1.Hypersensitivity pneumonitis induced by oyster mushroom spores.

Hypersensitivity pneumonitis due to the inhalation of oyster mushroom(pleurotos ostreatus) was demonstrated in a 44-year-old man who had cultiviated for 4 years. He had suffered from dyspnea, cough, myalgia and fever in the evening after working at first. After them he has experienced dyspnea aggravated. He showed not only positive reaction to the intradermal test but also to the bronchial challenge test such as 6 hr after inhalation falling down of FVC, FEV p, and DLCO in pulmonary function test and leukocytosis with fever upto 38.2C with antigenic extract of oyster mushroom spore. The size of spore was approximately 3.5 when measured by transmission electron microscopy.

5.Correlation between Apoptosis and Proliferative Activity in Squamous Cell Neoplasia of Uterine Cervix.

OBJECTIVE: Quantitative analysis of both apoptosis and proliferation should provide important insight into tumor biology. The aim of this study is to clarify the significance of apoptosis in the progression of uterine cervical neoplasia, including cervical intraepithelial neoplasia, and microinvasive and invasive squamous cell carcinoma categories. in relation to cell proliferation. METHODS:: Forty five cases of cervical intraepithelial neopasia. 10 of microinvasive carcinoma, and 10 of invasive squamous cell carcinoma were examined using formalin fixed and paraffin embedded samples. The TdT mediated dUTP-biotin nick end labelling(TUNEL) method for apoptotic cells, and Ki-67 and Topo-II immunojhistochemistry for proliferating activity were performed. RESULTS: Apoptotic and proliferative indices, calculated by counting positive nuclei among 1,000 nuclei of cancer cells, should significant positive correlation with histological malignancy grading in CIN and tumor cell invasion into stroma, and showed also significant positive correlation between them. CONCLUSION: These findings suggest that apoptosis in cervical neoplasia may be closely related to tumor cell differentiation and progression.

6.PSYCHOLOGICAL ASPECT OF PATIENT SATISFACTION AND ACCEPTANCE OF COMPLETE DENTURES.

In spite of the progress in techniques and materials of the prosthodontic rehabilitation of patients with complete edentulous arches, prosthodontists still face patients' complaints about dissatisfaction and discomfort from their dentures. In the past several decades, prosthodontists tried to find the factors that influence the patient's satisfaction with dentures. However the results are contraversial especially with the psychological factor. In this study using patient denture satisfaction questionnaire' and Hopkins Symptom Checklist, we tried to find the correlation between the patient's denture satisfaction and the patient's psychological aspects. 23 complete edentulous patients who have visited the Department of Prosthodontics, Yonsei University Dental Hospital from September 1998 to June 1999 for complete denture treatment were asked to complete the questionnaires 4 to 6 weeks after the upper and lower complete delivery. After the measurement of validity of the questionnaires, correlations between the patient's general satisfaction with their new upper and lower complete dentures and the other satisfaction questions including the satisfactions with esthetics, retention, mastication, speech, comfort and other peoples's opinion and between the patient's general satisfaction with their new upper and lower complete dentures and the 5 symptoms of Hopkins Symptoms Checklist including somatic, obsessivecompulsive, interpersonal sensitivity, depression and anxiety were analyzed. Among the several satisfaction questionnaire items, comfort with the lower denture showed highest relation to the patient's general satisfaction with dentures. However, only the anxiety scale of Hopkins Symptom Checklist among the other symptom scales was related to the patient's general satisfaction with dentures. The two questionaires used in this study turned out to be valid means of analyzing patient's denture satisfaction and psychological status before and after the complete denture treatment.

7.A Clinical and Histopathologic Study of Epidermal Cysts.

BACKGROUND: Epidermal cysts are the most common of all cysts. OBJECTIVE: The purpose of this paper is to report the clinical and histopathologic features of epidermal cysts. METHODS: We reviewed the clinical and histopathologic features of 205 cases that were diagnosed with epidermal cysts at the department of dermatology in Anam Hospital of Korea University for 11 years, from January 1983 to December 1993. RESULTS: 1. Of the 3,400 cases of skin biopsy specimens, 205 cases were epidermal cysts(6%). There were 141 male cases (68.8%) and 64 female cases (32.2%). The ratio of male to female was 1:0.45. 2. Most patients with epidermal cysts belonged to age ranging from 20 to 59 with the highest incidence in the 3rd decade. 3. The site of predilection of the epidermal cysts were face 94(45.9%), trunk 37(18.0%), neck 23(11.2%), leg 21(10.2%), arm 19(9.3%),scalp 7(3.4%), scrotum 2(1.0%), and vagina 2(1.0%). 4. Histopathologically, 61 cases (29.3%) were ruptured. The most common cystic wall change was acanthosis followed by atrophy, hyperplasia, hypergranulosis, parakeratotis, basalioma-like change, squamous eddies, dyskeratosis. The most common cystic content was keratinous material followed by parakeratotic cells, pigment, bacterial colony, RBC, inflammatory cells, hair shaft, calcification, trichilemmal keratinization, pilomatricoma-like change. The most common stromal change was giant cells followed by fibrosis, granulation tissue formation,vessel proliferation, pigment. CONCLUSION: According to our results, which agree with those of Chung, the epidermal cysts occur as a disease particular to young men, especially on the face. Histopathologically, when an epidermal cyst ruptures and the contents of the cyst are released into the dermis, a considerable foreign body reaction with numerous multinucleated giant cells results.

9.Sleep Apnea in Older Adults.

Sleep disordered breathing is highly prevalent in the elderly and affects older men and women similarly. Nocturia, cardiovascular diseases, cognitive impairment, traffic accidents and repeated falls are common manifestations of sleep apnea in the elderly and sleep study may be indicated in these patients. When a sleep study is planned, reliable markers of respiratory efforts and a tibial EMG should be included because central sleep apneas and periodic leg movements are common in the elderly patients. Although cardiovascular morbidity and mortality seem to be lower in the elderly than in middle-aged adults, these may not be as low as commonly believed. Excessive daytime sleepiness, cognitive dysfunction and nocturia can be improved by effective treatment with continuous positive airway pressure and a therapeutic trial should be recommended to all symptomatic patients. It is still unclear whether sleep apnea in older adults is a specific entity or the same disease as in younger adults. Further clinical research is warranted.
