1.The Effect of Anteriorization of The Inferior Oblique Muscle in +3 or +4 Inferior Oblique Overaction.

The author performed anterior transposition of the inferoir oblique muscle to a point 1mm posterior to the lateral end of the inferior rectus insertion in 36 eyes of 36 cases of +3 inferior oblique overaction, and to the lateral end of the inferior rectus insertion in eight eyes of five cases of +4 inferior oblique overaction. Postoperatively, the eyes that have normal action of the inferior oblique were 36 eyes(81.8%). Only one eye was overcorrected and four eyes were undercorrected. The overcorrected one case developed a hypotropia in primary position. It appears that this anterior transposition of the inferior oblique muscle is effective weakening method of the inferior oblique muscle for the treatment of +3 to +4 overaction of inferior oblique muscle.
2.Anesthetic Experience with Two Cases of Congenital Diaphragmatic Hernia .

The authors have experienced two cases of general anesthesia for repair of congenital diaphragmatic hernia. Both patients were female (1 year 5 months and 8 months of age) and had left sided Bochdalek hernia. Anesthesia was maintained with halothane-oxygen using nonrebreathing system with Ruben valve, and the patients tolerated the whole procedure very well. The importance of maintaining adequate ventilation with high concentrations of oxygen and avoiding abdominal distension has been stressed.

3.Infestation of larval trematodes from fresh-water fish and brackish-water fish in river Taechong, Kyungpook province, Korea.

: A study of infestation patterns for larval trematodes from fresh-water fish and brackish-water fish was carried out during the period from June to October in 1982. They were collected by netting, fishing with rod and line, and using the bait of crushed oil cake in a transparent plastic bowls at the three localities, Daebon-dong, Kugil-dong, and Hoam-dong in the river Taechong, Kyungpook Province, Korea. Five species of fresh-water fish, the crussian carp, Carassius carassius Linnaeus, the bullhead, Coreobagrus brevicorpus Mori, the fat minnow, Moroco oxycephalus (Bleeker), the flat bitterling, Paracheilognathus rhombea(Temminck et Schlegel), and the catfish, Parasilurus asotus (Linnaeus), and two kinds of brackish-water fish, the grey mullet, Mugil cephalus (Linnaeus) and the sweetfish, Plecoglossus altivelis (Temminck et Schlegel) were examined. Four species of the metacercariae, Exorchis oviformis, Metacercari hasegawai, Metorchis orientalis and undetermined larvae were found. Of these, the encysted larvae of the human intestinal fluke, Metagonimus yokogawai, were found from three kinds of fresh-water fish, the crussian carp, the bullhead and the fat minnow and one species of brackish-water fish, the sweetfish and the infestation rates for the larvae varied appreciably from fish to fish. The intensity of infestation with the Metagonimus larvae in the three species of fresh-water fish as low, and the average number of the larvae per gram of flesh varied from 0.5 to 0.2. By comparison, the brackish-water fish was more heavily infested and the mean number of cysts per gram of flesh in Plecoglossus altivelis was 109.2. It was found that infection with Metagonimus yokogawai is caused by consuming raw fresh-water fish and brackish-water fish caught in the river Taechong, Kyungpook Province, Korea.

4.A Case of Delayed Onset Tirofiban Induced Thrombocytopenia in a Chronic Renal Failure Patient.

Compared with heparin induced thrombocytopenia (HIT), glycoprotein (GP) IIb/IIIa inhibitor induced thrombocytopenia is characterized by a rapid and profound fall in the platelet count. In severe cases, the platelet count decreases below 20,000/mm3 within the initial 24 hours after exposure to the drugs. It is often associated with severe bleeding complications. Tirofiban is one of the GP IIb/IIIa inhibitors. A case of severe thrombocytopenia in a 60-year-old man, with chronic renal failure (CRF) and acute myocardial infarction (AMI), was recently experienced. Tirofiban was administered to treat the AMI without ST segment elevation. The platelet count fell to 1,000/mm3 with hemoptysis and petechiae 80 hours after the initiation of tirofiban infusion. In this case, the clinical course was similar to that of the typical GP IIb/IIIa inhibitors induced thrombocytopenia, with the exception that its onset time was unusually delayed. The platelet count was normalized in 9 days after cessation of tirofiban infusion. Thus, this unusual case is reported as delayed onset tirofiban induced thrombocytopenia in a CRF patient.

5.Effect of Snake Venom: Arginine-esterase on the Fibrinolytic Activity.

BACKGROUND AND PURPOSE: Arginine esterase(Ancrod), a thrombin-like enzyme, purified from the venoms of Agkistrodon halys, has known to cleave fibrinopeptide A from the fibrinogen and lead to circulation of soluble noncross-linked "ancrodfibrin', which stimulates endogenous T-PA release.Many authors have suggested clinical applicability of this enzyme,but clinical studies on its fibrinolytic action has been insufficient.Thus we studied the influence of this enzyme on fibrinolytic activity in cerebral infarction. METHOD: We observed the change of euglobulin fibrinolytic activity, t-PA antigen, t-PA activity, fibrinopeptide A, fibrinogen, FDP and D-dimer, during 12 hours after a bolus intravenous administration of 0.25 unit of the arginine esterase to the 9 patients with cerebral infarction. RESULT:There was no change of the euglobulin fibrinolytic activity, fibrinopeptide A and t-PA Ag but there was significant increase in both t-PA activity and FDP, D-dimer and significant decrease in fibrinogen. CONCLUSION: Our result suggest that arginine esterase converts fibrinogen to a fibrin polymer which has a increased susceptibility to lysis by plasmirl This enzyme seems to amplify T-PA activity through the consequent increase in FT)P, because there is no increase in the euglobulin fibrinolytic activity, fibr'mopeptide A and t-PA Ag suggesting direct T-PA release. Arginine esterase, having action of effective defibrinogenation and safe fibrinolysis,could be used for the thrombus related disease.

6.A Clinical Study on 212 Case of Pelviscopic Surgery.

Two-hundred and twelve cases of pelviscopic operation were performed at Pusan St. Benedict Hospital from November, 1992 to December, 1995. The results were as follows; The common indications were ovarian cyst(32.2%), ectopic pregnancy(16.9%), and endometriosis(14.7%). Frequent types of surgery were salpingo-oophorectomy(22.6%), salpingectomy(10.8%), and oophorectomy(10.0%). The mean age of the patients was 33.4 years. The mean parity of the patients was 1.0. The mean duration of hospitalization was 4.8 days. The most common complication was hemorrhage at the trocar sites. In our conclusions, pelviscopic surgery made minimal postoperative hemorrhage with postoperative discomfort, low incidence of complications, and short duration of hospitalization. Pelviscopic surgery was useful and recommended for the treatment of gynecologic diseases.

7.A Case Report of Delayed Hemolytic Transfusion Reaction after Administration of Apparently Compatible Blood .

Hemolytic transfusion reactions may occur after the administration of donor bloods even when they have been shown compatible apparently by crossmatch tests. Such episodes present a diagnostic challenge and raise serious doubts about our understanding of blood group incompatibility. Fever, hemoglobinuria, oliguria and marked fall in hematocrit values developed in a patient 30 hours after she had received one unit of apparently compatible blood during operation under general anesthesia. Results of routine serologic studies at the time of the transfusion reaction were normal. She died on 40 hours after operation.

MIN GLASSES is specially manufactured in order to enhance satisfaction of wearer and therefore increase the effectiveness of treatment of an amblyopia and a diplopia. One of the lens is specially grounded and coated to blur vision. Cosmetically, the eye appears to wear regular glasses through the blur lens. We prospectively studied 58 amblyopic children and 10 diplopic adults. Each patient was treated with MIN GLASSES for cosmetic problems. We checked linear visual acuity of pre- and post-treatment. Questionnaires were given to the patients and parents regarding cosmetic satisfaction with treatment. In amblyopic children. 44 patients(75.9%) showed visual acuity improvement of 1.61 +/- 1.50 lines by Han's visual acuity chart. Patient compliance was 87.9%(51 patients) with patient satisfaction based on a questionnaire response. Parents were 87.9%(51 parents) positive response. In diplopic adults, 10 patients(100%) were positive response with satisfaction and demonstrated to be free of symptom of diplopia. MIN GLASSES is effective in high compliance satisfaction of amblyopia and diplopia, and improvement of visual acuity of amblyopia.
