3.Hypersensitivity pneumonitis induced by oyster mushroom spores.

Hypersensitivity pneumonitis due to the inhalation of oyster mushroom(pleurotos ostreatus) was demonstrated in a 44-year-old man who had cultiviated for 4 years. He had suffered from dyspnea, cough, myalgia and fever in the evening after working at first. After them he has experienced dyspnea aggravated. He showed not only positive reaction to the intradermal test but also to the bronchial challenge test such as 6 hr after inhalation falling down of FVC, FEV p, and DLCO in pulmonary function test and leukocytosis with fever upto 38.2C with antigenic extract of oyster mushroom spore. The size of spore was approximately 3.5 when measured by transmission electron microscopy.

7.Hyperkeratosis of the Nipple and Areola: report of a case and review of literature.

We described here a case of hyperkeratosis of the nipple and areola. A 23-year- old woman had a verrucous thickening and brownish discoloration on her right nipple and areola that had been present for 4 yeare. The only symptom was an intermittent mild itching sensation. The patient had no other additioa.al hyperkeratotic conditians. She was not married and had never had a pregnancy. Riopsy spccimen showed papillomatous elongation of the epidermis and edema, of the underlying papillary eonnective tissue. The epidermis was parakeratotic and hyperpigmented, and showed focal intercellular and intracellular edema. The present case seems to be the nevoid form by the Levy-Franckel classification. The world literature was briefly reviewed.

8.COMPARISON OF WEAR RESISTANCE AMONG RESIN DENTURE TEETH OPPOSING VARIOUS RESTORATIVE MATERIALS.

The aim of this study was to compare wear resistance of resin denture teeth opposing various restorative materials. The wear resistance of conventional acrylic resin teeth (Trubyte Biotone) and three high-strength resin teeth(Bioform IPN, Endura, SR-Orthosit-PE) opposing different restorative materials (gold alloys, dental porcelain, composite resin) was compared. Wear tests were conducted with a sliding-induced wear testing apparatus which applied 100,000 strokes to the specimen in an mesio-distal direction under conditions of 100 stroke/min and constant loading of 1Kgf/tooth. Wear resistance of the resin denture teeth was evaluated by the following criteria : 1) wear depth, 2)weight loss, and 3) SEM observation. Results were as follows. 1. When opposed to gold alloys and composite resin, high-strength resin teeth showed superior wear resistance compared to acrylic resin teeth. But, in cases opposing dental porcelain, differences between the wear of the high-strength and acrylic resin teeth were not statistically significant (p<0.05). 2. When comparing wear resistance among high-strength resin teeth, opposing gold alloys, Endura was slightly more resistant and while in cases opposing dental porcelain, SR-Orthosit-PE was showed to be slightly resistant(p<0.05). 3. The wear of high-strength resin teeth was greater by 5 to 7 times when opposing porcelain and 2 to 3 times when opposing composite resin compared to gold alloys(p<0.05). 4. SEM observations of the wear surface showed that wear of resin teeth opposing gold alloys is a fatigue type of wear and wear of resin teeth opposing dental porcelain is fatigue and abrasion type of wear. Trubyte Biotone showed more severe fatigue type of wear than high-strength resin teeth. In conclusion, the use of dental porcelain should seriously be considered as restorative material in cases opposing resin denture teeth and improvement seems to be needed on resin teeth in the areas of wear resistance.

9.Prophylactic Endoscopic Variceal Ligation for Esophageal Varices with High - risk of Hemorrhage.

Prophylactic treatment of varices is an appealing concept because 50% of patients who experience variceal bleeding will die within the first 6 weeks of the first bleeding. However, the majority of trials which have evaluated prophylactic therapy gave failed to demonstrate advantage, We tried prophylactic endoscopic variceal ligation(EVL) in 10 patients, to evaluate the safety and effect of prophylactic EVL for esophageal varices with high-risk of hemorrhage. The eradication rate was 100% without bleeding and mortality, the mean session for eradication of varices 1.9, the number of bands per person 16.9 and the number of bands per session 8.9. Although mild chest pain(5.3%) and chest discomfortness(31.6%) were observed, no serious complication related with EVL resulted from 19 EVL sessions. The patients were followed for a mean of 327.0 days(85-708), during which recurrent esophagea1 varices were found in a case at 260 days from last session, but no bleeding nor death was occured. No late complication of EVL was documented. In conclusion, prophylactic EVL is safe and may be effective for esophageal varices with high-risk of hemorrhage. But, the large controlled-trial should be required.
