2.Papillary Cancer of the Thyroid Gland in Childhood and Adolescence: A Review of 3 Patients.

Thyroid cancer is not only an infrequent cancer, but also is the most common malignancy of the endocrine system. When thyroid cancer occurs in childhood and adolescence, it is an unusual but interesting malignancy. Thyroid cancer in childhood and adolescence has a slightly lower preference for female than for male subjects, as compared to adult patients. With regard to the pathogenesis the only factor definitely known to promote development of thyroid cancer in the pediatric age group is irradiation. The evidence demonstrates that thyroid carcinoma in the pediatric age group is a biologically self-reliant and more hostile nature than in adults but paradoxically the prognosis is good. Enhanced survival resulting from advances in therapy in patients with Hodgkin's disease is associated with long-term morbidity, including the potential for the development of a second solid malignancy. Second malignant neoplasms(SMNs) have become a principal consideration in evaluating the prolonged consequences of strategy in pediatric oncology. Thyroid carcinoma has proven to be a common SMN. We have experienced 2 cases of papillary thyroid cancer following radiation therapy for Hodgkin's lymphoma and a papillary thyroid cancer with pulmonary metastases.

5.A case of pilomatrixoma in the cheek.

We report a case of pilomatrixoma presenting in the right cheek of a 6-year-old girl. In the panoramic view, a small, ovoid-shaped, and nonhomogenous calcified mass was superimposed on the right mandibular angle. The mass was located on the skin overlying the right mandibular ramus area in the skull P-A view. The tumor had a strongly reflective pattern with acoustic shadowing in sonographic view. We also illustrate how these lesions can be effectively diagnosed.

6.The relationship among reference lines used for taking the extraoral radiography.

PURPOSE: The aim of present study is to estimate the relationship among reference lines used for taking the extraoral radiography and panoramic radiography. MATERIALS AND METHODS: Lateral cephalographs of 40 adults, aged 22 to 30 years were taken. Angles between reference lines (Frankfort line, canthomeatal line, infraorbitomeatal line, and occlusal plane) were measured. RESULTS: Angles between Frankfort line and canthomeatal line, Frankfort line and infraorbitomeatal line, Frankfort line and occlusal plane, canthomeatal line and infraorbitomeatal line, canthomeatal line and occlusal plane, and infraorbitomeatal line and occlusal plane were 16.1degrees(+/-2.2), 5.8degrees(+/-2.4), 8.7degrees(+/-3.5), 10.3degrees(+/-1.3), 24.8degrees(+/-4.0), and 14.5degrees(+/-4.4), respectively. CONCLUSION: Angle between frankfort and canthomeatal line is very different with commonly known angle. Frankfort horizontal reference lines used for panoramic radiography need an exact definition and more accurate standard is needed for the taking of panoramic radiograph

7.Comparison of static MRI and pseudo-dynamic MRI in tempromandibular joint disorder patients.

PURPOSE: The purpose of this study was to evaluate comparison of static MRI and pseudo-dynamic (cine) MRI in temporomandibular joint (TMJ) disorder patients. MATERIALS AND METHODS: In this investigation, 33 patients with TMJ disorders were examined using both conventional static MRI and pseudo-dynamic MRI. Multiple spoiled gradient recalled acquisition in the steady state (SPGR) images were obtained when mouth opened and closed. Proton density weighted images were obtained at the closed and open mouth position in static MRI. Two oral and maxillofacial radiologists evaluated location of the articular disk, movement of condyle and bony change respectively and the posterior boundary of articular disk was obtained. RESULTS: No statistically significant difference was found in the observation of articular disk position, mandibular condylar movement and posterior boundary of articular disk using static MRI and pseudo-dynamic MRI (P>0.05). Statistically significant difference was noted in bony changes of condyle using static MRI and pseudo-dynamic MRI (P<0.05). CONCLUSION: This study showed that pseudo-dynamic MRI didn't make a difference in diagnosing internal derangement of TMJ in comparison with static MRI. But it was considered as an additional method to be supplemented in observing bony change.

Lower gastrointestinal complications often develop in end stage renal disease patients, and among the more problematic is recurrent colon ulcer. The exact pathogenesis of this condition is not known and there were no specific therapeutic modalities concerning this type of disease entity. We report, with a literature review, a case of recurrent colon ulcer with intermittent hematochezia in an end stage renal disease patient on long term hemodialysis that improved after conversion to peritoneal dialysis.

10.Clinical Study for The Ovarian Malignancy Under the Age of Twenty Years Old.

OBJECTIVE: The purpose of this study was to review the incidence, types, chemotherapy regimens, operation methods, pregnancy rate and propiosis of the ov#arian malignancy under the age of twenty years old. METHODS: Retrospective reviews of the medical recordings for 22 patients with ovarian malignancy under the age of twenty years old in the Department of Obstetrics and Gynecology, Hanyang University from 1986 to 1997 were done. RESULTS: Gerin cell tumor accounts for the majority of cases (77%), whereas 5 patients(23%) belong to the common epithelial poup which were all mucinous type. Our experiences with 22 cases are as followings: 5 mucinous cystadenocarcinoma, 8 immature teratoma, 4 endodermal sinus tumor, 3 dysgernma and 2 mixed germ cell tumor. The incidence of this group was 8.2% (22/266) in all ovarian malignancy. Fourteen of them are stage I, each one is, stage II and IV, and six patients are stage III. The average age was 14.9 years old in germ cell tumor and 18.4 yems old in mucinous cystadenoearcinoma. Malignant ovarian cancer under the age of twenty can be treated with conservative surgery, followed by adjuvant chemotherapy. Commonly used chemotherapeutic regimens were VAC(Vincristine, Actinomycin-D; Cyclophosphamide), VBP(Vinblastine, Bleomycin, Cisplatin) and BEP(Bleomycin, Etoposide, Cisplatin). Second look operations were done in 11 patienth and histologic positive findings were detected in 4 of them. The length of follow-up ranged ftom 3 months to 137 months and median value was 44 rnonths. CONCLUSIONS: It is concluded that for young women who wish to preserve child-bearing capacity, regardless of the stage of the tumor, fertility preserving surgery with complete surgical staging, if necessary followed by combination chemotherapy is an appropriate treatment.
