1.Acute Suppurative Arthritis in Neonatal Period

Acute suppurative arthritis in neonates is very difficult to diagnose early since it does not show typical clinical picture and as a results, delayed diagnosis and improper treatment often leads to permanent disability in patient. To prevent the sequelae follwing acute suppurative arthritis, early surgical treatment and administration of antibiotics are essential. We analysed 15 affected joints in 11 neonates less than 1 month old who were treated by surgical treatment and administration antibiotics. The results were as follows. 1) As for the specific joint involved, those affecting hip joint were most frequent(8 joint: 53%), followed by knee joint(5 joints: 33%) and shoulder(2 joints: 14%). Acute osteomyelitis was simultaneously occurred in 10 joints(67%). 2) As for clinical symptoms and signs, 10 cases (91%) showed limited range of motion with the clinical symptoms and signs similar to neonatal sepsis. 3) As for the causative organism, Gram-negative bacilli was the most frequent one(40%), followed by Staphylococcus aureus(27%) and candida albicans. No organisms were identifiable in four joints(27%). 4) As for the sensitivity test to antibiotics, group of staphylococcus aureus was found sensitive to newer cephalosporins while those of gram-negative bacilli to amikacin in 83%. 5) Average time between the first clinical symptom and operation was 5.14days. 6) The hip joint had yielded most complications(5 cases) followed by knee and shoulder joint, one for each. The accompanying osteomyelitis and delayed treatment were contributing factors to complications.

2.Effect of 1% Povidone-iodine for Ophthalmia Neonatorum Prophylaxis.

It has been reported that 2.5% povidone-iodine solution was an effective antimicrobial agent on the conjunctivae of newborns and caused less toxicity than silver nitrate eye drops. The effectiveness and the safety of 1% povidoneiodine solution, a lower concentration solution, were evaluated on ophthalmia neonatorum prophylaxis. A bacterial culture was taken from each conjunctiva of 70 neonates within 30 minutes after birth. A drop of each of the two agents, which were randomly chosen among 1% povidone-iodine solution, 2.5% povidone-iodine solution. and 1% silver nitrate eye drops, was placed on each conjunctiva. Conjunctival bacterial cultures were again taken two to four hours after birth. Before the first culture and at 24 hours after birth, the eyes were examined for toxic changes. To evaluate the effectiveness of the medications, the numbers of bacterial species and colonyforming units from each culture were compared. All three agents significantly reduced the numbers of species and colonyforming units(p<0.02). Two povidone-iodine solutions did not show significant ocular toxicity(p>0.7) but silver nitrate eye drop did(p<0.02). Therefore, it was found that 1% povidone-iodine solution as well as 2.5% povidone-iodine solution are effective antibacterial agents on the conjunctivae of newborns and cause less toxicity than silver nitrate eye drops.

3.Clinical Analysis of Penetrating Keratoplasty in Herpes Simplex Keratitis.

We conducted a retrospective study of 20 penetrating keratoplasty (19patients) for corneal opacity due to herpes simplex keratitis from January 1992 to February 1997 at National Medical Center. We exaniined f requeuey of graft rejection and recurrence of herpetic keratitis after penetrating keratoplasty. The patients were grouped into control & treatinent, group according to the use of antivirial drug after penetrating keratoplasty and we examined the difference in rate of recurrence between two groups. The follow-up period was from 8 months to 68 months, mean follow-up period being 29.6 months. Graft rejection was seen in 3 eyes out of 20 eyes (15.0%) and recurrence was developed 3 eyes out of 20 eyes (15.0%). Recurrent herpetic keratitis developed in 15.4%(2 of 13) of the eyes in control group and 14.3% (1 of 7) of the eyes in treatment group. There was no statistically significant difference in recurrence rate between two groups (p)0.05). Postoperative prophylactic antiviral treatment is not associated with decreased rate of herpes simplex keratitis recurrence.

4.The Effect of Limbal Transplantation & Cyclosporine A for Chemically Damaged Rabbit Cornea.

The authors have evaluated the effect of topically applied 1% cyclosporine A for four weeks after limbal allograft transplantation for ocular surface stability in severely ocular surface damaged rabbits. A total eleven severely damaged rabbits were subdivided into three groups of four limbal autograft (AUTO), three limbal allograft (ALLO), four limbal allograft with topical application of 1% cyclosporine A five times daily for four weeks (ALLO-C). We examined ocular surface smoothness, clarity, corneal vascularization before surgery and three months after surgery, and classified three grade (success, partial success, failure) according to result. In AUTO, three eyes were success, one eye was failure. In ALLO, one eye showed inferior graft detachment and one eye was success, two eyes were failire. In ALLO-C, no graft detachment detected and one eye was success, two eyes were partial success, one eye was failure. We knew that. ALLO-C show more effective ocular surface stability than ALLO. These result suggest ALLO-C can be used as alternative treatment of AUTO in ocular surface reconstruction. But, rnore study for duration and concentration of cyclosporin A will be need.

5.The Effect of Phacoemulsification on the Corneal Endothelium in Diabetic Patients.

We compared the structural and functional corneal endothelial changes between patients with non-proliferative retinopathy and age-matched nondiabetic patients after phacoemulsification with posterior chamber lens implantation. Endothelial cell density was calculated preoperatively, and postoperative 4, 8 weeks by specular microscopy. Fluorophotometry to check corneal endothelial barrier function was underwent preoperatively and at postoperative 4 days. Preoperatively, no significant difference in corneal endothelial cell density and corneal endothelial permeability coefficient was noted between diabetic patients and age-matched non-diabetic patients. But the mean increase of corneal endothelial permeability coefficient at 4 days after surgery was 118.13% in diabetic patients and 61.88% in nondiabetic patients (P<0.05). The mean endothelial cell loss at 4 and 8 weeks after surgery were 10. 29%, 12.55% in diabetic patients and 7.05%, 9.39% in non-diabetic patients.(P<0.05)

6.Tear Turnover Rate Measurement Using Fluorotron Master.

Maintenance of normal rate of tear production and elimination is essential for the proper controlled envionment for the cornea and the conjunctiva. We analyzed tear turnover rates in normal subjects, which mainly depend on tear production. We scanned 65 eyes of 36 normal subjects after instillation of 2% fluorescein solution 1u into the lower conjunctival fornix, using fluorophotometer (Fluorotron Master). After instillation of fluorescein solution, we scanned the eyes every 3 minutes for 30 minutes to measure tear film fluorescence. We calculated the tear turnover rate using tear film fluorescence valuse. Mean tear turnover rates were 12.3+/-6.6 %/min in normal subjects, 12.7+/-6.9 %/min in men, and 11.7+/-6.6 %/min in women. Mean tear turnover rates of men did not differ significantly from those of women (p>0.05). Mean tear turnover rates of younger subjects(25 years old~40 years old) (16.2 +/-7.4 %/min) were found to be significantly higher than ghose of older subjects(41 years old~73 years old) (9.1+/-40 %/min) (p<0.05). The morning tear turnover rates (13.5+/-1.6 %/min) were found to be higher than those for the afternoon (10.2+/-4.9 %/min), but the differences were not significant(p>0.05). We think that this study might be useful to study the diagnosis and treatement of tear secretory system disorders.

7.Research Trends of Non-Pharmacological Interventions for Sleep-Wake Disturbances in Cancer Patients

Purpose:
This review aimed to explore current publication trends, major concepts, measurements, and intervention types regarding sleep-wake disturbance in cancer patients.
Methods:
Studies published up to 26 December, 2021 were examined through PubMed, EMBASE, The Cochrane Library, KMbase, CINAHAL, ScienceON and Google scholar using English and Korean keywords with appropriate definitions related to sleep-wake disturbances in cancer patients. A total of 3,542 articles were found and 73 articles introducing intervention to reduce sleep-wake disturbance were included in the final analysis.
Results:
Interventions for sleep-wake disturbance in cancer patients were classified into the following types: exercise interventions (38.1%), cognitive behavioral therapy (26.2%), complementary and alternative medicine intervention (19.0%), mindfulness-based stress reduction intervention (9.5%), herbal medication intervention (2.4%), dance intervention (1.2%), music intervention (1.2%) and mixed intervention (1.2%). Most of the interventions (52.1%) were performed on breast cancer patients. The Pittsburgh Sleep Quality Index was the most frequently used subjective measurement. Objective measurements to measure sleep quality or sleep disturbance included actigraphy and polysomnography. Sixty-four articles (87.7%) out of 73 found that interventions to reduce sleep-wake disturbance in cancer patients were effective.
Conclusion
Nursing intervention to reduce sleep-wake disturbance is important in oncology nursing practice. This literature review emphasizes the need to develop more tailored and suitable interventions considering various cancer patients’ characteristics.
8.Modified Weaver and Dunn Technique in the Treatment of Acromioclavicular Separation

Thirty one cases of acromioclavicular injuries were treated in both Kangnam Sacred Heart Hospital and Hangang Sacred Heart Hospital during the period from May 1974 to April 1981. Three cases were classified to type 2 injuries and twenty eight were type 3. Among 31 cases one of the type 2 injuries was treated with closed reduction followed by thoracobrachial cast and remaining thirty received surgical treatment. Twenty one cases were operated on by technique of Weaver and Dunn, 8 cases by modified Phemister method and one case by Stewart technique. Preperative and postoperative stress film were evaluated and cap the coracoclavicular distance were measured. In twelve complete dislocation the authors modified the original Weaver and Dunn procedure by taking a bone chip along with the acromial end of the coracoacromial ligament. This seemed to provide a strong bony fulcrum for the suture of the ligament to clavicle as well as promoting bone to bone union. The results of this modified procedure were excellent.

9.Surgical Treatment of Face Turn, and Up and Down Shoot in Duane Retraction Syndrome.

PURPOSE: To present the results of patients undergoing surgical treatment and determine clinical guidelines for the face turn, and up and down shoot in Duane retraction syndrome (DRS). METHODS: Thirty-four patients with DRS were treated with single horizontal rectus muscle recession, lateral rectus (LR) recession with Y-splitting, or LR recession with Y-splitting combined with medial rectus (MR) recession. The different treatment approaches were based on the angle of deviation in the primary position, versions, and ductions. In all patients, ocular alignment, face turn and up and down shoot were assessed preoperatively and postoperatively. RESULTS: The average standard deviation reduced from 18.5 +/- 6.5 prism diopters (PD) to 4.6 +/- 5.8 PD in horizontal rectus muscle recession and 14.4 +/- 5.5 PD to 1.6 +/- 2.8 PD in LR recession with Y-splitting. LR recession with Y-splitting combined with MR recession was effective in reducing globe retraction. Postoperative 4-mm LR recession with Y-splitting did not change ocular alignment; 7-mm LR recession with Y-splitting showed an average correction of 12.8 PD. Postoperatively, all patients presented a reduction in face turn; however, there was no statistically significant difference between the 2 procedures (Mann-Whitney U test, p = 0.620). CONCLUSIONS: Decisions regarding surgical approaches in DRS should be based on the amount of deviation in primary eye position, the expression of up and down shoot, and the degree of face turn. Surgeons planning Y-splitting of LR with MR recession must consider modifying the MR recession amount due to the smaller LR recession effect of Y-splitting compared with conventional LR recession.
