1.Modified Weaver and Dunn Technique in the Treatment of Acromioclavicular Separation

Thirty one cases of acromioclavicular injuries were treated in both Kangnam Sacred Heart Hospital and Hangang Sacred Heart Hospital during the period from May 1974 to April 1981. Three cases were classified to type 2 injuries and twenty eight were type 3. Among 31 cases one of the type 2 injuries was treated with closed reduction followed by thoracobrachial cast and remaining thirty received surgical treatment. Twenty one cases were operated on by technique of Weaver and Dunn, 8 cases by modified Phemister method and one case by Stewart technique. Preperative and postoperative stress film were evaluated and cap the coracoclavicular distance were measured. In twelve complete dislocation the authors modified the original Weaver and Dunn procedure by taking a bone chip along with the acromial end of the coracoacromial ligament. This seemed to provide a strong bony fulcrum for the suture of the ligament to clavicle as well as promoting bone to bone union. The results of this modified procedure were excellent.

2.Treatment of Diaphyseal Fractures of the Forearm Bones

In clinical practice fractures of the forearm bones are encoutered as frequently as fractures of other bones. As has been pointed out in many articles, however, the surgical anatomy of the forearm evokes problems in dealing with the diaphyseal fractures of the forearm bones not found in the treatment of diaphyseal fractures of other long bones, The authors have experienced 107 cases of diaphyseal fractures of the forearm bones during the fiveyear period from January, 1972 through December, 1976. A comparison has been made between the two groups one treated by conservative method and the other by open reduction and internal fixation. The results are as follows: 1. The time required for the healing of the fractures was shorter in the conservatively treated group. 2. Restoration of function was more satisfactory in the surgically treated group. 3. Rotational and angulatory deformities were less in the surgically treated group. 4. Forty-one fractures were internally fixed with compression plates, the union rate of which was 100%.

4.Effects of Intraoperative Exposure and Postoperative Subconjunctival Injections of 5-fluorouracil in Reducing Adhesions after Strabismus Surgery in Rabbits.

Recently, antimetabolotes such as 5-fluorouracil and mitomycin have been used successfully to reduce postoperative adhesions in animal models after strabismus surgery.Nine rabbits, 18 eyes, 36 areas were divided into 4 groups. The first group was a control one. The second group was treated with 5-fluorouracil intraoperatively. The third group was treated in the same manner as the second group, followed by subconjunctival injections of 5-fluorouracil from postoperative 7th day. The fourth group was treated with mitomycin C intraoperatively.The thied group(booster group) and the fourth group(mitomycin group) were superior to the others in preventing the postoperative adhesions. There was no statistically significant diference in scar length and adhesion grade between booster group and mitomycin group.This study reveals that the subconjunctival injections of 5-fluorouracil after intraoperative contact may reduce postoperative adhesions in secondary strabismus surgery and may prevent the possible serious complications of mitomycin C, giving the similar effect to potent mitomycin C.

5.The First In-Training Examination for Residents in Orthopaedic Surgery

On July 5, 1980, the first in-training examination for Korean orthopaedic residents was performed to give each of them an evaluation of his general level of orthopaedic knowledge, and to provide their program directer with a comparative basis for evaluation of their programs in various subject areas. Forty one of the 42 residency programs in this country presented in the examination. The results of the examination and the survey of public opinion performed after the examination are as follows. 1. Two hundreds and ninety one of the 312 Korean orthopaedic residents at all level of training participated in the examination, so that the participation rate was 93.3 percent. According to the survey, the respondents wanted to continue the in-training examination in 77 percent. 2. The examination was not sufficient in its reliability because the questions were small in number, and it was deficient in its objectivity since there were 40 percent of the subjective questions. The test was not enough in its practicability because the questions of applying the knowledge clinically such as problem solving one were relatively insufficient in its number. 3. The mean scores of the. residents are increasing regularly with increasing years, which can be interpreted as the questions are somewhat adequate for the test and the orthopaedic training in Korea is efficient one. But the standard deviations of each mean is large, which means the residents level of the orthopaedic knowledge and his training are greatiy varied. The scores of the senior residents are not yet reach a plateau and the scores of the junior residents are inferior to that of the 1966 American counterpart. 4. According to the survey, the majority wants to open the questions with their referencies after the examination, but wants to neither open his score nor reflect his score in that of the Korean orthopaedic board examination.

The authors report two unusual tumors detected in the right scapula and around left aciatic nerve. Although fibromatosis is benign tumor, the nature is notorious for an aggreasive clinical couras with a tendency to recur in high percentage. Recently we have encountered two instances of fibromatosis, and presented with abort discussion of the clinical pathologic aspects with some review in literatures.

7.Clinical Failures of Internal Fixation

Since the first attemt of internal fixation not much more than a century ago, implants have come to play an important role in orthopaedic surgery. Yet little has been reported about complications which may result from implant surgery. During the period of May 1980 to April 1984, 32 cases of fixation failures were treated in the Department of Orthopaedic Surgery, Hallym College, Kang Nam Sacred Heart Hospital and results were obtained as follows: 1. The most common area involved femoral shaft, but the highest rate was distal femur. 2. The most common period of failure was between 2 to 4 months postoperatively. 3. The leading cause of failure was remaining defects at fracture site. 4. The authors recommend accurate reduction of fracture and bone graft, if necessary, adequate immobilization postoperatively and co-coperation of patient to avoid failure of implant.

8.Conjunctival Bacterial Flora at Birth: Comparisons between Cesarean Section and Vaginal Delivery.

Conjunctival bacterial cultures for 113 neonates who were delivered by cesarean section or vaginal delivery were performed within 30 minutes after birth. These neonates were in the condition of no eye drops applied. The 43 newborns were delivered by cesarean section(CS group) and the 70 newborns were delivered vaginally(VD group). The 10 newborns(P1) (14 eyes(P2)) in the CS group and the 29 newborns(41 eyes) in the VD group showed culture positivity. These differences between two groups were statistically significant(p1, p2<0.05). The CS group had significantly fewer species and colony forming units cultured per subject (0.395 +/- 0.821(p3), 5.116 +/- 27.095(p4) than the VD group(0.971 +/- 1.393, 25.843 +/- 62.285) (p3,p4<0.05). Coagulase negative staphylococci were predominantly isolated in the CS group and coagulase negative staphylococci, gram positive bacilli, E. coli, nonfermenting bacilli, and S. aureus in the VD group. Therefore it was found that the conjunctiva of the newborns delivered by cesarean section bore significantly fewer bacteria and need less prophylactic measures than that of newborns delivered vaginally.

9.Tuberculous Lesion of the Esophagus.

Tuberculosis of the esophagus is rare. We experienced four patients with esophageal tuberculosis proven by esophagoscopic biopsy or surgical specimens. Case 1 was a patient with primary esophageal tabereulosis, case 2 was a patient with teberculous esophagobronchial fistula and case 3,4 were patients with esoyhageal perforation due to tuberculous traetion diverticulum of the esophagus. The clinical, radiological and esapbagoscopic findings may be nonspecific in diagnosis of esophageal tuberculosis. Therefore early diagnosis should be rested on a high index of suapieion and a cautious attention.

10.Visual Rehabilitation of Optic Atrophy Patients with Low Vision Aids.

In oder to evaluate the efficacy of low vision aids in patients with optic atrophy, we analyzed sex, age distribution, preexisting conditions, visual acuities before and after low vision aids use. The sorts and magnifications of prescribed low vision aids were studied in patients with optic nerve atrophy whose visual acuities were not corrected with conventional glasses or contact lens. Our results indicate that distant and near visual acuities with low vision aids were significantly improved in patients with optic atrophy(p<0.01, paired ttest). We think that low vision aids are required to rehabilitate the visually impaired patients caused by optic atrophy because of high probability of visual rehabilitation.
