1.A Case of Esophageal Granular Cell Tumor.

Esophageal granular cell tumor of is rare neoplesin, and 3 cases were reported in Korea so far. We report a case of esophageal granular cell tumor in a 28 year old man. The patient was a 28 year old man who visited this hospital because of upper abdominal pain, acid belching and weight loss. A small nodule, 5 mm in diameter, was found in mid esophagus on esophagoscopy. This nodule was proved to be a granular cell tumor.

2.Genomic Differentiation Among Oyster Mushroom Cultivars Released in Korea by URP-PCR Fingerprinting.

URP primers of 20 mer derived from repetitive sequence of rice were used to assess genetic variation of oyster mushroom consisting of 10 cultivars of Pleurotus ostreatus, two cultivars of P. florida and two cultivars of P. sajor-caju which were registered in Korea. URP2F and URP38F primers produced cultivar-specific PCR polymorphic bands in the Pleurotus species. UPGMA cluster analysis using the URP-PCR data showed that 14 Pleurotus cultivars are genetically clustered into large three groups. The URP-PCR data provided important information for more efficient breeding strategies of Pleurotus cultivars.

3.A Case of Corneal Perforation and IOL Prolapsed in Serratia Marcescens Endophthalmitis after Cataract Surgery.

PURPOSE: To report a case of Serratia marcescens endophthalmitis following phacoemulsification and posterior chamber intraocular lens implantation. CASE SUMMARY: A 64-year-old male with a history of diabetes, hypertension, cardiovascular disease was referred with right ocular pain and reduced vision 1 day after cataract surgery. On admission, hypopyon on anterior chamber by slit lamp examination and anterior vitreous was hazy according to ultrasonography. We immediately performed intravitreal antibiotics injection and microbiological analysis of anterior chamber and vitreous samples was performed. On day 3, persistent cornea stormal infiltration and cornea perforation were visible and the organism was identified as S. marcescens. Despite appropriate antibiotic treatment the eye continued to deteriorate. CONCLUSIONS: S. marcescens endophthalmitis was diagnosed and treated immediately, but rapid progression and outcome were unsatisfactory.

4.Endobronchial Sarcomatoid Carcinoma of the Lung: A Case Report.

Sarcomatoid carcinomas of the lung are rare malignant biphasic tumors composed of carcinomatous and sarcomatous components. We report a case of endobronchial sarcomatoid carcinoma in a 56-year-old man with a history of smoking, in whom the tumor was an endobronchial mass arising from the superior segmental bronchus of the left lower lobe and protruding against the main bronchus without parenchymal invasion. The patient underwent left pneumonectomy but four months later died of mesenteric sarcomatous metastasis.

5.Size and Location of Ruptured Intracranial Aneurysms.

OBJECTIVE: The aim of study was to review our patient population to determine whether there is a critical aneurysm size at which the incidence of rupture increases and whether there is a correlation between aneurysm size and location. METHODS: We reviewed charts and radiological findings (computed tomography (CT) scans, angiograms, CT angiography, magnetic resonance angiography) for all patients operated on for intracranial aneurysms in our hospital between September 2002 and May 2004. Of the 336 aneurysms that were reviewed, measurements were obtained from angiograms for 239 ruptured aneurysms by a neuroradiologist at the time of diagnosis in our hospital. RESULTS: There were 115 male and 221 female patients assessed in this study. The locations of aneurysms were the middle cerebral artery (MCA, 61), anterior communicating artery (ACoA, 66), posterior communicating artery (PCoA, 52), the top of the basilar artery (15), internal carotid artery (ICA) including the cavernous portion (13), anterior choroidal artery (AChA, 7), A1 segment of the anterior cerebral artery (3), A2 segment of the anterior cerebral artery (11), posterior inferior cerebellar artery (PICA, 8), superior cerebellar artery (SCA, 2), P2 segment of the posterior cerebral artery (1), and the vertebral artery (2). The mean diameter of aneurysms was 5.47+/-2.536 mm in anterior cerebral artery (ACA), 6.84+/-3.941 mm in ICA, 7.09+/-3.652 mm in MCA and 6.21+/-3.697 mm in vertebrobasilar artery. The ACA aneurysms were smaller than the MCA aneurysms. Aneurysms less than 6 mm in diameter included 37 (60.65%) in patients with aneurysms in the MCA, 43 (65.15%) in patients with aneurysms in the ACoA and 29 (55.76%) in patients with aneurysms in the PCoA. CONCLUSION: Ruptured aneurysms in the ACA were smaller than those in the MCA. The most prevalent aneurysm size was 3-6 mm in the MCA (55.73%), 3-6 mm in the ACoA (57.57%) and 4-6 mm in the PCoA (42.30%). The more prevalent size of the aneurysm to treat may differ in accordance with the location of the aneurysm.

6.An Assessment of the Accuracy of 3 Dimensional Acquisition in F-18 fluorodeoxyglucose Brain PET Imaging.

PURPOSE: To assess the quantitative accuracy and the clinical utility of 3D volumetric PET imaging with FDG in brain studies, 24 patients with various neurological disorders were studied. MATERIALS AND METHODS: Each patient was injected with 370 MBq of 2-[18F]fluoro-2-deoxy-D-glucose. After a 30 min uptake period, the patients were imaged for 30 min in 2 dimensional acquisition (2D) and subsequently for 10 min in 3 dimensional acquisition imaging (3D) using a GE AdvanceTM PET system. The scatter corrected 3D (3D SC) and non scatter-corrected 3D images were compared with 2D images by applying ROIs on gray and white matter, lesion and contralateral normal areas. Measured and calculated attenuation correction methods for emission images were compared to get the maximum advantage of high sensitivity of 3D acquisition. RESULTS: When normalized to the contrast of 2D images, the contrasts of gray to white matter were 0.75+/-0.13 (3D) and 0.95+/-0.12 (3D SC). The contrasts of normal area to lesion were 0.83+/-0.05 (3D) and 0.96+/-0.05 (3D SC). Three nuclear medicine physicians judged 3D SC images to be superior to the 2D with regards to resolution and noise. Regional counts of calculated attenuation correction was not significantly different to that of measured attenuation correction. CONCLUSION:: 3D PET images with the scatter correction in FDG brain studies provide quantitatively and qualitatively similar images to 2D and can be utilized in a routine clinical setting to reduce scanning time and patient motion artifacts.

8.A Study on the Prevalence and Associated Factors of Falls in Some Rural Elderly.

BACKGROUNDS: It is to find out ins and outs of falls, and then to study risky factors that are preventable. METHODS: Total 260 elderly people living in Nonsan who are 60 years old and over were analyzed for data. Stereotyped of those are populational, social specific, physical functions, and basis diseases. And concerning falls, it was analyzed if they have had experience of up to 5 times within 3 years recently. RESULTS: The analyzed are total 260 people, men 112(43%), and women 148(57%). And the ones who answered they have had experience of falls are 108(41.6%), men 31(28.7%), and women 77(71.3%), showing a distinguishable difference between two groups. Women have had more experience than men have(p<0.001), and according to their age, the average age of those experienced falls showed to be higher than the one of those who have not(p=0.036). And according to their education, the ones who did not go to any schools have had more experience than those who did, showing the education has something to do with falls(p<0.001). The mean mass index(p=0.043) and average weight(p=0.023) also showed a mere difference between the group of the people who have had experience and the one of those who have not. Taking into account the fact that there were more women in the group who have had experienced falls, it is shown that there is nothing practical to pay attention to. In Activity of Daily Living(ADL) the experienced is 10.8(+/-4.2), and the unexperienced 9.1(+/-3.1), showing a mere difference between two groups(p<0.000). Concerning chronic diseases, there was no difference between those two groups, but individual patient histories showed that the experienced group had less rheumatic trouble than the unexpedenced(p=0.033). Looking at the difference of the causes for the falls according to their gender, the range of their movements(p=0.043), illumination(p=0.012), influence of alcoho1(p=0.001), and the shoes when they were falling down(p<0.001), the first aid treatments after falls(p=0.014), and influence of medications(p<0.001) showed a mere difference between men and women, while did not show any difference in seasons, time of falls, places, and the descriptions of the surfaces of the places. The mean number of falls within recent 3 years of the experienced was 1.79 times, and the comparison of the relation between the frequency and the causes of falls was done between the ones with experience of I fall, and the ones with experience of more twice falls. The average height(p=0.046) was much higher thai those with experience of more than 2 falls. There was also a mere difference between two groups in movements(p=0.009), illumination(p=0.005), influence of alcohol(p=0.003), the shoes (p=0.048), and first aid treatments after falls(p<0.001). CONCLUSION: The group with higher risk rate includes women, age, poor education, skinny physical figures, and lack of ability to act in daily life. Therefore, those results should be considered when a preventive program of falls for elderly people is designed.

9.Multimedia system for management and presentation of medical educational materials.

Medical education and research require various forms of information, which form multimedia data. While traditional computer can only deal with numeric and coded data, modern computer technology enabl es intergrated management of multimedia data. The present system was designed to perform convenient, acquisition, processing and analysis, and efficient presentation of medical multimedia data. APC-486 DX2 system epuipped with 8MB DRAM and 250 MB HDD provided fast enough computation speed, and image quality guaranteed by 17-inch SVGA color monitor. Both sound and image data are simultaneously acquired through a condenser microphone and audiocard, and respectively, in a video signal format. Printed pictures are digitized by a color scanner. Data transmission is also possible through a local area network (LAN). Acquired media data are structured in a hierarchy with the image data as apex. A variety of image processing and analysis algorithms were implemented and the media data can be reproduced simultaneously. Furthermore, simple and dynamic image animation can be performed to enhance data editing and authoring capability. Selected data sets can be reproduced in a user defined sequence, enabling "multimedia slide presentation". User interface was made in Windows format and all application programs were written in Borland C++ for future flexibility and extension. The present system will be of convenient use to acquire, manage, and present multimedia data for medical education and research.

10.An Insulin-induced, Endothelium-dependent Relaxation Mechanism of Normal Rabbit Cavernous Smooth Muscle.

Purpose: Insulin is known to induce relaxation in various vasculatures by increasing the release of nitric oxide or the expression of nitric oxide synthase. However, its action mechanisms on the corpus cavernosum remain to be uncovered. This study aimed to investigate the relaxative responses and the mechanism of normal cavernous smooth muscles to insulin. Methods and Materials: Rabbit corpus cavernous tissues were prepared in 2x2x8 mm sized strip for isotonic tension recording. The dose-dependent relaxation responses of norepinephrine (10(-4)M)-precontracted strips to insulin (10(-6)M)were measured. The relaxation responses of NE (10(-4)M)-precontracted strips to insulin (10(-6)M) were measured under low to high glucose concentrations (0, 0.1, 1 and 11mM) in physiological solution. The relaxation responses of the NE-precontracted strips to insulin (10(-6)M) were also observed after endothelial denudation, 30-minute preincubation with L-NAME (5mM) or 30-minute preincubation with indomethacin (10(-4)M). Results: The cavernosal strips were relaxed by insulin in a dose-dependent manner. The insulin-induced relaxation was dose-dependently increased by glucose. The endothelial denudation or indomethacin pretreatment almost abolished the insulin-induced relaxation, but L-NAME rarely affected the relaxation. Conclusions: Insulin induces an endothelium-dependent relaxation of rabbit cavernous smooth muscles, which is mostly NO-independent, but seems to be related with prostaglandins or their metabolites.
