosteroid on the suppression of UVB erythema METHODS: Sixteen male medical students who had not exposed their back skin during the last year were included in this study. According to the individual´s MED, 1,2, and 3 MED of UVB were irradiated on each back in triplicate lines. Immediately after UVB irradiation, 2.5% indomethacin solution and 0.25% desoximethasone were applied to each row with one row left for control. 24 hours after the initial application the intensity of each erythema was measured by the naked eye and by colorimeter. RESULTS: The suppressive effectiveness of 2.5% indomethacin solution on UVB induced erythema was superior to that of 0.25% desoximethasone. The L* and a* value of colorimeter were significantly correlated to the differencies of UVB induced erythema among the experimental and control groups is a useful and rapid method to evaluate the UVB induced erythema, and can give a numerical expression to eye perception. CONCLUSION: Our data confirm that topical indomethacin has a stronger suggestive effect on UVB erythema than that of topical corticosteroid.. We suggest that the suppressive effect of indomethacin is mainly due to the inhibition of prostaglandins biosynthesis. The colorimeter CR-200(MINOLTA) is well correlated with the naked eye score and is a useful instrument for objective measurement of the degree of erythema.
