2.Review of 28 Cases of Testicular Tumor.

We reviewed 28 pts. with testicular tumors admitted to the department of urology, Catholic medical college during last 12 yrs. The results are summarized as follows; 1. Age distribution ranged from 4 months to 59 years old, showing the highest incidence below 10 yrs old (39%). 2. There were 28 cases of testis tumor, and all cases were germ cell tumors which consisted of 9 cases of seminoma, 10 cases of teratoma, 5 cases of embryonal carcinoma, 2 cases of teratocarcinoma. 1 case of seminoma and embryonal carcinoma, and 1 case of choriocarcinoma and seminoma. 3. Serum HCG and AFP were evaluated in 13 cases. All seminoma and teratoma, 1 case of seminoma and embryonal ca. ranged within normal limit. But, among 4 cases of embryonal ca. 2 had elevated serum HCG, and 3 had elevated serum AFP. 1 case of teratocarcinoma and 1 case of seminoma and teratoma had elevated serum AFP and HCG. 4 The clinical stage was A in 21, B in 6, C in 3. 5. Radical orchiectomy was done in all cases, and retroperitoneal lymph node dissection was given for 9 cases. Radiation therapy and chemotherapy with PVB regimen were performed in 10 and 3 cases. 6. On 12 cases follow up study was made. Of them 3 (1 of teratocarcinoma, 1 of embryonal carcinoma, 1 of seminoma and embryonal carcinoma) in stage C were died at 3.3 yrs, 1.2 yrs and 15 days after orchiectomy. And 1 case(teratocarcinoma) in stage A were died at 1.5 yrs. after orchiectomy due to pulmonary metastasis. Remaining 8 cases are alive without evidence of relapse for mean follow up 39.7 months.

3.Strumal Carcinoid of the Ovary: Report of a case.

Strumal carcinoma of the ovary is rare tumor of germ cell origin characterized by intimate mixture of thyroid tissue and carcinoid. We report a strumal carcinoid tumor associated with ipsilateral follicular cyst and contralateral mature cystic teratoma of the ovary in a 47-year-old woman. Histologically it was composed largely of trabeculae partly of insular carcinoid and focally of individual thyroid follicles. Many cells in the carcinoid areas and even some follicular areas contained abundant argyrophile granules, and they also, in the corresponding areas, demonstrated thyroglobulin. Electron microscopic findings revealed abundant neurosecretory granules and colloid material in the same cell. We agree that this tumor is derived from hybrid showing thyroid and neuroendocrine differenciation.

4.Result of Intraarticular Fracture of the Calcaneus by Open Reduction

The Fracture of the os calcis is quite common in relation to fracture in other tarsal bone, and fractures involving the subtalar joint may cause serious and persistant disabilities. The treatment of these calcaneal fractures remains controversial So there is no universal agreement in the treatment of these fractures, especially involving the subtalar joint. Twenty displaced intraarticular fractures of calcaneus in 15 patients were treated by open reduction and internal fixation with a plate and screws and bone graft through a lateral approach to elevate the depressed lateral portion of the posterior facet at the Inchon Chritian Hospital from Jan., 1990 to Dec, 1991. Twenty feet in 15 patients of calcaneal fractures involving the subtalal joint were treated and were clinically analysed and following result were obtained. 1. The 20 cases were classified according to the Sanders classification, 9 in type II, 9 in type III and 2 in type IV. 2. The 20 cases were treated by open reduction andinteranl fixation with plate and screws and iliac bone graft through an Sanders lateral approach. The bone graft is packed under the elevated posterior to prevent late collapse of the central fragment and to supply structral stability. 3. The preoperative average Böhler's angle was 0.4°. The post operative average Böhler's angle was 23.4° which was increased significantly. 4. Based on assessment of result of Salama, excellenwas 6, good was 7, fair was 4 and poor was 3. 5. The most common complication after treatment was persistant pain, especially beneath the lateral malleolus.

5.The Association between the Depression and the Perception of Obesity by Adolescent Girls.

BACKGROUND: This study was performed to find the association between the depression and perception of obesity by adolescent girls in Jung Eup City, Chollapukdo. The data were collected by questionnaire from July to October, 1996. RESULTS: 1. While the obesity rate of the girls by BMI criteria was 6.0%, 41.1% of girls perceived that they were obese. 2. Forty-eight percent of respondents tried weight control already, and as the method of weight control 33.3% tried exercise, 27.2% reduced food, 23.6% avoided snack before going to bed. 3. while 39.3% among those whose BMI was lower than 20 and 54.2% among those with BMI 20-24 tried weight control, only 5.1% among those who perceived their bodies as slim tried weight control. 4. By the analysis of self-rating depression scale (SDS) only 14.8% was normal without depression and the rest showed high SDS scale; 36.9% as mild, 38.5% as moderate, and 9.7% as severe state of depression. 5. While no statistically significant difference of SDS scores by the size of BMI was shown, significant different of SDS scores was shown among groups with different perception of their obesity and practice of weight control. 6. As a result of stepwise multiple regression analysis, dieting, perceived body image, and body mass index were significantly associated with depression of the adolescent girls. CONCLUSION: Adolescent girls who perceived themselves overweight regardless of actual weight were more likely to diet than other girls. And these characteristics had an influence on the depression of the adolescents. It is suggested that appropriate education should be required to prevent thoughtless weight control of the adolescents and to maintain mental health of the adolescents.

7.Kaposi's Sarcoma: A report of three cases.

The classic type of Kaposi's sarcoma, or multifocal hemorrhagic sarcoma histologically characterized by proliferating fibroblastic and microvascular elements was described by Kaposi as a relatively rare neoplasm. During the past nine years, we experienced three cases of sporadic, classic Kaposi's sarcomas. They were presented as multiple papules, macules and nodules on the skin of the hands, lower logs and feet without systemic involvement. Histologically, Kaposi's sarcoma is divided into three stages, early patch, plaque and nodular stages. The nodular lesions(case 1, 2 and 3) showed extensive proliferatiion of spindle shaped, somewhat pleomorphic cells having dark prominent nuclei, proliferation of small vessels with solid aggregates of endothelial cells, and extravasation of erythrocytes. In early patch stage(case 3), widely dilated, anastomosing, thin-walled vascular spaces are noted in the upper half of the dermis. In plaque stage(case I and 3), there are proliferation of spindle shaped cells with extravasated erythrocytes and aggregates of blood vessels lined by prominent endothelial cells.

Retroperitoneal liposarcoma comprises 12 to 45 percent of all liposarcoma. It has been a therapeutic dilemma and a less prognosis because of its tendency to attain a tremendous size due to early silent growth and difficult wide en bloc excision in the retroperitoneum. Here in we report a case of retroperitoneal liposarcoma in 51 year old male patient with a brief review of the literatures.

9.Parallel Technique of Endobronchial Balloon Catheter Tamponade for Transient Alleviation of Massive Hemoptysis.

Endobronchial balloon tamponade is an alternative method that can be used to control massive hemoptysis. Several different techniques have been used for this purpose. We describe a new parallel method, in which biopsy forceps introduced through a bronchoscope channel is used to grasp a balloon catheter. As the bronchoscope is advanced to the bleeding site, the balloon catheter is pulled into position, and subsequently inflated. There are several advantages of this technique. It needs no specialized catheter or guide wire, the procedure is relatively easy to perform, and applicable to other purposes such as introduction of an additional suction catheter.
