Kimura's disease is a benign, uncommon, chronic inflammatory condition that usually presents with painless subcutaneous nodules or plaques. Head and neck are the most frequently involved sites in Kimura's disease. Mandible is the most commonly involved, followed by neck, cheek, scalp and forehead. Other possible sites are oral cavity, inguinal area and extremities, but there have been no reports involving the nose, especially the one that looks like a rhinophyma. We describe a case of Kimura's disease presenting like a rhinophyma.

In our series with old electrical burn around the the wrist, there are several characteristics: for example, bad scarring in palm and volar aspect of wrist and distal forearm, large multiple defects of the flexor tendons, low median and ulnar nerve palsy, and occasionally insufficient blood supply. Between 1992 and 1995, we treated 8 cases of 7 patients with staged reconstruction. 4 patients with bilateral involvements had below-elbow amputee of contralateral upper extremity. The common approachs in each were soft tissue coverage, staged tendon reconstructiuon, and opponensplasty. The length of time between the injury and tendon reconstruction was 9 months on an average. Soft tissue coverage consisted of: 1. free tissue transfer (n=3), 2. abdominal (n=3) or groin (n=2). Total numbers of flexor tendon reconstruction were 26. Among them, staged reconstruction using silicone prosthesis were done in 18 tendons and primary tendon graft following tenolysis were done in 8 tendons. The time between first and second stage tendon reconstruction was 4.13 months. In all cases, opponen-splasty using extensor indicis proprius (n=4), extensor pollicis longus (n=2), and extensor digiti mini- mi (n=l), were performed. Good functional improvement of the hand were obtained in all cases. Patient s satisfaction were much better than the degree of functional improvement of the hand.

PURPOSE: This study was done to investigate the degree of self-care, family support and depression, and relationship among these variables for elder with diabetes mellitus. METHOD: Participants were 202 diabetic patients, 65 years or over, living in Seoul, Korea. Data were collected using the self-care tool for diabetic patients by Kim (1996), the family support tool for diabetic patients by Park (1984), and Korea\' s BDI scale by Lee (1995). RESULTS: Of the patients, 43.1% showed HbA1c levels than higher 7%. The highest mean score was for self-care for medication compliance, and the lowest for blood glucose testing compliance. Factors affecting self-care were employment, education, HbA1c level, diabetic self-care education and complications. Factors affecting family support were living with family, diabetic self-care education, hospitalization and complications. Factors affecting depression were gender, living with family and complications. All of these factors were significant. Patients experiencing depression were 16.8% of patients. There was a significant positive correlation between self-care and family support, and significant negative correlations between self-care and depression, and family support and depression. CONCLUSION: For more effective management of diabetes mellitus in elders, improvement in self-care compliance, and family support are needed

This study was conducted to investigate distribution of the house dust mites in the bedroom of allergic rhinitis patients in Pusan. From July to December 1995, 51 dust samples were drawn from the bedroom of the patients by using electronic vaccum cleaner. House dust mites were isolated from 50mg dust by applying the modified wet sieving method. The results were as follows: 1 Out of a total 49 dust samples(96.1%) were isolated house mites. 2 Total 977 mites were collected and identified the most prevalent house mites was Pyroglypidae mite with 92.2%. Among them D.pteronyssinus was predominant species. 3 In 1gm of the house dust, 100-499 mites were found in 17 samples(33.3%), more then 500 mites in 12 samples (23.6%). 4 The density of Pyoglypidae mite was close associated with the month. 5 The density of house dust mite was not affected by the type of house, region and the relative humidity of the bed room

Subperiosteal Cortical Defect is a symptomless rarefaction of cortical bone which begins most often on the external surface of the medial and posterior cortical walls of the lower end of the femur in children between the ages of 3 and 6 years. The name of Subperiosteal Cortical Defect was first used by Aegerter who differentiated it from non-osteogenic fibroma. The authors found out 5 positive cases of incidental findings among 62 cases of both knee joint X-ray of which 3 was male and 2 was female. Also we experienced 3 cases which necessiat d a correct diagnosis and differential diagnosis. Because this is a common lesion, it frequently accompanies more serious bone conditions, and because this is self-limited disease, treatment is not necessary. It was emphasized that its nature should be understood by all those dealing with bone disease lest it be overdiagnosed and overtreated.

Rhabdomyosarcoma which was first described by Weber has generally been considered an uncommon tumor. Recently it was classified as four types such as pleomorphic, embryonal, botryoid and alveolar type by Stout, Stobbe et al, Horn et al. and Riopelle et al. Five cases of rhabdomyosarcoma treated recently in Seoul National University Hospital were repored in this paper. 1. There were three pleomorphic types, one alveolar type and one undifferentiated malignant mesenchymal tumor. 2. Four cases were between 18-23 years old and one was 37 years old. 3. Three were male and two were female. 4. Time to be consulted to us was average 8 months. 5. Three were in lower extremity (two in thigh, one in calf) and two were in upper extremity (one in elbow, the other in forearm). 6. Follow-up study was done for three cases. One was exoired at,three months after hindquarter amputation, the other was expired at two years after supracondylar amputation and no recurrence and metastases were developed one year after local resection in another one case.

Five cases of the spinal tuberculosis with severe kyphosis and paraplegia were treated with radical surgery at the Department of Orthopedic Surgery, Seoul National University Hospital. All the cases were involved at the thoracic spine and kyphosis were ranged 100 through 150 degrees. Severe fixed chest deformity and impaired pulmonary function were observed. By thoracotomy, aorta was seen to be adherent to the surrounding tissue and this was separated from the verterbral lesion with meticulous procedure. The focus was curetted radically exposing spinal cord which was decompressed with anterior transposition. Then the spine was stabilized with anterior interbody fusion. Postoperative intensive pulmonary care and antituberculous therapy are imperative for the good result. Spinal tuberculosi with severe kyphosis and paraplegia could be benefited with the radical surgery by the experienced surgeon.

BACKGROUND: When T. pallidum invades through skin and mucous membrane, there may be some changes in the expression of cell surface molecules and cell adhesion molecules on the keratinocytes and vascular endothelial cells. OBJECTIVES: The purpose of this study was to investigate histologic changes and changes in the expression of cell surface molecules and cell adhesion molecules on the keratinocytes and vascular endothelial cells according to the time course of primary syphilic lesion. METHODS: We obtained primary syphilitic lesions by inoculation of T. pallidum into the back skin of the rabbit. Biopsies of the syphilitic lesions were performed according to the stages. and H&E and immunohistochemical stains for cell surface molecules were done. RESULTS: 1. Out of the 39 injected sites(103 T. pallidum were inoculated into the back skin of the rabbit), 24(61.5%) primary syphilitic lesion could be found. The duration for the developement of papules, ulcers, and softenings is an average 15 days, 27 days, and 47 days respectively. 2. H & E findings :Acanthosis, spongiosis, and exocytosis in the epidermis were observed in the papule of the primary syphilitic lesion. Infiltration of inflammatory cells, including many lymphocytes, a few histiocytes and plasma cells was also observed. Some cases showed endothelial cell swelling of vessels. Compared to papules, the number of lymphocytes in the ulcer reduced but the number of histiocytes increased. Softened lesion showed infiltrating cells, consisted of lymphocytes and histiocytes, and fibrosis. 3. Immunohistochemical findings :Keratinocytes of the lower epidermis, upper portion of hair follicles, vessel, and infiltrating inflammatory cells in papules and ulcers showed expression of the MHC class II molecule. Most of the infiltrating cells in all cases of papules showed CD5 expression. Keratinocytes, inflammatory cells, and vascular endothelial cells showed positive reaction to ICAM-1 stain in papules and ulcers. VCAM-1 showed the positive reaction to the vascular endothelial cells in the papules and ulcers. In softened lesions, the intensity of the positive reaction to MHC class II, ICAM-1 and VCAM-1 was weakened. CONCLUSION: The skin of the rabbit which was invaded by T. pallidum increased the expression of the cell surface molecules and cell adhesion molecules of MHC class I, MHC class II, ICAM-1, and VCAM-1. We believe that these expressions of cell surface molecules and cell adhesion molecules by T. pallidum, inflammatory cells, activated keratinocytes, and vascular endothelial cells play important roles in the host defence mechanism and the T. pallidum infection.
