For the sake of simplicity, it is as well to divide nail pathogens into dermatophytes (moulds) and yeasts. Most dermatophyte infections of nails, some over 80% are caused by Trichophyton rubrum (T. rubrum), with Trichophyton mentagrophytes (T. mentagrophytes) found in around 10% of cases. Occasionally there are mixed or pure infections with yeast mostly Candida albicans. Rarely nondermatophytic fungi such as Scopolariopsis, Aspergillus, Scytalidium and Fusarium species are found to be causative agents in onychomycosis. Importance of accurate diagnosis of onychomycosis has been increasing because more than half of the patients with nail disorders are non-onychomycotic. Direct smear with KOH is the most simple and valuable laboratory test which will takes about 20 to 30 minutes. the positive rate of direct smear is relatively high (40~60%), but permanent keeping of the positive microscopic findings is not possible. Fungus culture is the most essential laboratory test for identifying the species of the causative fungi even though the positive rate is low (20~50%) and takes 3 to 4 week to have the result. Histopathologic test is a non-invasive diagnostic method with high positive rate (70~90%). The test is very helpful for confirming the mixed infection of the onychomycosis. KONCPA test abbreviated by "melting with KOH of the nail clippings and PAS staining" is a new diagnostic method. Though the test requires some skill and equipments, the positive rate is high (70~90%) and permanent keeping of the positive findings is possible.

BACKGROUND: One of the most, probable pathogenesis of vitiliga is autoimmune. Systemic cor tico st,eroids suppress immunity and may arrest the progression of vitiligo and lead to repigmentation. OBJECTIVE: We have assessed the clinical effect of a oral small oral dose of corticosteroid to minimize side effects in vitiligo patients. METHODS: Thirty four patients(9;male, 25;female) with vitiligo were evaluated in this study. The patients took 7.5mg-20mg prednisolone initially for 2 months and then the dosage was tapered to half of the initial dosiat the 3rd month and half of dose of 3rd month for the last 4th month. We compared the effcct of treatment of vitiligo before and aft.er the study by photographs. and side efferts were issessed at. 1, 2, 3 and 4 month. RESULTS: The arrest of the progression of vitiligo was noticed in 79% of patients and repigmentation was noticed in 59% of patients which is statistically significant. The effect, of treatment according to extent, duiation, type, and site of vitiligo were not statistically significant. The side effects of treatment were minimal and did not affect the course of treatment. CONCLUSION: Small doses of iral corticosteroids are effective without any significant side effects in preventing progression and loiiduce repigmentation of active spreading vitiligo and generalized type of vitiligo that is difficult to treat with topical corticosteroids.

Subungual exostosis are not uncommon, however, they are infrequently mentioned in the dermatologic literature. We report herein a rase of subungual exostosis in the great toe of 16-year-old female student which was confirmed by histopathologic and radiologic findings. Histopathologic examination showed that the mass was ooeered by a dense fibrous tissue which merged into a fibrocartilage cap and bone. Computed tomography showed a well defined, oval shaped, radiopacity of bony density capped by a radiolucency.

We report herein a case of subcutaneous granuloma annulare occured in a 3-year-old girl who had 7 subcutaneous nodules on the scalp, lumbar area, buttock, right tibial area and both ankles. liistopathologically, multiple foci of complete and incomplete collagen degeneration surrounded by histiocytes in a palisading arrangement were seen in the subcutaneous layer. Some vessel walls were thickened and infiltrated with inflammatory cells and many eosinophils were scattered among the inflamrnatory cell infiltrate around the palisaiing granuloma. Five of the lesions disappeared spontaneously and other s decreased in size without specific treatment in 2 months.

We report a case of congenital vellus hamartoma, which consists of numerous matured vellus hair follicles and sebaceous glands. This patient is 4-month-old male with multiple, variable sized nodules on the right epicanthsl fold area since birth. Histopathologically, some of the infundibular portion of the vellus hair shows irregular and reticulated hyperplasia. There are numerous well-formed vellus hair follicles surrounded with thick fibrous sheath and some of them are associated with rudiment sebaceous glands.

Urticaria*

Kimura's disease is a benign, uncommon, chronic inflammatory condition that usually presents with painless subcutaneous nodules or plaques. Head and neck are the most frequently involved sites in Kimura's disease. Mandible is the most commonly involved, followed by neck, cheek, scalp and forehead. Other possible sites are oral cavity, inguinal area and extremities, but there have been no reports involving the nose, especially the one that looks like a rhinophyma. We describe a case of Kimura's disease presenting like a rhinophyma.
