3.The Effect of Topical Indomethacin and Topical Corticosteroid on UVB Induced Erythema.

BACKGROUND: Indomethacin is a potent inhibitor of prostaglandins biosysnthesis. Sunburn erythema is mainly mediated by prostaglandins. OBJECTIVE: Our purpose was to compare objectively the effectiveness of topical indomethacin with topical corticosteroid on the suppression of UVB erythema METHODS: Sixteen male medical students who had not exposed their back skin during the last year were included in this study. According to the individual´s MED, 1,2, and 3 MED of UVB were irradiated on each back in triplicate lines. Immediately after UVB irradiation, 2.5% indomethacin solution and 0.25% desoximethasone were applied to each row with one row left for control. 24 hours after the initial application the intensity of each erythema was measured by the naked eye and by colorimeter. RESULTS: The suppressive effectiveness of 2.5% indomethacin solution on UVB induced erythema was superior to that of 0.25% desoximethasone. The L* and a* value of colorimeter were significantly correlated to the differencies of UVB induced erythema among the experimental and control groups is a useful and rapid method to evaluate the UVB induced erythema, and can give a numerical expression to eye perception. CONCLUSION: Our data confirm that topical indomethacin has a stronger suggestive effect on UVB erythema than that of topical corticosteroid.. We suggest that the suppressive effect of indomethacin is mainly due to the inhibition of prostaglandins biosynthesis. The colorimeter CR-200(MINOLTA) is well correlated with the naked eye score and is a useful instrument for objective measurement of the degree of erythema.

4.A case of zosteriform cutaneous metastatic adenocarcinoma of cholangiocarcinoma.

We report a case of zosteriform cutaneous metastasis from choangiocarcinomasin a 56-year-old male patient who has asymptomatic and painless erythematous papules and scaly patches over the anterior side of his right upper abdomen along the back, Tendernatome level. Histopathologic examina.tions of the cutaneous lesion reveaed a tumor cell showing glandular structure and multifocal tumor emboli within dilated lymphalics. and a few signetring cells in the dermis. We found that the retrograde studies revealed cholangiocarcinom. after confirmation of zosteriform cutaneous metastasis. He died eight months after the appearance of zosteriform cutaneous metastasis.

7.The Effects of SCH-T2 Seaweed Extract.

BACKGROUND: Melanocytes synthesize melanin pigment by the action of specific enzyme tyrosinase. Melanogenesis inhibitors such as ascorbic acid, kojic acid, arbutin, hydroquinone have been developed for use in cosmetic preparations for the skin hyperpigmentation,but they are still unsatisfactory to efficacy and tolerance. OBJECTIVES: In this study, the inhibitory effects of seaweed extract(SCH-T2)on tyrosinase activity and melanogenesis were investigated with B-16 melanoma cell line. METHODS: The patients were randomly assigned to one of three groups;control, diazepam, and ketorolac plus diazepam groups. Pain intensity was assessed 5 times at every ten minutes from the beginning of the peeling using visual analog scale(VAS). RESULTS: The seaweed was added with 21 organic solvents and extracted during 12 hours at 20-To evaluate the inhibitory effects of SCH-T2 on tyrosinase activity following to extraction time and temperature, this study was done at 4, 8, 12, 24, 72 hours and 4, 10, 20, 30, 40℃. Also, the effects of whitening agents (kojic acid, arbutin, licorice extracts,SCH-T2)on tyrosinase activity were compared by measuring the IC50,the concentration of the compound at which half of the original tyrosinase activity is inhibited. RESULTS: 1.The inhibitory effects of SCH-T2 on tyrosinase activity were high at 60-80% ethanol as an extraction organic solvent and showed increase in proportion to concentration following to extract concentration, but showed little differences following to extraction time and temperature. 2.SCH-T2 has relatively stronger inhibitory effects on tyrosinase activity than arbutin and licorice extracts. 3. The inhibitory effects of SCH-T2 on melanogenesis following to extract concentration were greatly increased at more than 20㎍/ml. CONCLUSION: This study showed that SCH-T2 seaweed extract has strong inhibitory effect on tyrosinase activity and melanogonesis. So, the results of this study suggest that SCH-T2 seaweed extract can be used as a new whitening agent.

Cutaneous metastases from internal carcinomas are relatively rare, especially from pancreatic carcinoma. Pancreatic carcinomas are usually adenocarcinomas which arise in the head of the gland, and are known to rapidly metastasize to the lymphatic system by permeation and embolization. We report a case of cutaneous metastasis from pancreatic adenocarcinoma. in 74-year-old male patient who have two pea sized, slight erythematous nodules on the lower abdomen and posterior side of the neck.

9.Decreased Expression of Type 5 17beta-Hydroxysteroid Dehydrogenase (AKR1C3) Protein Identified in Human Diabetic Skin Tissue.

BACKGROUND: Diabetes is characterized by chronic hyperglycemia, and hyperglycemia can increase reactive oxygen species (ROS) production from the mitochondrial electron transport chain. The formation of ROS in cells induces oxidative stress and activates oxidative damage-inducing genes. There is no research on the protein levels of oxidative damage-related genes AKR1C3 in human diabetic skin. We explored the expression of AKR1C3 in diabetic skin compared with normal skin tissue. OBJECTIVE: To compare the expression of AKR1C3 in normal skin versus diabetic skin. METHODS: AKR1C3 expression was evaluated by western blotting in 6 diabetic skin tissue samples and 6 normal skin samples. Immunohistochemical staining was carried out to analyze AKR1C3 expression in the 6 diabetic skin tissue samples (July 2009 to December 2011; Department of Plastic and Reconstructive Surgery at Soonchunhyang University Seoul Hospital, Seoul, Korea). RESULTS: The western blotting showed a significant reduction in AKR1C3 protein expression in diabetic skin tissue compared to normal tissue. Immunohistochemical examination of AKR1C3 showed that it was weakly expressed in all diabetic skin samples. CONCLUSION: We believe that AKR1C3 is related to diabetic skin in altered metabolic states which elevate ROS production.

10.A Case of Generalized Lichen Nitidus.

Lichen nitidus(LN) is an unaommon chronic eruption of numerou tiny, discrete, usually flesh colored, shiny papules most cornmonly located on the penis, arms, for arms, and abdomen. It was described by Pinkus, first in 1901. The distribution of LN is most often locaized, but in some cases it may become generalized. A 10-year-old healthy boy was seen by our department because of many flesh colored, discrete, shiny papules on the whole body. The papules are skin colored, pinhcad sized, round, and flat-topped. The lesions appeared first at the age of 9 on the forehead and then neased in the number and extended downward to the neck, trunk, abdornen and both extremities. This case was successfully treated with topical corticosteroid.
