2.Acute Suppurative Arthritis in Neonatal Period

Acute suppurative arthritis in neonates is very difficult to diagnose early since it does not show typical clinical picture and as a results, delayed diagnosis and improper treatment often leads to permanent disability in patient. To prevent the sequelae follwing acute suppurative arthritis, early surgical treatment and administration of antibiotics are essential. We analysed 15 affected joints in 11 neonates less than 1 month old who were treated by surgical treatment and administration antibiotics. The results were as follows. 1) As for the specific joint involved, those affecting hip joint were most frequent(8 joint: 53%), followed by knee joint(5 joints: 33%) and shoulder(2 joints: 14%). Acute osteomyelitis was simultaneously occurred in 10 joints(67%). 2) As for clinical symptoms and signs, 10 cases (91%) showed limited range of motion with the clinical symptoms and signs similar to neonatal sepsis. 3) As for the causative organism, Gram-negative bacilli was the most frequent one(40%), followed by Staphylococcus aureus(27%) and candida albicans. No organisms were identifiable in four joints(27%). 4) As for the sensitivity test to antibiotics, group of staphylococcus aureus was found sensitive to newer cephalosporins while those of gram-negative bacilli to amikacin in 83%. 5) Average time between the first clinical symptom and operation was 5.14days. 6) The hip joint had yielded most complications(5 cases) followed by knee and shoulder joint, one for each. The accompanying osteomyelitis and delayed treatment were contributing factors to complications.

6.A study of skin responses produced by sodium lauryl sulfate and ethanol, and quantification of erythema using a chroma meter in healthy men.

Irritant skin reactions can be evaluated by several techniques. Using different scores for the degree of erythema, edema, scaling and fissuring is the time-honored approach but implies t,he disadventagrs of lacking objectivity and pararretric properties. This paper describes the objective nteasurement of irritant. skin responses to various concentrations of Sodium Lauryl Sulfate(SLS) and Ethanol by a color reflectance meter(Chroma Meter) and compares with visual scoring The results are summarized as follows : 1. Pretreatment assessments by a Chroma Meter on normal forearm area showed an average score of 7.70+1.53 for Chroma Meter value a*(rediies:; score). 2. Skin responses to SLS in various concentrations of 0.5, 1.0, 2.0, 3.0, 4.0, 5.0, 7.5% under occlusion for 24 hours were assessed by visual scoring system. They are measured 0.56+1.10, 0.74+1.29, 1.19 1.36, 2.11+1.69, 2.19+1.91, 2.56+1.50 2.81+2.16 respectively. 3. Skin responses to SLS in various concentrations of 0.5, 1.0, 2.0, 3.0, 4.0, 5.0, 7.5% under occlusion for 24 hour were assessed by a Chroma Meter valu a*(redness score). They are measured 8.29+1.98, 8.37+2.47, 9.31+2.27, 10.34+2.97, 10.35+3.18, 10.51+2.47, 11.61+3.45 respectively. 4. Skin responses to Ethanol in various concentrations were nearly negligible. 5. We have demonstrated there is a highly significant correlation between skin redness measured by the Chroma Meter and visually assessed erypthma(p<0.0001). It yields reproducible, objective, quantitative measurements of iryhema that parallel the subjective visual assessments.

8.The Positive Rate for Serum Anti-HBs in Korean since the Universal Carrying Out of HB Vaccination.

The positive rate for serum anti-Hbs was analized among 424 0f HA vaccinated and 2035 of non-vaccinated cases at the Yeungnam University Hospital, Computed Automated Med-screening Center. Most of them from Kyungbook province and they had the last HB vaccination in the periods of 3 to 42months prior to this study. The followings were obtained. 1. The positive rates for serum HBsAg were 0.7% in the vaccinees, 9.6% in the non-vaccinated and 8.0% in the whole cases. 2. The positive rates for serum ant-HBs were 66.3% in the vaccinees, 47.9% in the non-vaccinated and 51.6% in the whole cases. 3. The positive rates for serum anti-HBs were 51.4% in the cases with one time of vaccination and 68.6% in the cases with two times of vaccination. On the vasis of these findings the positive rate for serum anti-HBs among the vaccines was significantly higher than of non-vaccinated (P<0.05). The positive rate for serum anti-HBs shortly after vaccination was higher than that of present our study which was made relatively long period after vaccination. As the reason a natural decrease of the titers of the serum anti-HBs can be postulated as one of the contributing factors for the discrepancy. In order to keep to serum antibody of perfect protectivity against HBV, it may be better to check the serum anti-HBs just vaccination, follow up and take booster injection when it is needed.

9.Three Cases of Tetracyclines Induced Esophageal Ulcer.

In recent years, an increasing number of drugs have been reported to cause direct esophageal damage. More than 30 cases on tetracyclines induced esophageal ulcer have been reported since the first description of tetracycline induced esophageal ulcer by Bokey and Hugh in 1975. In Korea, only one case of doxycycline-unduced esophageal ulcer has been reported. Authors have experienced 3 cases of esophageal ulcer probably caused by tetracyclines. The patients had taken their capsules just before going to bed with little fluid intake. About 6-8 hours later they had felt substernal burning sensation and epigastric discomfort. Gastrofiberscopy revealed relatively well demarcated circular ulcers on the mid esophagus. An esophagogram showed no apparent abnormality. Patients's symptoms became negligible with antacid treatment within 2-5 days. One of the causes of the esophageal ulcer is thought to be the delay in transit time of drugs and direct esophageal damage from mucosal contact when tablets are ingested in the recumbent position without an accompanying proper quantity of fluid. If only physicians endow patients with more concern about drug induced esophageal ulcer, they could find out more increasing number of drug induced esophageal ulcers by gastroscopic examination and thereby could prevent tetracycline induced esophageal ulcer.

10.Treatment of Diaphyseal Fractures of the Radius and Ulna with Dynamic Compression Plates: A Retrospective Study of 52 Fractures in 33 Patients

Reduction of displaced diaphyseal fractures of the radius and ulna in adult must be nearly anatomical for restoration of mormal function. Most of the fractures require operative management, and various methods of open reduction and internal fixation have been recommended. Because we believe that anatomical reduction followed by rigid internal fixation provides the most satisfactory results for these injuries, dynamic compression plating method is frequently used in our hos pi tal. We studied 52 diaphyseal fractures of the forearm bone in 33 patients treated at the S.R.C.H. from May 1980 to December 1985. Of the 33 patients, 20 had fractures of the radius and ulna; 6, fractures of the radius only; and 7, fractures of the ulna only. The fractures of both bones were treated with plate and screws in 19 patients. In another one, the ulna was treated with plate and screws and the radius was treated by screw fixation only. The results were as follows: l. Of the 52 fractures, 51(98.1%) were united after the initial operation within 23 weeks. 2. The average time for radiological union of the fracture, excluding those complicated by infection or non-union, was 11.1 weeks for 24 radii and 10.9 weeks for 25 ulnae. 3. Only one patient(3.0%) was complicated by infection and only one(1.9%) non-union of radius occurred. 4. The functional results were excellent or satisfactory in 25 patients(83.3%). 5. We have found that in adults the auto compression plating is a successful method for the diaphyseal fractures of the forearm.
