1.Atopic Cataract Associated with Atopic Dermatitis.

Atopic dermatitis is one of the commoner dermatoses of children, adolescents and young aduIt. Atopic persons manifest (1) a high degree of allergic hypersensitivity with the developmmt of atopic reagins in the blood. (2) an exaggerated reaction to various forms of physical and ernotional stress. (3) a favorable symptomatic response to steroidal therapy. (4) an exaggerated reaction to infection with the virus of vaccinia and herpes simplex and (5) an abnormal physiologic response in the skin and the superficial vascular network to physical agents and the acetylcholine. The relation of atopic dermatitis (neurodermatitis disseminata) to various manifestations of ocular disease has been well documented: in the ophthalmologic literature. Cataract formation, re tinal detachment and keratoconjunctivitis have been noted most often. and keratoconus and atopic uveitis are apparently uncommon. Frequency of occurrence of atopic cataract and keratoconjunctivitis was evaluated clinically in 117 patients suppering from atopic dermatitis who were visited to out-patient clinic of dermatology of Eul-Ji Geaeral Hospital during the years 1980 to 1981. A distinctive form of ocular cataract, as a significant complication of atopic dermatitis in the years of adolescence and young adult life, was found in 7 of 117 patients with atopic dermatitis.

2.Clinical Study on Diabetic Cataract.

A review has been made of 500 cases of which had been undertaken fundoscopic and slit lamp examination. Senile cataract in diabetics tends to be associated with diabetes of long duration. They were analyzed according to sex, age, duration, fasting blood sugar level, visual acuity and fundus findings. The results are obtained as follows. 1. Incidence of cataract among 500 cases of diabetics is 41%. 2. Prevalence of cataract is significantly associated with duration of diabetes(P<0.005). 3. Prevalence of cataract is significantly associated with age over 40. 4. Prevalence of cataract is significantly correlated with FBS level with duration of diabetes of under 5 years(r=0.8). 5. prevalence of cataract is significantly associated with visual acuity(p<0.005). 6. Diabetic Retinopathy was found in 50% of those with cataract

3.Tattooing of the Cornea with Platinum Chloride.

Untill now, we have been using Chinese ink for corneal tattooing. The reason is that it was difficult to get Platinum Chloride and Hydrazine Hydrate. As we got the result by using Platinum Chloride and Hydrazine Hydrate, we are reporting as follows: 1. Platinurn Chloride tattming has stained in black at corneal opacity equally. 2. It stains at corneal stroma without any injury and good especialIy when corneal tissue is thin. 3. It fully stains by seperating corneal epithelium and there are no complication and side reaction. 4. The method is simple and aseptic operation is possible. 5. The pain disappears after 2 days from the stain and conjuntiva injection disappears after 4~5 days from the stain. 6. The condition of stain is perfect although it has passed 4 months from the stain.

4.Changes of Intraocular Pressure after General Anesthesis with Succinylcholine Pretreated with d-Tubocurarine and Pancuronium.

Small dose of nondepolarizing muscle relaxants are often recommended as prior medication of succinylcholine in order to avoid the elevation of intraocular pressure elicited by succinylcholine The effects of muscle relaxants on the intraocular pressure were studied in 45 human subjects. Intraocular pressure was significantly lowerd by d-tubocurarine 3 mg from 14.7 mmHg to 11.3 mmHg, and by pancuronium 0.08 mg/kg from 15.6 mmHg to 13.4 mmHg. When succinylcholine was given alone, subjects shows a mean elevation in intraocular pressure. There were no significant changes in intraocular pressure between d-tubocurarine and pancuronium given prior to succinylcholine.

5.Two Cases of Giant Corneal Dermoids.

6.Retinal Hemorrhages in Newborn Infants.

In newoborn infants, we can find the retinal hemorrhage, hyphema, vitreous hemorrhage, cataract, iridodialysis, choroidal rupture, lens dislocation and the injuries to the conjunctiva and lid due to birth injury. We studied the frequency, influencing factors, shape and absorption period of retinal hemorrhages in newborn infants by the following method. The 152 newborn infants that were delivered at the obstetric department of S.C.H. college hospital from May 25th '80 to June 29th '80 are examined with direct ophthalmscope after instillation of 1% mydriacyl solution. Fundus eyamination was done within 24 hour after delivery, 3rd day, 2nd week, and 3rd week. 1) The retinal hemorrhage is seen in 19.1% of newborn infant. 2) Bilaterality is over 55%. 3) Frequency is higher in primipara than in multipara and higher in over 30 years old mother and vaccum deliveries. 4) The retinal hemorrhages are absorpted about 1/2 of cases within 3 days and almost within 3 weeks.

7.The Detection of the p53 Protein in Cervical Cancer and CIN by Immunohistochemistry.

The cell cycle is composed of a series of steps which can be negatively or positively regulated by various factors. p53 gene aberrations are common in human malignancies, and recent studies suggest that in cervical carcinoma p53 function is inactivated either by complex formation wilh human papilloma virus (HPV) E6 product or by gene mutation. To study the expression of p53 gene in the cervical cancer and cervical intraepithebal neoplasia, immunohistochemistry for the p53 protein was done in the 47 cases of squamous cell carcinoma, 6 cases of adenocarcinoma and 32 cases of cervical intraepithelial neoplasia. I. The p53 protein was detected in the 31% of cervical intraepithelial neoplasia (10/32 cases). 2. The p53 protein was detected in the 55% of invasive cervical cancer (29/53 cases). 3. By the histologic type of cervieal cancer, the p53 protein was detected in the 57% of squamous cell carcinoma (27/47 cases) and 33% of(2/6 cases) adenocarcinoma. The p53 protein wes more frequently detected in the squamous cell carcinoma than in the adenocarcinoma. 4. By the staging in cervical cancer, the p53 protein was detected in the 31% of stage 0, 50% of Stage Ia, 50% of stage I b, 75% of IIa and 50% of stage II b.

8.Two Cases of Redundant Nerve Root Syndrome of Cauda Equina.

Two cases of redundant nerve root syndrome of cauda equina in patient was described. The general clinical features were the intermittent neurologic claudication with improvement at rest, low back pain of long duration and radicular pain in one or both legs, sensory & motor change in one or both legs. Myelography revealed a partial to complete block with characteristic serpentine filling defects above or below the block simulating a vascular malformation. One case was associated with lumbar H.N.P and the other cases were associated with L-H.N.P and pseudospondylolithesis. In addition to, we reviewed the literature of redundant nerve root syndrome of the cauda equina.

9.Exercise Pulmonary Function Test in Preresection and Postresection of the Lung due to Malignancy - follow up after 4 weeks and 6 months.

OBJECTIVES: At present, the overall incidence of lung cancer is increasing, causing the age-adjusted lung cancer death rate to double every 15 years. Surgical resection of the tumor offers the best prospect of long-term survival in patients with lung cancer. Accurate prediction of postoperative ventilatory function should be helpful in determining in which patients the risk of surgery are acceptable. Exercise pulmonary function test has been widely applied to objective measure of work capacity, and can be extremely helpful for investigating exertional dyspnea. But no attempt is made to assess the contribution of the lung to be resected to performance, and it has been advocated as an additional predictor of postoperative complications. The present investigation was designed to evaluate the factors such as FEV1, FVC, VEmax, Vo2max, anaerobic threshold, heart rate reserve, breathing reserve and the corelation between FEV1 and VO2max at 4 weeks and 6months after resection in patients with lung cancer. METHODS: The eighteen of patients with lung cancer who considered surgically resectable underwent progre ssively incremental exercise test to symptom-limited stage before opera-tion, and also have done at surgical resection after 4 weeks, and 6 momths. Measurements were made of metabolic, cardiorespiratory, blood gases and symptoms during exercise test. RESULTS: 1) There were significant decreases in FEV1, FVC, VO2max, VE max in 4 weeks after operation and were more decreased in FEV1, FVC, VE max with significance, but fall in VO2max without significance in 6 month after operation. 2) There was a significant corre lation between the decrease of FEV1 and that of VO2max in 6 month after operation . Conclusions This result suggests that change of FEV1 is a relatively useful predictor of change in exercise performance after lung resection in 6 month after operation.

10.Assessment of Allergic Rhinitis Websites in Korea.

OBJECTIVES: The internet has become an important source of medical information and a great amount of information related to allergic rhinitis (AR) is available on the internet. However, the quality of this information is still a matter of debate. Therefore, this study was conducted to assess the AR-related information on Korean websites. METHODS: The key word "allergic rhinitis" was entered into 4 popular search engines, and this led to identifying 40 websites. After being categorized according to authorship, the informational value of these websites was evaluated using 4 different assessment tools such as the Journal of the American Medical Association (JAMA) benchmarks, the DISCERN questionnaire, the Allergic Rhinitis and its Impact on Asthma (ARIA) 2008 Update and the Health On the Net (HON) code. RESULTS: The 40 websites containing AR-related information were categorized according to their authorship as Western physician: 20, Oriental physician: 14, commercial: 1, and others: 5. The mean citation frequencies of the JAMA benchmarks and the ARIA 2008 Update concepts was 1.23 out of 4 and 4.33 out of 8, respectively, while the mean DISCERN score was 1.92 out of 5. When the websites were evaluated based on the type of authorship, the mean citation frequencies of the ARIA 2008 Update concepts were Western physician: 5.35, Oriental physician: 2.64. Additionally, three websites authored by Western physicians and 13 authored by Oriental physicians contained unreliable information. Among these 16 websites, only 3 websites met the requirements for the HON code "Justification". CONCLUSION: AR-related information available on Korean websites is of variable quality and not all of the information provided is justifiable. Thus, performing surveillance of the medical information on these websites is necessary. Furthermore, common criteria that can be used to evaluate the websites created by both Western and Oriental physicians are also needed.
