A group of 180 men who visited Urology Department of Severance hospital, including 115 patients with nongonococcal urethritis (N.G.U.), 27 patients with prostatitis, 13 patients with gonococcal urethritis (G.U.) and 25 healthy medical student controls were investigated for the isolation of Ureaplasma urealyticum (T-strain mycoplasma) from the specimen of urethral discharge, urine and semen. Taylor-Pobinson media of T-broth and T-agar was used for the isolation of Ureaplasma urealyticum. To the best of our knowledge, the study on the culture of Ureaplasma urealyticum was reported for the first time in Korea. The following results were obtained: 1. The isolation rate of Ureaplasma urealyticum in nongonococcal urethritis (53.0%) revealed highest of those in the other three groups of prostatitis, gonococcal urethritis and control (40.7%, 38.4% and 16.0% respectively). 2. As for the specimens, urethral discharge revealed higher isolation rate of Ureaplasma urealyticum (54.6%)than first voided urine (50.0%). 3. The more consorts patients had, the higher positive culture rate of Ureaplasma urealyticum were revealed. The isolation rate in case of more than one causal in nongonococcal urethritis (71.1%) revealed much higher than in case of marital only (42.8%), one regular (41.1%) and one causal (41.0%). 4. 18.7% of isolation rate of Ureaplasma urealyticum revealed in patients with nongonococcal urethritis who visited the clinic in later than 4 weeks after the symptoms developed. However, the isolation rate in patients who visited within 4 weeks revealed 58.9%. The lower isolation rate of Ureaplasma in the late treatment seekers might be probably due to the suppression effect against Ureaplasma urealyticum from the possible previous self antibiotic treatment. 5. Attachment of Ureaplasma urealyticum mostly to the neck and head portion of the spermatozoa seemed to play a role to affect the motility of sperms.

Tuberculous spine has high incidence in children. Many cases of the tuberculous spine are complicated by deformities of the spine and disability such as paraplegia, cardio-plumonary dysfunction, and also early death. There are now several reports on the pulmonary dysfunction due to spinal deformities such as scoliosis and kyphoscoliosis, but there are few papers on the pulmonary function of patients with the tuberculous spine and kyphosis in children. This article is a report on the study of chest excursion in 70 kyphotic children and of the pulmonary function in 10 cases of severe kyphotic patients with collapsing tuberculous spine. The following results are obtained through the study: 1) Chest excursion was evidently diminished in the cases of moderate and severe thoracic, and severe lumbar involvement. The remainders were nearly within normal limits. 2) The study revealed that the chest excursion and the radiological kyphosis has a very gradually sloped negative correlation in thoracic involvement. 3) Pulmonary function in the severe kyphotics who had the curve over 50 degree were revealed as follows; a) Over 50 percent of the cases had diminished vital capacity. That is, over-all average was 67.7 percent of normal capacity. Especially inspiratory reserve volume was diminished, it was 63.7 percent of normal. Maximum breathing capacity was 68.8 percent of normal. b) Tidal volume, timed vital capacity, minute ventilation rate and O2 consumption were within normal limits or nearly normal.

Tuberculosis of the spine still remains as an important problem and also have high incidence in children in Korea. Many cases of the tuberculous spine are complicated by deformities of the spine and disability such as paraplegia, incordination of the cardio-pulmonary function and also result in early death. For the treatment of the tuberculous spondylitis following measure should be considered. First, eradication of the tuberculous lesion by conservative treatment or combined surgery, secondly, prevention of the deformity of the spine. Lastly, care for physical and psycological or emotional changes, and care for cardio-pulmonary dysfunction which develops after establishment of spinal deformity. Authors also attempted to analyse the kyphotic changes of the tuberculous spine of 70 children who were ambulating treatment at Pusan University Hospital and Pusan Charity Hospital, and obtained the result as follows; 1. Lower thoracic and upper lumbar were mostly involved. Nearly all cases of the new involvement of adjacent vertebra within 18 months. Only 4 cases now involvement occured after 18 months. 3. Kyphosis aggrevated grossly in 61.3% of cases. Most of them were apparent within 18 months but the changes developed after 18 months in 4.23% of cases. 4. In measuring the radiological kyphosis, Salters angle usually more simpler one than the internal gibbus angle. 5. Salters angle was larger involved than the internal gibbus angle except in mild and severe cases of the lumbar and lumbosacral involvement. Increase of kyphosis was 13.6 degree by internal gibbus angle and 16.3% by Salters angle. Kyphosis increased after 18 months were 3.1 degree by internal gibbus angle and 5.0 degree by Salters angle. 6. Radiological kyphosis changed in all stages and at least we should consider the changes of the kyphosis could be measured. 7. By the Kawakamis spinometer, the compensatory curvature, the posture of the patient and state of the kyphosis could be measured. 8. The compensatory curvature measured by Kawakamis spinometer were as follows: a) compensatory curvature in decrement in mild or morderate degree of cases, changed lumbar only or both cervical and lumbar curvature. b) compensatory curvature in increment in severe thoracic involvement changed both cervical and lumbar curvature, or cervical one only, the value in the cervical one was larger than that in the lumbar curvature.

Respiratory distress syndrome (RDS) of newborn is a disease revealed high morbidity and mortality rate, especially in premature infant. To evaluate the predictive value of Stable Microbubble Rating (SMR) and Shake test on RDS in premature infant, the anthors carried out the gastric aspirates Shake test and SMR test at birth, 3 and 6 hours after birth respectively on 124 premature infants who were born at the department of Pediatrics, Dong-San Hospital, Keimyung University during the period of 6 months form June 1993 to November 1993. The following results were obtained: 1) Among the 124 premature infants, RDS was developed on 23 (18.5%). 2) The birth weight, gestational age and Apgar score were significantly lowered in RDS cases than non-RDS cases (p<0.005). 3) The sensitivity, specificty, positive-and negative-predictive values of Shake test were 87.0% 79. 2% 48.8% and 96.4%, and those of SMR test were 100% 93.1% 76.7% 100% at birth, 95.7% 88.1% 64.7% 98.9% at 3rd-hour of life, and 95.7% 72.3% 44% 98.6% at 6th-hour of life. With the results of this study we concluded that SMR test at 6th-hour of life.

Sebaceous epithelioma is an uncommon and primary neoplasm of sebaceous gland. We report herein a case of sebaceous epithelioma in an 85-year-old female. Physical examination showed a solitary, 2.5 x 3.5cm sized, reddish. irregularly surfaced, and well defined tumor on the intercanthal area. Histopathological findings showed tumor parenchyma. romprised of several large nests of three kinds of epithelial cells, mature sebaceous cells, undifferentiated cells arranged in pa1isade fashion at the periphery of a cell nest, and transitional cells showing beginning fatty vacuolizatiorn of the cytoplasm. Electron micocopy revealed immature dark cells, transitional cells, and mature sebaceous cell which showed many scattered foci of sebaceous ditferentiation characterized by intracytoplasmic lipid droplets and glycogen particls.

Cysts of the kidney from a unique and important disease in urology. Although many of them are benign, a few have a tendency to become malignant. Especially the solitary cysts of the kidney are fairly frequent abdominal tumors. So the differential diagnosis between solitary cysts and other malignancies of the kidney is very important, and the surgeon should always hold in reserve a diagnosis of malignancy until the tumor is seen and examined in situ because this solitary cysts occasionally are associated with malignant disease of the kidney. Recently we have experienced two cases of the renal solitary cysts in 63-year old female patient and 52-year old male patient and we present two cases of the renal solitary cysts with the review of the literature on this paper.

The possible role of Chlamydia trachomatis in non-gonococcal urethritis and chronic prostatitis was investigated by cultural technique in a study of 281 and 215 respectively. Chlamydia trachomatis was isolated from 106(37.7%) of 281 non-gonococcal urethritis patients and 49(22.8%) of 215 chronic prostatitis patients. The effectiveness of tetracycline hydrochloride and doxycycline was evaluated by reculture method after 2 weeks` treatment. Thus in this study Chlamydia trachomatis could appear to play a major etiological roles in non-gonococcal urethritis and chronic prostatitis in Korea. And 2 weeks prescription of tetracycline or doxycycline is efficient for eradication of Chlamydia trachomatis.

The causes of pediatric urolithiasis are urinary tract infection as a result of urinary stasis, metabolic diseases and idiopathic origin. Geographically the incidence is higher in Asia than in any other parts of the world. Most recently, PNL was started to be applied not only for adult cases but also for pediatric urolithiasis. Our group was successful in treating two pediatric urolithiasis patients of 4 and 6 years of age by endourologic technique. In 4 year old male patient, renal stone and lower ureter stone were combined and severe hydronephrosis were present. PNL and URS were done simultaneously for this case. In 5 year old male patient, renal pelvis stone was treated by PNL. The compositions of the stones were calcium phosphate and calcium oxalate, respectively. The postoperative hospitalization was 5 days in both cases and they were discharged without any problem. In both cases, follow up IVP`s were done. We are reporting these two cases to two the successful treatment with adult endoscopes.
